A review of magnetic resonance imaging (MRI) performed to exclude the presence of spinal dysraphism in children presenting with lower limb pathology is unrewarding in the absence of abnormal neurology. Over a 5-year period, 29 children ages 2 weeks to 15 years with a mean age of 6 years presenting with lower limb abnormalities were referred for MRI of the spine to exclude an occult neurologic cause for the deformities. More than one limb abnormality, for example pes cavus and limb length discrepancy, was present in 93% of the children, and 11 children had severe or recurrent talipes equinovaru

BACKGROUND: Leg length discrepancy (LLD) is a musculoskeletal condition in which leg length is asymmetric. LLD can adversely affect adjacent joints. A typical example is hip-spine syndrome. We aimed to investigate how LLD affects spinopelvic alignment and mobility, under conditions simulating LLD.
METHODS: Fifty healthy adults (31 women, 19 men ; mean age 37.1 ± 8.9 years) participated in this study. A shoe lift was applied under the left foot to simulate LLD of 1, 2, 3, and 4 cm. Spinopelvic alignment while standing upright and mobility in flexion, extension, and lateral bending were measure

PURPOSE: As the relationship between leg-length discrepancy (LLD) and scoliosis has not been clearly defined, the purpose of this study was to explore the impact and severity of LLD in children and adolescents with scoliosis and the consistency between lower limb length discrepancy and pelvic height difference.
METHODS: This retrospective study was conducted using prospectively collected data from 63 patients with functional scoliosis-associated LLD who received treatment at our hospital from March 2021 to July 2024. The inclusion criteria included: ①Children or adolescents with functional sc

OBJECTIVE: Total hip arthroplasty (THA) combined with proximal femoral reconstruction is a novel osteotomy technique developed to address severe hip deformities. There is a paucity of robust clinical and radiological evidence regarding the outcomes of this novel osteotomy technique. This study aimed to evaluate the clinical and radiological outcomes of THA combined with proximal femoral reconstruction during the early follow-up.
METHODS: This is a retrospective case series of 63 hips who underwent THA combined with proximal femoral reconstruction at a single institution between January 2020 a

BACKGROUND: Total hip replacement (THR) for Crowe type IV developmental dysplasia of the hip (DDH) is still challenging due to specific joint deformities and the high incidence of post-operative complications.
OBJECTIVE: This study aimed to evaluate the clinical effect of trochanteric slide osteotomy (TSO) combined with a cementless femoral conical stem in THR for the treatment of Crowe type IV DDH.
METHODS: Thirty-one total hip replacements (26 patients) with Crowe type IV DDH were performed using TSO combined with a cementless femoral conical stem. Surgical outcomes were evaluated using le

BACKGROUND AND PURPOSE: Total hip arthroplasty (THA) is a successful approach to treat unilateral symptomatic neglected hip dislocation (NHD). However, the extensive leg length discrepancy (LLD) can hereby only be partially corrected. In case of residual LLD of more than 2 cm, subsequent femoral lengthening can be considered.
PATIENTS/MATERIAL/METHODS: Retrospective analysis of clinical data and radiographs of five patients (age 38.1 (28-51) years) with unilateral NHD who underwent THA with (n = 3) or without (n = 2) subtrochanteric shortening osteotomy (SSO) and secondary intramedullary fem

BACKGROUND: Physeal fractures and resultant physeal bars can pose significant problems in skeletal development for the injured growing child. Although now well-recognized, only a small body of experimental literature covering this problem is available. The goal of this study was to help further develop an understanding of the different regions of the physis and the way in which each region responds to injury/fracture.
METHODS: This Institutional Animal Care and Use Committee (IACUC)-approved study assessed bar formation using radiologic and histologic methods and measured leg lengths of skele

BACKGROUND: Limb-length discrepancy (LLD) in children with congenital lower extremity shortening is constant in proportion from birth to skeletal maturity (known as constant inhibition), but its developmental pattern in utero is unknown. The popular prenatal multiplier method to predict LLD at birth assumes constant inhibition in utero to be true. Verifying the in utero developmental pattern of LLD, and thus confirming the validity of the prenatal multiplier method, is crucial for meaningful prenatal parental counseling.
OBJECTIVE: To elucidate the in utero developmental pattern of LLD in fet

