Video Versus Direct Laryngoscopy in Novice Intubators: A Systematic Review and Meta-Analysis.

Video laryngoscopy (VL) is increasingly used in airway management and has been shown to decrease the rate of failed intubation in certain clinical scenarios, such as difficult airways. Training novices in intubation techniques requires them to practice on living patients; however, this is less than ideal from a safety perspective given the increased risk of complications after multiple attempts or failed intubation by inexperienced trainees. One setting in which VL may be beneficial is in training, although whether these devices should be used among novices instead of direct laryngoscopy (DL) …

Comparing laryngeal view in neutral and sniff position during video laryngoscopy-guided intubation.

Appropriate positioning of the head is a crucial step for a successful intubation. Laryngoscopy is a commonly used method to facilitate the intubation process. This study evaluated the quality of intubation and its difficulty in sniff and neutral position. This was a clinical randomized trial, conducted in 2021 in Aja university of medical sciences. 40 patients, meeting the inclusion criteria were randomly assigned to sniff and neutral group and were intubated with standard procedure. The required time, number of attempts, laryngeal view, cord status, and the need for maneuver were evaluated b…

High-definition videolaryngoscopy is superior to fiberoptic laryngoscopy: a 111 multi-observer study.

PURPOSE: This study aims to analyse differences in fiberoptic laryngoscopy (FOL) versus high definition laryngoscopy (HDL) by examining videolaryngoscopy images by a large group of observers with different levels of clinical expertise in ear, nose and throat (ENT) medicine. METHODS: This study is a 111 observer paired analysis of laryngoscopy videos during an interactive presentation. During a National Meeting of the Dutch Society of ENT/Head and Neck Surgery, observers assessed both FOL and HDL videos of nine cases with additional clinical information. Observers included 41 ENT consultants (…

Dipping and rotating: two maneuvers to achieve maximum magnification during indirect transnasal laryngoscopy.

BACKGROUND: Since many years, office-based flexible transnasal laryngoscopy is a common routine procedure. The development of new technical equipment such as high-definition cameras and flexible tip-chip endoscopes nowadays allows for much more precise examination than a few years ago. In contrast to rigid laryngoscopy, it is possible to move the tip of the endoscope close to the vocal folds and to other structures of interest. Nevertheless, without professional handling of the equipment, one cannot benefit from the potential of the newest technology. METHOD: Two easily performed and very hel…

Measurement of end-expiratory pressure during transtracheal high frequency jet ventilation for laryngoscopy.

An anaesthetic technique using high frequency jet ventilation has been proposed for direct laryngoscopy, but this may expose the patients to the risk of barotrauma. In order to assess this risk, we have measured end-expiratory airway pressure (EEP) through the injector using two three-way solenoid valves mounted in series. At the end of insufflation the first valve was switched off and the apparatus deadspace connected to atmosphere through a large exit port during an adjustable time (decompression time). Then the second valve was switched off and the injection line connected to a transducer, …

Asystole during laryngoscopy with a videolaryngoscope in a patient with an anticipated difficult airway: A case report.

Asystole during tracheal intubation using a direct laryngoscope has been reported, but there has been no report of asystole during tracheal intubation using a videolaryngoscope. We report a case of asystole during videolaryngoscopy in a 76-year-old woman with marked tracheal deviation caused by a large thyroid adenoma. General anesthesia was induced with remimazolam 6 mg/kg/h, fentanyl 0.1 mg, and remifentanil 0.25 μg/kg/min, under apneic oxygenation using high-flow nasal oxygenation. After administration of rocuronium 40 mg, a McGrath™ MAC videolaryngoscope provided an excellent glottic view.…

A taxonomy of key performance errors associated with hyperangulated video laryngoscopy for emergency intubation.

BACKGROUND: Video laryngoscopy (VL) is now ubiquitous in emergency airway management. Hyperangulated video laryngoscopy (HAVL) differs from standard geometry VL (SGVL) in blade design, technique, and required microskills, yet the two are often regarded similarly. Despite widespread use, no comprehensive taxonomy of HAVL-specific performance errors exists. METHODS: We conducted an observational study of video-recorded endotracheal intubations (ETIs) performed in an urban academic emergency department (2020-2024). Fifty HAVL videos were first analyzed to derive 20 performance errors, followed b…

Comparison of Dental Injury Rates in Perioperative Intubation and Suspension Laryngoscopy for Otolaryngology Procedures.

OBJECTIVE: Direct laryngoscopy is an essential skill during perioperative intubation and otolaryngology procedures. Dental injury is a common complication of direct laryngoscopy. However, the technique and tools used by anesthesiologists, nurse anesthetists, and others during perioperative intubation and by ear, nose, and throat surgeons for their procedures are different. The purpose of this review is to explore the literature for all studies detailing rates of dental injury in each of these settings and to compare them to see if the approaches have a significant difference in rate of dental …

Comparison of inhaled versus intravenous anesthesia for laryngoscopy and laryngeal electromyography in a rat model.

BACKGROUND: Propofol and remifentanil intravenous combination is one popular form of total intravenous anesthesia (TIVA) in mainstream clinical practice, but it has rarely been applied to a rat model for laryngoscopy and laryngeal electromyography (LEMG). Our objective was to establish a safe and reproducible general anesthetic protocol for laryngoscopy and endoscopic LEMG in a rat model. Our hypothesis is that TIVA allows a minimally morbid, and feasible laryngoscopy and LEMG. METHODS: Sprague Dawley rats were subjected to either inhalational anesthesia (IA) (isoflurane) or TIVA (propofol an…

A novel device for measurement of subglottic stenosis in 3 dimensions during suspension laryngoscopy.

IMPORTANCE: A challenge in treating subglottic stenosis is assessment of airway caliber before and after management. At present, surgeons lack a simple, efficient, and precise method of measuring subglottic stenosis intraoperatively. We present a novel, easily reproducible tool for measurement of the diameter, location, and length of subglottic stenosis during suspension laryngoscopy. METHODS AND OBSERVATIONS: A set of 5 Kirschner wires (30-cm length and 1.6-mm diameter) were bent 90° at both ends to produce a series of 10 short ends designed to measure airway diameter (0.3- to 2.1-cm length …
