Model of functional restriction in chronic obstructive pulmonary disease, transplantation, and lung reduction surgery.

Mechanical interactions between lung and chest wall are important determinants of respiratory function. When chest wall expansion during maximal inhalation generates insufficiently negative pleural pressures, the lungs remain functionally underinflated; this may be termed functional restriction. To explore mechanisms and effects of functional restriction in patients with emphysema, and to predict effects of single lung transplantation and lung volume reduction surgery (LVRS), we used a computational model based on standard physiology and measurements from individual patients. The model's lungs

Lung function and respiratory muscle strength after propranolol in thyrotoxicosis.

Thirty-five thyrotoxic patients were assessed before treatment, after treatment with propranolol, and after antithyroid drugs. The first group of patients (n = 17) performed the following tests at all three assessment points: forced expiratory volume in the first second (FEV1), vital capacity (VC), functional residual capacity (FRC), residual volume (RV), total lung capacity (TLC), maximal mid-expiratory flow rate (MMFR), diffusing capacity for carbon monoxide (DLCO), and maximum static inspiratory and expiratory mouth pressures (PImax and PEmax). Arterial blood gas analysis was also performed

The Diaphragm Acts as a Brake during Expiration to Prevent Lung Collapse.

RATIONALE: The diaphragm is the major inspiratory muscle and is assumed to relax during expiration. However, electrical postinspiratory activity has been observed. Whether there is an expiratory diaphragmatic contraction that preserves lung patency has yet to be explored. OBJECTIVES: We hypothesized the occurrence of an expiratory diaphragmatic contraction directed at stabilizing peripheral airways and preventing or reducing cyclic expiratory lung collapse. METHODS: Mild acute respiratory distress syndrome was induced in 10 anesthetized, spontaneously breathing pigs. Lung volume was decrease

Prediction of lethal pulmonary hypoplasia among high-risk fetuses via 2D and 3D ultrasonography.

OBJECTIVE: To compare accuracy between 3D and 2D ultrasonography for predicting lethal pulmonary hypoplasia (LPH) among high-risk fetuses. METHODS: In a cross-sectional prospective study at a fetal medicine referral center in Brazil, bilateral fetal lung scans were evaluated for 54 fetuses with suspected LPH between May 2008 and June 2011. Measurements for predicting LPH were ultrasonographic fetal lung volume/estimated fetal weight ratio (US-FLW), observed/expected thoracic circumference (o/e-TC), observed/expected thoracic circumference/abdominal circumference (o/e-TC/AC), observed/expected

Perspectives on the Edinburgh study of speech breathing.

This article offers critical perspectives on the Edinburgh study of speech breathing reported in this journal (Draper, Ladefoged, & Whitteridge, 1959) and elsewhere (Draper, Ladefoged, & Whitteridge, 1960; Ladefoged, Draper, & Whitteridge, 1958). These perspectives concern: (a) errors in establishing a backdrop of mechanical information; (b) discrepancies between data and statements about them; (c) counterpredictive features between data and other knowledge about breathing; and (d) inadequacies in acquiring, portraying, and interpreting electromyographic information relative to the muscular co

A new approach for the detection of obesity-related airway obstruction in lung-healthy individuals.

BACKGROUND: Subjects with obesity show an increased prevalence of airway obstruction but it is not clear in each case whether this reflects genuine lung disease. Via intentional increase in end-expiratory lung volume we studied the detection of obesity-induced airway obstruction in lung-healthy obese subjects. METHODS: The primary study population comprised 66 lung-healthy obese subjects and 23 normal weight subjects. Measurements were performed in a body plethysmograph allowing for recording and quantification of breathing loops in terms of specific airway resistance at both normal and inten

Physiology of body lateralization on regional lung ventilation and lung volumes in healthy subjects: Within-subjects design.

INTRODUCTION: Lateral positioning improves pulmonary mechanics and lung volumes, but its effects in healthy adults remain unclear due to individual variability. OBJECTIVE: To analyze the acute effects of lateral body positioning on regional lung ventilation and lung volumes in healthy adults. METHODS: This within-subject study included two protocols: supine and left-lateral position (unilateral) with repeated measures and supine, left, and right-lateral positions (bilateral). All positions were performed at 30° for 5 minutes on an automated rotation bed. Electrical Impedance Tomography measu

Esophageal pressure as an estimate of average pleural pressure with lung or chest distortion in rats.

Pressure-volume curves of the lungs and chest wall require knowledge of an effective 'average' pleural pressure (Pplav), and are usually estimated using esophageal pressure as Ples-V and Pwes-V curves. Such estimates could be misleading when Ppl becomes spatially non-uniform with lung lavage or shape distortion of the chest. We therefore measured Ples-V and Pwes-V curves in conditions causing spatial non-uniformity of Ppl in rats. Ples-V curves of normal lungs were unchanged by chest removal. Lung lavage depressed PLes-V but not Pwes-V curves to lower volumes, and chest removal after lavage in

Ventilatory failure in shrinking lung syndrome is associated with reduced chest compliance.