An 18-year-old male with neurofibromatosis type I was treated for congenital pseudarthrosis of the tibia using the Masquelet induced-membrane technique with internal fixation by retrograde implantation of a transplantar intramedullary nail. Bone healing was obtained at the expense of malunion with external rotation and 5.5cm of lower limb shortening. A motorised intramedullary-lengthening nail (Fitbone®, Wittenstein, Igersheim, Germany) was implanted. This treatment was successful in correcting the rotational malalignment and limb length discrepancy. The motorised nail Fitbone® may be a valid

A 26-year-old woman with a 1-year history of right knee pain had failed to respond to analgesia and activity modification in the community. Her general practitioner referred her to the orthopaedic department for specialist review. A thorough history revealed that she had multiple orthopaedic interventions as a child. The patient had significant postoperative infection of the left knee following knee surgery at 7 years of age. Examination demonstrated the presence of a limp, scoliosis and suspected leg length discrepancy. Plain film radiography confirmed the presence of leg length discrepancy,

Total hip replacement is one of the most successful surgical procedures of the 20th century (World Health Organisation). The success rate is dependent on the chosen endpoint. Evaluation of the outcome in joint replacement surgery has shifted from the revision rate toward patient satisfaction and quality of life. Patient satisfaction is reported to be up to 96% 16 years postoperatively, but the prevalence of groin pain after conventional total hip replacement ranges from 0.4% to 18.3% and activity-limiting thigh pain is still an existing problem linked to the femoral component of uncemented hip

OBJECTIVE: To investigate the method to avoid lengthening lower limbs after total hip arthroplasty in patients with congenital short femoral neck.
METHODS: The clinical data were analyzed retrospectively from 38 patients undergoing unilateral total hip arthroplasty between April 2005 and December 2010. There were 26 males and 12 females, aged 45-78 years (mean, 62.3 years). Among these cases, there were 11 cases of avascular necrosis of the femoral head, 17 cases of hip osteoarthritis, and 10 cases of femoral neck fracture. Before operation, 29 cases had leg length discrepancy; and the shorte

BACKGROUND: Ollier's disease (multiple enchondromatosis) is a rare non-hereditary condition characterised by the occurrence of multiple enchondromas, usually unilateral and asymmetrically distributed in the metaphyseal regions of the long bones and in the phalanges of the hand and foot.
CASE DESCRIPTION: A 10-year-old girl visited our outpatients' department with growth retardation of the left leg. Radiographic examination showed reduced growth and deformities in the femur and tibia, confirmed by MRI and bone scan. Since the aetiology of the deformities was not clear, we consulted the Netherl

Limb lengthening using external fixation may be associated with problems such as pin-track infections, poor patient acceptance, muscle transfixation, secondary axial deformity and re-fractures. Intramedullary lengthening nails have been designed to address these issues. We present our results for femoral limb lengthening in adults managed by intramedullary lengthening nails. A retrospective review was undertaken for 8 femoral lengthening procedures performed in adults using intra-medullary lengthening nails over a three-year period. The average age of our patients was 34 years; the average dur

BACKGROUND: Staple epiphysiodesis is an option for the treatment of limb-length discrepancies, but it is not without complications. The purpose of this study was to review the outcomes of staple epiphysiodesis, including changes in the mechanical axis.
METHODS: The study included patients who underwent, between 1990 and 2005, staple epiphysiodesis of the femur or tibia, or both, to address limb-length discrepancy. We reviewed preoperative, postoperative, and final long standing anteroposterior radiographs of fifty-four patients to assess limb-length discrepancy, shifts in the mechanical axis,

Congenital short femur is a rare and complex deformity that is often associated with other anomalies. There are few reports of either hip or knee dislocations during femoral lengthening in congenital short femur predisposed by associated anomaly. However, there are probably no case reports of both hip and knee joint dislocation occurring in same case during femoral lengthening in congenital short femur. We have this case of congenital short femur, where both hip and knee dislocation occurs during femoral lengthening. We reviewed different article to understand the cause and avoid such unwanted

PURPOSE: Although they are the most frequent physeal separations of the lower limb, epiphyseal separations (Salter I and II) of the distal tibia are rare injuries, peaking in adolescence. Generally they are considered lobe benign fractures with a low rate of complications. However, in the literature, concrete statements on the incidence of fracture-associated growth disturbances are infrequent and inconsistent, even contradictory.
MATERIALS AND METHODS: A review of textbooks, classical references and recent literature was performed to identify the available evidence on distal tibia physeal se
