Herpetic keratitis after corneal crosslinking.

Corneal crosslinking is considered a safe procedure even if sight-threatening side effects have been reported. Other han our case report, few reports of herpetic keratitis (HK) after crosslinking have been published. We report a new onset HK after corneal crosslinking for keratoconus. A 40-year-old woman underwent corneal collagen crosslinking in her right eye for progressive keratoconus. The patient returned with pain and photophobia. Infiltration in the corneal stroma was observed. Fluorescein staining specified epithelial defects overlying the corneal infiltrates. The diagnostic of HK was c…

Biologics in peripheral ulcerative keratitis.

Over the past two decades biologic therapies have seen a rapid uptake in the management of ocular inflammation. Peripheral ulcerative keratitis (PUK), once a harbinger of blindness and mortality in refractory rheumatological disease, is now increasingly being treated with these agents. We conducted a review to evaluate the evidence base for this application and to provide a road map for their clinical usage in PUK, including dosage and adverse effects. A literature search across Medline, Embase and Cochrane Database of Systematic Reviews was undertaken to identify all patients with PUK that we…

Toxic non-inflammatory fungal keratitis.

PURPOSE: To report a new entity called "toxic non-inflammatory fungal keratitis." METHODS: Eyes manifesting infective keratitis with a history of prior administration of topical steroids were included in the study. The details pertaining to the type of injury, duration of injury, and primary treatment for corneal trauma were meticulously documented. The corneal tissues were scraped from the patients and were analyzed for fungal filaments by using a 10% KOH mount under a compound microscope. Moreover, these scraped materials were plated on blood agar and Sabouraud dextrose agar plates. RESULT…

Pythium insidiosum keratitis - A review.

Pythium insidiosum is an oomycete and is also called "parafungus" as it closely mimics fungal keratitis. The last decade saw an unprecedented surge in Pythium keratitis cases, especially from Asia and India, probably due to growing research on the microorganism and improved diagnostic and treatment modalities. The clinical features such as subepithelial infiltrate, cotton wool-like fluffy stromal infiltrate, satellite lesions, corneal perforation, endoexudates, and anterior chamber hypopyon closely resemble fungus. The classical clinical features of Pythium that distinguish it from other micro…

Peripheral Ulcerative Keratitis: A Review.

Peripheral ulcerative keratitis (PUK) is a rare but serious ocular condition that is an important clinical entity due to its ophthalmological and systemic implications. It is characterized by progressive peripheral corneal stromal thinning with an associated epithelial defect and can be associated with an underlying local or systemic pro-inflammatory condition, or present in an idiopathic form (Mooren ulcer). Associated conditions include autoimmune diseases, systemic and ocular infections, dermatologic diseases, and ocular surgery. Cell-mediated and auto-antibody-mediated immune responses hav…

Biology of peripheral ulcerative keratitis.

Peripheral ulcerative keratitis (PUK) is a progressive peripheral thinning of the corneal stroma caused by proinflammatory mediators' release from corneal limbal vasculitis. The clinical presentation is an epithelial defect with a crescent-shaped stromal inflammation. Its exact pathophysiologic mechanisms of PUK remain partially understood, but the overall understanding of the fundamental processes that mediate and effect corneal immunity has continued to expand over the past 25 years. The unique anatomical and physiological characteristics of the periphery in relation to collagen bundles and …

[Necrotizing scleritis after acanthamoeba keratitis].

Nodular scleritis and necrotizing scleritis are rare complications of acanthamoeba keratitis. This article presents the case of a 61-year-old female patient who had suffered from persistent keratitis in the right eye for more than 4 months. The patient was initially treated with propamidine isethionate and polyhexamethylene biguanide eye drops. A penetrating limbo-keratoplasty was performed. Examination of the corneal explant showed acanthamoeba cysts. In the following 5 months the sclera showed recurrent abscesses. A total of two thermal cauterizations and three amniotic membrane grafts were …

[Surgical management of infectious keratitis].

Infectious keratitis is one of the most common causes of blindness worldwide. Despite the existence of a wide arsenal of quite effective antimicrobial drugs, some forms of bacterial and viral keratitis are resistant. Advanced acanthamoeba and mycotic lesions of the cornea, as well as mixed forms of infection usually do not respond well to conservative treatment. In the absence of positive dynamics from the applied etiotropic therapy with observed further progression of the microbial process, there is a risk of corneal perforation and spread of infection to the sclera or deep ocular structures …

Alternaria keratitis after corneal crosslinking.

PURPOSE: To report a case of fungal keratitis after corneal collagen crosslinking (CXL) surgery. OBSERVATIONS: We report a case of fungal keratitis after CXL for post-refractive surgery ectasia. The patient presented 12 days after surgery with a corneal ulcer that was culture positive for Alternaria species of fungus. She subsequently developed a bacterial superinfection. The keratitis resolved with medical therapy, although the patient required a penetrating keratoplasty (PKP) due to central corneal scarring. CONCLUSIONS AND IMPORTANCE: To our knowledge, this is the first case of fungal ker…

Pseudomonas aeruginosa and microbial keratitis.

Pseudomonas aeruginosa, a versatile Gram-negative pathogen that can cause a wide range of infections, is the most common causative agent in cases of bacterial keratitis associated with contact-lens use. Corneal infections with P. aeruginosa often have poor clinical outcomes and can result in long and costly treatments. During the infection process, the pathogen exploits its large genome, encoding complex regulatory networks and a wide range of virulence factors, including motility and the secretion of various proteases and toxins. Although antibiotic resistance levels in the UK are low, higher…

Pharmacologic therapy of mycotic keratitis.

Mycotic keratitis continues to be an important cause of corneal blindness, especially in tropical and subtropical countries. The prognosis is poor compared with many other forms of keratitis because of the lack of effective antifungal drugs. The currently available antifungal drugs suffer from multiple drawbacks such as poor ocular penetration, unpredictable bioavailability, and adverse effects associated with systemic medications. Over the last decade, several new drugs and drug-delivery systems have been introduced in an attempt to improve the treatment outcomes. Thorough knowledge of the cu…

[Fungal keratitis after penetrating keratoplasty].

The article presents a case of fungal keratitis after penetrating keratoplasty (PKP). A 35-years old patient had previously undergone surgical removal of an intraocular foreign body and developed chronic keratouveitis and epithelial-endothelial corneal dystrophy. PKP and early postoperative period were uneventful. The patient was frequently monitored with anterior segment photos. Two weeks later, fungal keratitis occurred on the edge of the donor flap. Direct microscopic evaluation revealed fungus spores and filaments, but growth of fungi in culture was negative. Instillation of steroids and a…

Interstitial keratitis diagnosis and treatment.

Interstitial keratitis is a non-ulcerative, non-suppurative, more or less vascularized inflammation of the corneal stroma. The corneal lesions result from the host response to bacterial, viral (40% of cases) or parasitic antigens, or from an autoimmune response (1% of cases) without active corneal infection. The natural history of the disease is divided into two phases: acute and cicatricial. This type of keratitis is less common than ulcerative bacterial keratitis, but it is a non-negligible cause of visual loss. It is associated with systemic or infectious disease and requires early diagnosi…
