Keloid scars result from excess collagen deposition in the dermis or subcutaneous tissue in response to surgery or tissue trauma. The penis is a rare site for keloid formation, even in predisposed individuals. Only few cases of penile keloids have been reported so far. In this report, we present penile keloids complicating neonatal circumcision in twin brothers. Risk of recurrence in previous reports and measures to guard against its occurrence are also discussed.

Keloidal morphea is a rare variant of scleroderma, which often can be clinically confused with keloid or scar formation. We report a 34-year-old woman with a medical history of asthma and Raynaud's phenomenon, presented for the evaluation and management of multiple erythematous hyperpigmented annular plaques reportedly developed after taking trimethoprim/sulfamethoxazole. An initial skin biopsy showed findings supportive of a drug eruption. She was treated with oral prednisone and achieved some improvement. She presented 1 year later with enlargement of the plaques and emergence of new lesions…

Most of the widely used scales to evaluate scars are subjective relying on clinical observations. There is a growing need to find out a noninvasive objective tool for this purpose. The study is aimed at evaluating the value of the high-resolution ultrasound in the assessment of the scars when compared with a clinical evaluation scoring system, the Vancouver Scar Scale (VSS). The study included 22 patients with hypertrophic scars or keloids. At baseline, scars were assessed using the Vancouver Scar Scale and high-resolution ultrasound (13-MHz probe). Patients received three Nd:YAG laser session…

Auricular keloids are common following ear piercing, infection, trauma, burns, or spontaneously, and they are highly resistant for treatment and are followed by severe cosmetic problems, especially for patients with bulky auricular keloids. The risk of recurrence and the need to return to the normal anatomy of external ear following resection is a challenge to the plastic surgeon. The authors present their experience of treating bulky auricular keloids with surgical excision, followed by immediate postoperative radiotherapy and intralesional steroid injection.

The aim of this study was to evaluate the effect of intralesional tamoxifen on keloids, particularly on the concentration of fibroblasts, dermal inflammatory infiltrate, and collagen degeneration. A prospective study was carried out to evaluate keloids in 13 patients of both genders pre- and post-treatment with intralesional tamoxifen. Two samples of keloid lesions were obtained by 4-mm punch biopsies during the study: the first at the time of diagnostic confirmation of keloid and the other eight weeks later at the end of intralesional tamoxifen treatment. The biopsy samples were placed in 10%…

Keloid and hypertrophic scars are difficult to manage and remain a therapeutic challenge. Verapamil has shown a great potential in the management of keloid and hypertrophic scars. Comparing with conventional corticosteroid injections, verapamil could improve the appearance of keloid and hypertrophic scars, and is associated with a lower incidence of adverse effects. Is verapamil an effective alternative modality in the prevention and treatment of keloid and hypertrophic scars? The aim of this study was to assess the effectiveness of verapamil in preventing and treating keloid and hypertrophic …

OBJECTIVE: The mechanisms of keloid invasion are largely unknown. This study aims to analyze the differentially expressed genes between keloid peripheral and central areas and thus to define the molecule that might be responsible for keloid invasion.
METHODS: The gene chip of transforming growth factor-β (TGF-β) superfamily signaling pathway was used to analyze differentially expressed genes of the fibroblasts derived from peripheral area and central area of 3 keloids. The differential expression of growth differentiation factor-9 (GDF-9) was also confirmed by quantitative PCR and Western-blo…

As acute burn patients have experienced increasing survival rates, the number of patients who need specific care due to aberrant scarring is also increasing. The burned skin often responds with fibrotic tissue proliferation, which can lead to a hypertrophic scar or a keloid. Non-physiologic scars are mostly not acceptable for the burn patient. Intradermal and topical therapy in burns comprise the treatment of the skin injury and its possible texture, elasticity and color alterations with the aid of active substances that result in fibroblastic modulation. An alteration of cytokine levels may m…

Isotretinoin-induced keloid formation has occasionally been reported in patients who have undergone dermabrasion or laser treatment. This report describes a man with Behçet's disease (BD) who was prescribed isotretinoin for nodulocystic acne. After approximately 8 weeks of this treatment, the patient developed extensive keloids. There are no previous reports of possible isotretinoin-induced keloid formation in the setting of BD. This article discusses possible connections between BD, keloid formation and isotretinoin, which may be manifested in this case.

