Cicatricial pemphigoid confined to the larynx.

The main features linking and yet distinguishing bullous pemphigoid from cicatricial pemphigoid have been discussed. This report demonstrates the usefulness of direct immunofluorescence study in establishing a diagnosis of cicatricial pemphigoid when lesions are confined to a single and unusual site such as the larynx.

Granular cell tumors of the larynx.

Granular cell tumors of the larynx are uncommon lesions. Only 15 were identified at the Columbia-Presbyterian Medical Center, New York, during the past 45 years. These tumors produce hoarseness usually and are identified as small keratotic, polypoid, or sessile nodules involving the glottic region (most often posterior glottis, arytenoid, or vocal process). These lesions can usually satisfactorily be treated by endoscopic excision. On histologic examination, pseudoepitheliomatous hyperplasia is frequently seen and may be misinterpreted as squamous cell carcinoma.

Chronic stenosis of the larynx in children.

Severe laryngeal stenosis in children can be corrected successfully with surgery. Re-canalization of the larynx allows for nasal respiration, reduces the danger of respiratory infections, contributes to improved tissue oxygenation, and stimulates the child's psychosomatic development. Contrary to prior otolarngeal approaches to laryngeal stenosis in children, it has been shown that surgery does not hinder the growth of the larynx, but rather stimulates the normalization of laryngeal function; and this function itself stimulates laryngeal skeleton. It is important that surgery be performed as s…

Reinnervation of the paralyzed larynx: a review.

Our high-speed and increasingly aggressive society, coupled with a continuing rise in necessary surgery of the neck and adjacent areas, accounts for the current increase in patients affilicted with vocal-cord paralysis. Such patients may sustain relatively little persistent loss of function (e.g., when only one cord is involved, which then regains function or becomes compensated) or considerable loss (as in cases of bilateral vocal-cord paralysis). To improve on existing methods of dealing with these problems, the author has developed and refined the nerve-muscle-pedicle technique for reinnerv

Computed tomography of the injured larynx.

Computed tomography (CT) permitted a much more detailed appraisal of laryngeal dysfunction in patients with blunt laryngeal trauma (8 cases) and iatrogenic injury caused by radiation therapy (7 cases), surgery (2 cases), or intubation (1 case). In thyroid cartilage fractures, the fragments may be widely displaced. The cricoid ring breaks in two places, frequently involving the signet. CT facilitates diagnosis of the mechanism of injury based on the site and extent of fractures. The patterns of distortion and fibrosis of the laryngeal cartilage following radiation therapy may be manifested as e

Primary pleomorphic liposarcoma of the larynx.

Liposarcoma is a rare tumor in the larynx. A case of such lesion occurring in a 52 year old male patient is described. This is the second case to be reported in the world literature. The patient underwent hemilaryngectomy and later total laryngectomy for a recurrent tumor and is living and well six years after diagnosis.

Solitary extramedullary plasmacytoma of the larynx.

Plasmacytomas, indistinguishable histologically from plasma cell tumors arising in bone marrow, occur almost anywhere in the body-usually independently of multiple myeloma. Thus, they represent an isolated area of plasmacytic dyscrasia but occasionally can be the first evidence of multiple myeloma. From 1949 to 1974, six patients with solitary extramedullary plasmacytomas of the larynx were seen at the Mayo Clinic; during this period, approximately 3,100 patients with malignant laryngeal neoplasms were examined. For these six patients, the median age was 53 years (range, 32 to 63 years). The f…

Reinnervation of the unilaterally paralyzed larynx.

Because of early success with reinnervation of the abductor musculature of bilaterally paralyzed larynges, experimental work and ultimately surgery in human patients has been undertaken to endeavor to apply the same basic concept to reinnervation of the unilaterally paralyzed larynx. A nerve-muscle pedicle is obtained from the omohyoid muscle in a manner identical to that previously reported. The lower 50% of the thyroid ala is carefully removed outside the perichondrial envelope and the lateral fibers of the thyroarytenoideus muscle (major laryngeal adductor) is exposed. The previously repair…

Granular cell tumors of the larynx.

Granular cell tumors of the larynx are relatively uncommon, always benign, and most commonly located in the posterior portion of the larynx. They are easily identified and should be differentiated from other lesions. A possible problem in the differential diagnosis is the presence of pseudoepitheliomatous hyperplasia, which overlies the granular cell tumor and which may mimic squamous cell carcinoma. Careful histopathologic differentiation is important because the laryngeal granular cell lesion should be managed conservatively, with transoral local excision usually being adequate. The histogen…

Act of swallowing in the fixed larynx.

In a 16-year-old female, complete stenosis of the larynx and hypopharynx developed as a consequence of the ingestion of lye cristals. Supraglotic laryngectomy, plastic surgery of the hypopharynx and antethoracic oesophagus were performed and the upper aero-digestive tract reconstructed. The dilated larynx remained fixed by the surrounding fibrous tissue, but the sphincteric action of the mobile vocal cords was partly salvaged. In that condition, the act of swallowing could be reestablished. The pathophysiology of this new mechanism of swallowing is discussed.
