The role of limbal stem cells on neurotrophic cornea.

Corneal innervation and corneal epithelium integrity are essential for the maintenance of ocular surface homoeostasis, which is necessary for the preservation of the overall ocular surface health. Neurotrophic keratopathy (NK) is a condition characterised by a reduction or absence of corneal sensation, while limbal stem cell deficiency (LSCD) is defined as the loss of limbal epithelial stem cells. The co-occurrence and common pathophysiological pathways of LSCD and NK necessitate the consideration of both diseases in a unified framework. Interactions between limbal stem cells and the corneal s

Production and Limbal Lineage Commitment of Aniridia Patient-Derived Induced Pluripotent Stem Cells.

Congenital aniridia is caused by heterozygous mutations on the PAX6 gene leading to reduced amount of PAX6 protein (haploinsufficiency), abnormal eye development, and aniridia-associated keratopathy (AAK). This progressive corneal opacification resembles late-onset limbal stem cell (LSC) deficiency, leading to disrupted corneal epithelial renewal. The factors leading to AAK are not known and defects in native LSC differentiation and/or features leading to ocular surface dysfunction like inflammation and loss of innervation could contribute to development of AAK. Here, we produced induced pluri

A unique case of Cryptococcus laurentii keratitis spread by a rigid gas permeable contact lens in a patient with onychomycosis.

PURPOSE: We report the first case of fungal keratitis caused by Cryptococcus laurentii, a nonneoformans species. METHODS: A case of C. laurentii keratitis in a diabetic, gas permeable contact lens wearer was diagnosed after microbiologic evaluation of the corneal button after emergent keratoplasty. The excised cornea was also culture positive for Staphylococcus aureus and Fusarium solani. The medical history was significant for onychomycosis of the right great toe 2 months previously. Cultures of the toenail and of the contact lens case were also positive for C. laurentii and F. solani. RESU

The surgical management of an advanced pterygium involving the entire cornea.

PURPOSE: Conjunctival autografting is a well-established surgical procedure to prevent recurrence after pterygium excision. We described a modified technique of harvesting an annular autograft to cover a large conjunctival defect after excision of an extensive primary pterygium that involved three quadrants of the limbus. METHODS: A 73-year-old Chinese farmer underwent an excision of an advanced right pterygium involving the entire cornea. A free donor graft was then harvested from the remaining superior bulbar conjunctiva to cover the conjunctival defect, which involved approximately-three q

Multiple roles of Pax6 in postnatal cornea development.

Mammalian corneal development is a multistep process, including formation of the corneal epithelium (CE), endothelium and stroma during embryogenesis, followed by postnatal stratification of the epithelial layers and continuous renewal of the epithelium to replace the outermost corneal cells. Here, we employed the Cre-loxP system to conditionally deplete Pax6 proteins in two domains of ocular cells, i.e., the ocular surface epithelium (cornea, limbus and conjunctiva) (OSE) or postnatal CE via K14-cre or Aldh3-cre, respectively. Earlier and broader inactivation of Pax6 in the OSE resulted in th

[Model of inflammatory angiogenesis in rabbit cornea induced by punctual alkaline cauterization].

PURPOSE: To establish a model of angiogenesis in rabbit cornea induced by punctual alkaline cauterization. METHODS: Six rabbits were submitted to punctual cauterization in right cornea. It was used a circular piece (3 mm) of filter paper, that was previously soaked in 1 M NaOH and placed 1 mm from the superior limbus for 2 minutes. The animals were evaluated at 3, 6, 9, 12, 15, 18 and 21 days after cauterization. Quantification of angiogenesis was performed according to 4 methods: radial vessel length (V), angiogenic index (I), area of the triangle (T) and area of the circular band sector (S)

Pseudopterygium: An Algorithm Approach Based on the Current Evidence.

Pseudopterygium is a non-progressive conjunctival adhesion to the peripheral cornea secondary to a corneal-limbus damage. According to the literature, the main etiology is a previous eye trauma. Nevertheless, this could be biased by the existence of other underdiagnosed causes of pseudopterygium, some of which may have severe consequences for the integrity of the eye and patient's life. This comprehensive literature review was performed based on a search on the PubMed and Google Scholar databases of relevant pseudopterygium published papers according to our current knowledge and seeks to gathe

Molluscum contagiosum of the palpebral conjunctiva. Report of a case.

A molluscum contagiosum nodule, situated at the palpebral conjunctiva of the left lower eyelid at the level of the marginal and tarsal conjunctiva, was observed in a 53-year-old male. A mild follicular conjunctivitis was present, except in a zone immediately below the molluscum. On the corresponding site of this molluscum at the bulbar conjunctiva, the limbus and the cornea, no follicles, epithelial keratitis nor erosions were seen. Clinical diagnosis in atypically situated mollusca can be difficult. Cytological studies in these cases are very helpful. Alcohol-carbowax fixated and Papanicolau

The effect of corticosteroid and cyclosporin A on murine corneal allograft rejection.