Keloid scarring presents a difficult therapeutic problem. It is a recognized sequel to acne vulgaris, and may be extensive and disfiguring. The data from this double-blind study, in which intralesional triamcinolone or cryosurgery were used as treatment for keloids, demonstrate that by treating early, and in particular vascular, keloids with the latter, 85% will show a moderate to good response in terms of flattening. Treatment with intralesional triamcinolone was also beneficial, but the response to cryosurgery was significantly better in early, vascular lesions.

The healing of a deep surface wound in humans begins with the formation of granulation tissue and includes a marked microvascular regeneration, initially in an inflammatory milieu. The inevitable sequel is usually a hypertrophic scar or keloid in which there is significant microvascular occlusion. The occlusion begins in the granulation tissue and is the result of an excess of endothelial cells. Several other examples of fibroses contain significant microvascular occlusion. The evidence demonstrates that hypertrophic scars and keloids are hypoxic, undoubtedly due to the microvascular occlusion…

Molluscum contagiosum is an infectious disease of the skin, found primarily on the thighs and genitalia. It is a sexually transmitted disease of increasing prevalence. The traditional treatment of this condition is surgical ablation, although CO2 laser has been advocated recently as an alternative mode of therapy. Six patients with molluscum contagiosum were treated by either continuous or intermittent CO2 laser. Four of the six have developed severe keloid scars. Removal of lesions by cold knife, however, was not followed by keloid scars. In the same patient, areas affected by human papilloma…

Keloids are a refractory fibroproliferative disorder characterized by excessive extracellular matrix (ECM) deposition and invasive growth beyond the original wound boundaries, severely impairing patients' quality of life. Accumulating evidence highlights that metabolic reprogramming and mitochondrial dysfunction are important in keloid pathogenesis, particularly in regulating the biological behavior of keloid fibroblasts (KFBs). This narrative review synthesizes current research on the interplay between mitochondrial dysfunction and metabolic abnormalities in keloids. Mitochondrial structural …

Keloids are a complex type of scar tissue formed by exaggerated wound healing, characterised by the overgrowth of thick fibrous tissue beyond the original wound boundary. While the crucial relationship between keloid formation and the vascular system has been highlighted, conflicting findings have been reported regarding the characterisation of keloid vasculature. Here, we successfully characterised the detailed three-dimensional (3D) structure of vasculature in keloid tissues using tissue clearing methods combined with 3D imaging. First, we compared two optical tissue clearing methods, the cl…

Keloids are thickened raised scars that develop due to injury and grow beyond the boundaries of their original wound, mostly affecting individuals with skin of color. This review explores the use of energy-based devices to treat keloids, both using laser monotherapy and in combination with other drugs. Laser therapy alone has shown efficacy in treating keloids. Combination laser therapy has better keloid reduction when administered with steroids, 5-fluorouracil (5-FU), and verapamil. However, monotherapy has had less adverse reactions including dermal atrophy and local pain. Therefore, physici…

Keloid infections reduce patient-reported quality of life greatly. Characteristics and risk factors of keloid infections have not been thoroughly studied. So, a retrospective cohort study was conducted focusing on the potential risk factors, microbiologic cultures and histological findings. Keloid patients consulting for surgical interventions were included in this study. Data were collected from their electronic medical records. 564 patients were recruited with the keloid infection rate being 22.4%. For adult patients, age above 40 years (OR, 2.84; P = .000), disease duration of 12 years or m…
