Peripheral T-cell lymphoma: a clinicopathologic study of 42 cases.

Clinical and histopathologic material from 42 patients with peripheral T-cell lymphoma (PTCL) was reviewed. The median age was 63.5 years (range, 11-97 years). The male:female ratio was 2.8:1. Prior immune or lymphoproliferative diseases occurred in 36% of the patients. PTCL was advanced at presentation with B symptoms (67%), generalized adenopathy (69%), and stage III/IV disease (79%). Suspected lung or pleural involvement (21%), hepatomegaly (29%), and splenomegaly (43%) were common; marrow involvement was documented in 37% of the patients at presentation and in 51% of patients during the il

Follicular peripheral T-cell lymphoma expands the spectrum of classical Hodgkin lymphoma mimics.

Epstein-Barr virus (EBV)-infected B cells with Reed-Sternberg-like cell (RS) features may occur in peripheral T-cell lymphomas (PTCLs), especially in angioimmunoblastic T-cell lymphoma. Here, we report 5 patients presenting with lymphadenopathy whose first biopsies demonstrated nodular lymphoid proliferations containing scattered CD30+, CD15+, EBV+ Hodgkin and Reed-Sternberg-like cells, which led to an initial diagnosis of lymphocyte-rich classical Hodgkin lymphoma. However, the uncommon clinical features and/or the occurrence of relapse as PTCL prompted review of the biopsies with expanded im

EBV-positive immunodeficiency lymphoma after alemtuzumab-CHOP therapy for peripheral T-cell lymphoma.

Chemotherapy with alemtuzumab and the combination of cyclophosphamide, adriamycin, oncovin, and prednisone (CHOP) has become experimental trial therapy for aggressive T-cell lymphoma. Several multicenter phase 3 trials will incorporate this scheme. As part of an ongoing phase 2 trial in which we recently treated 20 patients with 8 cycles of CHOP every 2 weeks with 3 additional doses of 30 mg alemtuzumab per cycle, we observed the development of Epstein-Barr virus (EBV)-positive lymphoproliferative disease, after completion of the immunochemotherapy in 3 patients with peripheral T-cell lymphoma

The role of allogeneic stem cell transplantation in peripheral T-cell lymphoma.

Peripheral T-cell lymphomas (PTCLs) are rare, biologically heterogeneous malignancies associated with poor outcomes following conventional chemotherapy. Despite advances such as brentuximab vedotin-based regimens in CD30-positive disease, relapse rates remain high, particularly in relapsed/refractory (R/R) PTCL. Allogeneic hematopoietic cell transplantation (allo-HCT) represents a potentially curative treatment option through a graft-versus-lymphoma effect capable of overcoming chemoresistance. Retrospective and prospective studies report 5-year overall survival rates of approximately 40-55% i

[A case of peripheral T-cell lymphoma with lung involvement].

Peripheral T-cell lymphoma (PTCL) is a rare tumor that originates in mature T cells or NK cells. PTCL is extremely rare in the lungs, either as a primary or secondary lesion, and the clinical course usually progresses rapidly, resulting in a poor prognosis. This article reported a case of a 35-year-old young female patient who was diagnosed with PTCL by percutaneous lung puncture pathology. The patient was admitted to the hospital due to cough and sputum. Chest CT scan revealed multiple patchy, high-density shadows and scattered nodules of varying sizes in both lungs. The patient was admitted

Therapy of Peripheral T Cell Lymphoma: Focus on Nodal Subtypes.

PURPOSE OF REVIEW: Peripheral T cell lymphomas (PTCLs) are a heterogeneous group of non-Hodgkin lymphomas with inferior prognosis compared with their B cell counterparts characterized by frequent relapses, resulting in a median 5-year survival of approximately 30%. Their diverse clinicopathologic features challenge existing treatment paradigms that treat all patients uniformly. Here we review recent advances in the treatment of these diseases. RECENT FINDINGS: While current treatment still relies largely on combination chemotherapy, the introduction of more effective novel and targeted therap

A Case of Lung Involvement in Peripheral T Cell Lymphoma.

