[Spreading pigmented actinic keratosis--diagnosis and therapy].

Spreading pigmented actinic kereatosis is a rarely described premalignant epidermal lesion occurring on the skin following exposure to the sun. On clinical examination it may be difficult to distinguish it from pigmented seborrheic keratosis or lentigo maligna. Spreading growth and a diameter greater than 1.5 cm are characteristic features. We report a case of spreading pigmented actinic keratosis in a 61-year-old patient.

The detectability of intraepidermal melanocytes-A narrative review of immunohistochemical studies.

Some intraepidermal lesions, even the nonmelanocytic ones, may mimic melanoma in situ. The cytoplasmic clarity in atypical basal cells, architectural disorder with cell aggregates or nest-like structures within the epidermis often cause challenge particularly in sun-damaged skin. The aim of this review is to evaluate the effectiveness of the most often used immunohistochemical melanocyte markers in the diagnosis of intraepidermal lesions. All the analyzed markers can be useful in detection of intraepidermal melanocytes that occur more densely in lentigo maligna than in solar lentigo or sun-dam…

Clinical and dermoscopic triage of head and neck macules: The role of reflectance confocal microscopy in a prospective study of 2006 lesions.

BACKGROUND: Flat head/neck pigmented macules can be diagnostically challenging. OBJECTIVE: To assess the impact of reflectance confocal mic … ed as malignant (226 biopsied/excised; 10 noninvasively treated: 8 basal cell carcinoma [BCC] and 2 lentigo maligna [LM]). Twenty-six lesions changed at follow-up and were removed, with 4 malignancies (0.4%; 3 LM, 1 BCC). Histology (n = 252) confirmed 114 malignancies (62 LM/lentigo maligna melanoma [LMM]; 52 BCC/squamous cell carcinoma [SCC]). Dermoscopy …

Reflectance confocal microscopy of facial neoplasms: Follicular involvement as a clue to diagnosis.

BACKGROUND: Facial skin is characterized by high density of follicles. Facial neoplasms may present overlapping clinical and dermoscopic findings. Our goal was to evaluate and compare, via reflectance confocal microscopy (RCM), follicular involvement in facial neoplasms. METHODS: We retrospectively searched our image database, between January 2008 and December 2020, for all facial lesions with (1) a standardized set of clinical, dermoscopic, and RCM images, and (2) a biopsy-proven diagnosis of lentigo maligna/lentigo maligna melanoma (LM/LMM, n = 39), basal cell carcinoma (BCC, n = 51), squam…

Topography of Protein Kinase C βII in Benign and Malignant Melanocytic Lesions.

BACKGROUND: Protein kinase C βII promotes melanogenesis and affects proliferation of melanocytic cells but is frequently absent or decreased … ytic lesions by immunohistochemistry. RESULTS: Melanocytes in the basal layer of normal skin or in lentigo simplex stained strongly for PKC-βII. Common nevi lacked completely PKC-βII. All other lesions expressed variably PKC-βII, with cutaneous melanoma metastases displaying the lowest rate of positivity (14%). In the topographical analysis within a lesi …

An analysis of risk factors for cutaneous melanoma by anatomical site (Australia).

OBJECTIVE: Emerging evidence suggests that melanomas arising on the head and neck that are not lentigo maligna melanomas have different associations with phenotypic and environmental risk factors than those on the trunk and other sites. We sought to test this hypothesis in a population-based study in Queensland, Australia. METHODS: Risk factor data were collected from 2360 participants with incident cutaneous melanoma diagnosed 1982--1990, including 167 participants with lentigo maligna melanoma. For each risk factor, polytomous logistic regression analysis, using the trunk as a reference cat…

Pigmented solar (actinic) keratosis: an underrecognized collision lesion.

BACKGROUND: The lack of well-established diagnostic criteria for pigmented solar (actinic) keratosis (PSK) along with its poorly understood … hat PSK represents a collision between a nonpigmented solar keratosis and a pigmented lesion. Solar lentigo (72%) was the most commonly associated pigmented lesion followed by seborrheic keratosis and melanoma. PSK was suspected clinically in 17% of the cases. There were no significant differences in the quality and quantity of the melanocytes between pig …

The anatomic distribution of melanoma and relationships with childhood nevus distribution in Colorado.

The nature of the relationship between nevus development in childhood and later development of melanoma is unclear. Data on melanoma diagnos … f nevi versus melanoma. Superficial spreading melanoma was the most frequent histology, followed by lentigo maligna melanoma. Nodular melanoma was the least common histology. For males, there was no significant difference between the distribution of medium-sized (> or =2 mm) nevi and the distribution of both superficial spreading and nodular melanomas. Fo …

[Clinical and histopathological characteristics of malignant melanoma cases seen at "Dr. Manuel Gea González" General Hospital].

BACKGROUND: Melanoma is a type of tumor that arises from melanocytes generally located in the dermoepidermal junction. Although melanoma is found in less than 10% of cases, mortality is high representing 75% of deaths attributed to cutaneous cancer. There are four major subtypes: Superficial spreading melanoma, lentigo malignant melanoma, acral lentiginous melanoma and nodular melanoma. Superficial spreading melanoma is the most common type among Caucasians. In a Mexican case series, the nodular type is the most common type reported. The aim of this study was to determine the most common type …

The significance of eccentric and central hyperpigmentation, multifocal hyper/hypopigmentation, and the multicomponent pattern in melanocytic lesions lacking specific dermoscopic features of melanoma.

OBJECTIVE: To examine the significance of eccentric hyperpigmentation (EH), central hyperpigmentation (CH), multifocal hyper/hypopigmentation (MH/HP), and the multicomponent pattern (MCP) in melanocytic lesions lacking specific dermoscopic features of melanoma. DESIGN: A total of 3367 benign and malignant melanocytic lesions (n = 341 melanomas, excluding lentigo maligna and lentigo maligna melanoma) were examined to identify those lesions lacking specific dermoscopic features of melanoma but having any of the global patterns of EH, CH, MH/HP, and MCP. SETTING: Dermoscopic images were collect…

Halo dermatitis around tumours.

Circular dermatitis around different benign or malignant lesions were examined in 19 patients. The morphological picture was the same whether it occurred around an acquired naevus cell naevus, a congenital naevus cell naevus, a seborrheic keratosis, a stucco-keratosis, a keloid, a benign lentigo, an insect bite, a basal cell carcinoma, or a squamous cell carcinoma. The central lesions were macroscopically unaffected by the surrounding inflammation.

Eruptive Lentiginosis Within Resolved Psoriasis Plaques: A Case Report and Systematic Review of the Literature.

BACKGROUND: Many patients with psoriasis suffer from post-inflammatory pigmentary alteration (PIPA), which remains an unmet treatment need. Eruptive lentiginosis (EL) is a rare form of PIPA where multiple dark brown macules arise within a resolving psoriatic plaque. We present a case of a 68-year-old male with a history of chronic plaque psoriasis and prothrombin deficiency, who developed extensive EL following ixekizumab treatment. In addition, this article will discuss the current literature on risk factors, clinical outcomes, and treatments for EL. METHODS: The systematic literature review

Newborn infant with congenital lentigines as a manifestation of Carney Complex.

Carney Complex (CNC) is a rare syndrome characterised by skin pigmentation, endocrine over activity and myxomas, with the median age of detection being 20 years. We present a case of CNC diagnosed in infancy after being noted to have multiple lentigines over his face, abdomen, back and thighs at birth. We consider the differential diagnoses of similar cutaneous presentations in the well neonate and review the prognosis and suggested surveillance of patients with CNC.