Shrinking lung syndrome is an extremely rare feature of systemic lupus erythematosus (SLE). We report on a 49-year-old woman with SLE who presented with dyspnoea in type 2 respiratory failure requiring mechanical ventilation. Medical imaging investigations revealed markedly reduced lung volumes and the absence of pulmonary emboli, pulmonary fibrosis or any significant parenchymal infiltrate consistent with shrinking lung syndrome. We observed significantly reduced chest compliance during positive pressure ventilation and noted that this contrasts with a widely held view that diaphragmatic weak

The human diving response, its function, and its control.

The purpose of this review is to outline the physiological responses associated with the diving response, its functional significance, and its cardiorespiratory control. This review is separated into four major sections. Section one outlines the diving response and its physiology. Section two provides support for the hypothesis that the primary role of the diving response is the conservation of oxygen. The third section describes how the diving response is controlled and provides a model that illustrates the cardiorespiratory interaction. Finally, the fourth section illustrates potential adapt

Pulmonary function in preeclamptic women receiving MgSO4.

Pulmonary function was studied in ten preeclamptic women in labor receiving continuous infusions of MgSO4 (group I) and six normotensive, healthy parturients in labor (group II). In group I, the forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and maximum voluntary ventilation (MVV) were measured prior to, two hours after and six hours after the start of infusions. The mean FVC decreased from a baseline value of 3.05 +/- 0.41 to 2.67 +/- 0.50 L (P less than .005) at two hours and to 2.71 +/- 0.42 L (P less than .005) at six hours; the mean FEV1 decreased from a contro

Respiratory mechanics in the adult respiratory distress syndrome.

Recent methods developed for noninvasive determination of the mechanical properties of the respiratory system have been discussed. These methods have already provided valuable information in patients mechanically ventilated in the ICU setting, and could readily be applied to ARDS. More extensive use of such methods should help to provide a better understanding of the physiologic processes and adaptive mechanisms present in both health and disease.

Long-term effects of exposure to sulfur dioxide. Lung function four years after a pyrite dust explosion.

The lung function of 7 men accidentally exposed to sulfur dioxide (SO2) in a pyrite dust explosion was followed for 4 yr. The greatest decrease in forced vital capacity, forced expiratory volume in one second, and maximal midexpiratory flow was observed 1 wk after the accident. After about 3 months no further decrement occurred. The pattern of spirometric findings was obstructive in 6 and restrictive in 1 of the patients. Four years after the accident a reversible obstruction of the bronchi was still observable in 3. Four patients reacted positively to the histamine challenge test. Two patient

Lung volume, dynamic lung compliance, and blood gases during the first 3 days of postnatal life in infants with meconium aspiration syndrome.

Serial measurements of pulmonary function and arterial blood gases during the first 3 postnatal days of life were obtained in 12 infants with meconium aspiration syndrome (MAS). Nine normal neonates with similar weight and gestational age were studied as controls. Infants with MAS has significantly lower pH on day 1, and had greater P(A-a)O2 throughout the study period than that of normal controls. The PCO2 was comparable between the groups. Both dynamic lung compliance (Cdyn) and specific lung compliance (C/VL) were lower in infants with MAS as compared with those of normal infants. The funct

Posterior packs and the nasopulmonary reflex.

Increased mortality and arterial hypoxemia have long been associated with posterior nasal packs placed for control of severe epistaxis. Several authors have postulated a nasopulmonary reflex to partially explain this clinically observed phenomenon. In ten young, healthy subjects, using a multiparameter pulmonary evaluation, posterior nasal packs were placed and no significant changes were observed in lung volumes, flow and alveolar gas exchange, especially oxygenation. These findings suggest that aspiration, sedation, and degeneration of pulmonary function with age, not a nasopulmonary reflex,

Lung volumes and distensibility, and maximum respiratory pressures in thyroid disease before and after treatment.

Lung volumes, static pressure volume curves of the lung, and maximum respiratory pressures were measured in six patients with thyrotoxicosis and three with myxoedema before and after treatment. After treatment three of the thyrotoxic patients had increases in vital capacity of the same order as previously reported, but in contrast to previous reports, the lung volume changes were not associated with, nor due to, concomitant changes in respiratory muscle performance. Static pulmonary compliance increased in four of the six patients. The initial findings were more in keeping with the presence of

Arterial oxygenation differences with carbon dioxide-induced versus voluntary increases in minute ventilation in chronic airway obstruction.

Two groups of patients with similar degrees of chronic airway obstruction exhibited different degrees of increased arterial oxygenation with CO2-induced hyperpnea versus voluntary increases in ventilation matched for ventilatory pattern. Patients who failed to increase arterial PO2 as much with voluntary hyperventilation as with CO2 challenge also had a lower dynamic compliance with the voluntary maneuver, whereas those with similar increases in arterial PO2 had similar dynamic compliances during the 2 periods of increased minute ventilation. The differences between the 2 groups are believed t

Postural changes in lung volumes in patients with heart failure and Cheyne-Stokes respiration: Relationship with sleep apnea severity.

BACKGROUND AND AIM: It has been proposed that the increased severity of sleep apnea frequently observed in heart failure (HF) patients with Cheyne-Stokes respiration (CSR) when sleeping in the supine compared to the lateral position, may be caused by the concomitant reduction in functional residual capacity (FRC). We assessed positional changes in FRC in patients with CSR and investigated the relationship between these changes in the laboratory and corresponding changes in CSR severity during sleep. METHODS: After a diagnostic polysomnography, 18 HF patients with dominant CSR and an apnea-hyp
