A new technique for complete intraluminal repair of iatrogenic posterior tracheal lacerations.

Tracheal laceration is a rare complication of endotracheal intubation. Early surgical treatment is mandatory in cases of pneumomediastinum w … th an endoscopic needle holder to allow for complete intraluminal repair of posterior tracheal wall lacerations. Four patients were admitted with an iatrogenic tracheal laceration due to emergency intubation. In all cases, the repair of the tracheal laceration started with the introduction of a 14-mm rigid tracheoscope and subsequent jet-ventilation. Three of …

The rate of upper-extremity deep-structure injuries through small penetrating lacerations.

Despite the abundance of epidemiologic studies concerning hand injuries, there is no study that emphasizes the significance of the initial laceration size. The aim of this study is to investigate the incidence of tendon, nerve, and artery injuries that may result from small, penetrating lacerations of the hand and forearm. A total of 226 patients with small lacerations were included in the study. Glass (68.7%) and knife (31.3%) lacerations were the only etiological factors. One hundred thirty-four patients (59.3%) had at least 1 deep-structure injury. One hundred twenty-four patients (92.5%) h…

A modified surgical technique for repairing third-degree perineal lacerations in mares.

A modified surgical technique has been developed for repairing third-degree perineal lacerations in mares. Complications of the currently used methods include rectovaginal fistula formation, urine pooling, complete dehiscence of the repair, constipation, tenesmus and difficulty of performance in the practice. The modified method is simpler and more practical. This method was performed on eight Thoroughbred mares with third-degree perineal lacerations after delivery. The rectovestibular septum was reconstructed by three lines of sutures in a transverse direction in relation to the longitudinal …

Results of treatment of the extensive volar wrist lacerations: 'the spaghetti wrist'.

'Spaghetti wrist' describes a volar wrist laceration in which at least 10 structures, inclusive of tendons, at least one major nerve and usually one major vessel are divided. This retrospective study, was performed between January 1990 and December 1995 at the Department of Orthopaedic and Traumatology of Erciyes University Medical School. The outcome of 21 patients who sustained simultaneous laceration of the median and ulnar nerves as well as flexor tendons at the wrist is described. The mean follow-up was 6.4 years. The flexor tendons were assessed using criteria from Lister et al. Vascular

A prospective trial of prophylactic povidone iodine in lacerations of the hand.

A series of 418 patients with lacerations of the hands were allocated randomly to a control group or to a group where the injury was treated with povidone iodine before suture. The incidence of infected and imperfectly healed wounds was determined seven days later. As well as the effect of povidone iodine on infection, thirteen other factors were also analysed. The overall infection rate of 5.0% and the 38.5% imperfect healing rate were not significantly affected by povidone iodine treatment, although both were reduced. The figures of four other trials were combined with this trial and this sh…

Safety and effectiveness of nonoperative management in liver lacerations: a retrospective cohort study.

BACKGROUND: Liver trauma remains a major contributor to morbidity in patients with abdominal injuries. Although nonoperative management (NOM) is widely accepted in hemodynamically stable patients, the relationship between American Association for the Surgery of Trauma (AAST) injury grade, laboratory parameters, transfusion requirements, and the need for surgical intervention remains unclear. METHODS: This retrospective observational study included 53 patients with radiologically or intraoperatively confirmed liver lacerations treated at a tertiary surgical center between October 2024 and May …

Nonoperative Management of Pretibial Lacerations in a Nurse-Led Clinic: An Observational Study.

In our tertiary plastic surgery center, patients with wounds that will not be treated surgically, including complex pretibial wounds, that w … gnificant (R2 = 0.246, F(1,74) = 24.1, p < .005). In patients with conservatively managed pretibial lacerations, the total number of different dressings used (as a surrogate marker for wound complexity ) is predictive of the time the patient spends in follow-up and the total number of hospital contacts. These data may be used to help understand when conservat …

Post-intubation tracheal lacerations: Risk-stratification and treatment protocol according to morphological classification.

BACKGROUND: Post-intubation tracheal laceration (PITL) is a rare condition (0.005% of intubations). The treatment of choice has traditionally been surgical repair. Following our first report in 2010 of treatment protocol tailored to a risk-stratified morphological classification there is now clear evidence that conservative therapy represents the gold standard in the majority of patients. In this paper we aim to validate our risk-stratified treatment protocol through the largest ever reported series of patients. METHODS: This retrospective analysis is based on a prospectively collected series

Risk Factors for a False-Negative Examination in Complete Upper Extremity Nerve Lacerations.