BACKGROUND: The immunomodulatory T-helper type 1 (Th1) cytokine interferon-gamma (IFN-gamma) was measured in serum and cornea to ascertain its general contribution to corneal graft rejection and to establish a rational basis for the decision for or against systemic therapy. METHODS: Eight groups of differently treated BALB/c (H-2d) mice received a C3H (H-2 k) corneal graft. There was one saline-treated control group and two groups that received intramuscular cyclosporin A (CsA) for 14 or 40. Three groups received systemic or topical, systemic plus topical corticosteroid treatment, which was c

Cell Morphology as an In Vivo Parameter for the Diagnosis of Limbal Stem Cell Deficiency.

PURPOSE: The aim of this study was to investigate basal epithelial cell morphology (CM) in the central cornea and limbal areas of eyes with limbal stem cell deficiency (LSCD). METHODS: This was a prospective, cross-sectional comparative study. We developed a CM scoring system based on basal epithelial cell phenotypes graded from 0 (normal) to 3 (severe morphologic alterations); this system was evaluated by 2 independent masked observers. The CM score was compared with the LSCD clinical score, mean best-corrected visual acuity, and in vivo laser scanning confocal microscopy parameters used to

Concise review: the coming of age of stem cell treatment for corneal surface damage.

The cornea is a vital component of the eye because it provides approximately 70% of the refraction and focusing of incoming light. Being the outermost surface of the eye, it faces continuous stress from dryness, photodamage, infection, and injury; however, like the skin, the cornea regularly refreshes itself by shedding its epithelial cells, which are readily replaced, keeping the ocular surface stable and functional. This regular turnover of the corneal epithelial cells occurs through the stem cells in the limbus, an annular ring of a tissue surrounding the cornea, separating it from the scle

[HLA antigenicity of normal and pathological corneas].

Fresh human corneas and corneal buttons were studied for expression of HLA antigens. Using monoclonal antibodies in an indirect immunofluorescence assay, corneal layers were examined for class I (HLA-A, B, C) and class II (HLA-DR) histocompatibility antigens. Twenty-one human corneas were studied, 6 normal and 15 pathological: 4 buttons of allograft rejection, 9 buttons of pseudophakic bullous keratopathy. In fresh control corneas, HLA-A, B, C antigens were localized on corneal epithelium and on stromal keratocytes but were never found on endothelial cells. HLA-DR antigens were not detected on

Illuminate the concealed: An innovative method to identify the path of Schlemm's canal.

BACKGROUND: Trabeculotomy is a widely used surgical procedure for reducing intraocular pressure in primary congenital glaucoma. [ 1 ] Accurate identification of Schlemm's canal is the most critical step for surgical success. [ 2 ] Conventional landmarks such as the blue-white limbal junction and perforating vessels often become indistinct in buphthalmic eyes due to limbal stretching, making localization difficult and increasing the risk of inadvertent entry into the anterior chamber. [ 3 , 4 ]. PURPOSE: To present a novel, low-cost technique for identifying the path of Schlemm's canal during

Limbal Cysts: A Subset Exhibiting Cornea-Specific Cytokeratins.

Two cases of limbal cysts lined by nonkeratinizing epithelium were studied with a panel of cytokeratins. One was a long-standing lesion in a 30-year-old man, whereas the other was excised from a 40-year-old man following pterygium surgery. Each cyst was immunostained with a panel of cytokeratins that were specific exclusively and separately for corneal and conjunctival epithelia. The epithelial lining of each cyst was CK12 positive for corneal epithelium and CK13 negative for conjunctival epithelium. It is hypothesized that a subset of corneoscleral cysts contain corneal epithelium, probably d

Iris intraocular foreign bodies: safe and successful removal through limbus.

Metallic iris intra ocular foreign body (IOFBs) with minimal ocular damage pose an interventional challenge. We report safe removal of metallic intraocular foreign bodies embedded on the iris, in three cases by use of intraocular magnets. Two eyes had a clear lens with best corrected visual acuity (BCVA) of 20/20, while the third eye had a BCVA of 20/200 with siderotic cataract. Ultrasound biomicroscopy confirmed the superficial impaction of IOFB into the iris. Foreign bodies were successfully removed via the limbal route with a 20 g intravitreal magnet atraumatically in all 3 cases with prese

Limbal and corneal calcification in patients with chronic renal failure.

In patients with chronic renal failure on regular dialysis treatment, limboconjunctival degenerations and calcifications are commonly observed. In this study three groups of patients were followed over a period of 6 years. The first group consisted of 47 patients with renal failure, the second group of 17 patients with renal failure and hyperparathyroidism not controlled by drugs, and the third group seven patients with primary hyperparathyroidism without renal failure. The aim of this study was to determine the progression of the limboconjunctival changes over time. The hypothesis that an inc

[Excimer laser phototherapeutic keratectomy (PTK) and modified bare sclera technique for treatment of pterygium].

Forty-six pterygia were surgically removed using a modified "bare sclera" technique. Additionally, an excimer laser PTK was performed using the immersion method to smooth the irregular surface of the cornea. Follow-up was 6 to 10 months. We found 24 recurrences. In patients with pterygia of more than 4 mm who were from southern Europe, the Middle East, or Africa, the recurrence rate was 100%. In pterygia from 2 to 4 mm the recurrence rate was 50%; in pterygia up to the limbus no recurrences were noted. The excimer laser has no positive influence on pterygia recurrences that have been removed u