BACKGROUND: Lung involvement is rare in peripheral T cell lymphoma, and there is a lack of sufficient clinical study. The authors describe a rare case of lung involvement in a 40-year-old male patient with peripheral T cell lymphoma. METHODS: Hematological investigation, bone marrow aspirate, and lung biopsy were performed. RESULTS: The patient received 4 courses of CHOP regimen chemotherapy. He achieved partial response (PR) at the 2nd course, but showed disease progressive (PD) at the 4th course. Then he received 2 courses of GDP regimen chemotherapy. Unfortunately, the patient died of tum

Recent Advances in the Treatment of Peripheral T-Cell Lymphoma.

UNLABELLED: Peripheral T-cell lymphoma (PTCL) is a heterogeneous group of clinically aggressive diseases associated with poor outcome. Despite progress in the last several years, resulting in a deeper understanding of the natural history and biology of PTCL based on molecular profiling and next-generation sequencing, there is a need for improvement in efficacy of chemotherapeutic regimens for newly diagnosed patients. Treatment in the front-line setting is most often cyclophosphamide, doxorubicin, vincristine, and prednisone (CHOP) or CHOP-like regimens, which are associated with a high failur

MicroRNA profiling in peripheral T-cell lymphoma, not otherwise specified.

BACKGROUND: Peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS) is an obviously heterogeneous and highly invasive malignancy without definite phenotype. MicroRNAs (miRNAs) are powerful gene regulators and have been reported as biomarkers in many malignancies. OBJECTIVE: To discover potential signaling miRNAs in PTCL-NOS distinguished from reactive lymphoid hyperplasia (RLH) and to explore the molecular characteristics based on the discovery. METHODS: We measured the expression profile of miRNAs in PTCL-NOS and RLH from our patients using a panel PCR array. We used GO-Analysis to e

Peripheral T-cell lymphoma with unusual clinical presentation of rhabdomyolysis.

Primary extranodal lymphoma is known to occur in nose, gastrointestinal tract, skin, bone, and central nervous system. However, it is extremely rare for primary lymphoma to arise in skeletal muscle. We report a case of a 32-year-old man who presented initially with fever and fatigue. He had a history of alcohol abuse. Laboratory studies and computerized tomography scan showed results consistent with rhabdomyolysis, but the cause of the rhabdomyolysis was undetermined. After biopsy of abdominal skeletal muscle with histologic examination and T-cell receptor gamma chain gene rearrangement analys

HIV-associated peripheral T-cell lymphoma with a cytotoxic phenotype.

Most human immunodeficiency virus (HIV)-related lymphomas are of B-cell origin, and the T-cell type is very rare. We experienced a Japanese case of HIV-associated peripheral T-cell lymphoma (HIV-PTCL). Sudden intestinal hemorrhage necessitated emergent surgical resection of the small intestine, in which an ulcerative lesion was detected. A histopathological examination revealed large tumor cells in the base of the ulcer, which were immunohistochemically CD30(+), CD56(+), granzyme B(+), CD3(+) (focally), CD4(-), CD8(-) and EBER(+). A diagnosis of PTCL, not otherwise specified, was therefore mad

Cervical presentation of peripheral T-cell lymphoma not otherwise specified.

Peripheral T-cell lymphoma not otherwise specified (PTCL-NOS) is an uncommon type of lymphoma with a clinically aggressive course and a relatively poor prognosis. Many affected patients present with nodal involvement, and there is also potential for extranodal involvement of the liver, gastrointestinal tract, bone marrow, and/or skin. We describe the case of a 68-year-old woman who presented with a 6-week history of an intermittently tender left-sided neck mass. Findings on imaging and fine-needle aspiration biopsy were inconclusive. Pathologic studies of excisional biopsy specimens ultimately