Background: Many patients with complete nerve lacerations after upper extremity trauma have a documented normal peripheral nerve examination at the time of initial evaluation. The purpose of this study was to determine whether physician-, patient-, and injury-related factors increase the risk of false-negative nerve examinations. Methods: A statewide health information exchange was used to identify complete upper extremity nerve lacerations subsequently confirmed by surgical exploration at 1 pediatric and 2 adult level I trauma centers in a single city from January 2013 to January 2017. Charts…

Bicanalicular Lacerations: Clinical Characteristics and Surgical Outcomes With a New Bicanalicular Silicone Stent.

BACKGROUND: Bicanalicular lacerations are relatively rare and more of a surgical challenge in clinical practice. The purpose of this study is to evaluate the clinical characteristics and surgical outcomes of bicanalicular lacerations with a new bicanalicular silicone stent at a tertiary eye care center. METHODS: All patients who underwent bicanalicular lacerations repair from January 2013 and December 2018 were retrospectively reviewed. Data collected for each patient included patient demographics, affected sides, cause of injuries, the timing of management, associated ocular injuries, the ti…

Comparison of cosmetic outcomes of absorbable versus nonabsorbable sutures in pediatric facial lacerations.

OBJECTIVE: We sought to compare cosmetic outcomes, complication rates, and patient/caregiver satisfaction of absorbable versus nonabsorbable sutures in children. METHODS: Healthy patients, 1 to 18 years old, with facial lacerations 1 to 5 cm, were randomized to repair with fast-absorbing catgut (FAC) or nylon (NYL) sutures. Patients returned in 4 to 7 days and in 3 to 4 months, at which time photographs and caregiver surveys were completed. Unlike part I, all FAC sutures were permitted to absorb rather than be removed. Using a 100-mm visual analog scale (VAS), a noninferiority (NI) design was…

Lacerations to Zones VIII and IX: It Is Not Just a Tendon Injury.

Extensor tendon injuries are widely believed to be straightforward problems that are relatively simple to manage. However, these injuries can be complex and demand a thorough understanding of anatomy to achieve the best functional outcomes. When lacerations occur in the forearm as in Zones VIII and IX injury, the repair of the extensor tendon and muscle, and posterior interosseous nerve (PIN) is often challenging. A review of the literature shows little guidance and attention for these injuries. We present four patients with injuries to Zones VIII and IX as well as a review of surgical techniq…

Congenital absence of flexor digitorum superficialis: implications for assessment of little finger lacerations.

Absence of flexor digitorum superficialis function in the little finger is a relatively common congenital anomaly that can complicate assessment of little finger injuries. We reviewed the prevalence of unilateral and bilateral absence of superficialis function in the published literature. In appropriate studies identified (1352 people), the anomaly was unilateral in 92 individuals (6.8%) and bilateral in 81 (6.0%). If superficialis function is absent in one little finger, the probability of absence in the opposite hand is 0.64. If superficialis function is present, the probability of absence i

Extraperitoneal bladder rupture with severe lacerations of the urogenital diaphragm: a case report.

BACKGROUND: A 65-year woman injured by automobile had an open-book type pelvic fracture and extraperitoneal bladder rupture. CASE PRESENTATION: Non-surgical management was selected because of suspected minor leakage. A follow-up CT cystography on day 21 showed a large urinoma. In the sagittal reconstruction CT images, the bladder was displaced downwards. Surgery was performed. Surgical findings showed that the bladder neck and the urethra were not anchored because of a laceration in the urogenital diaphragm. The bladder base had descended to the inferior margin of the pubic bone and a lacerat

Does epidural analgesia affect the rate of spontaneous obstetric lacerations in normal births?

The precise relationship between epidural use and genital tract lacerations in normal childbirth is unclear. Data from a clinical trial on measures to lower genital tract trauma in vaginal birth were used for a secondary analysis. The goal was to assess whether epidurals affect the rate of spontaneous obstetric lacerations in normal vaginal births. Maternal characteristics and intrapartum variables were compared in women who did and did not use an epidural in labor, and also in those with and without any sutured lacerations following vaginal birth. Variables that were statistically different i…
