Mechanisms of fluoroquinolone and macrolide resistance in Campylobacter spp.

The incidence of human Campylobacter infections is increasing worldwide, as well as the proportion of isolates resistant to fluoroquinolones and/or macrolides, the drugs of choice to treat campylobacteriosis. In this review, we report recent developments in the understanding of the resistance mechanisms to fluoroquinolones and macrolides. In addition, we will discuss the recent findings on multidrug resistance in Campylobacter spp.

Bacteremic pneumococcal pneumonia associated with macrolide failure.

Despite rising rates of macrolide resistance to Streptococcus pneumoniae, the current guidelines for outpatient treatment of community-acquired pneumonia include a macrolide as initial empiric therapy. There have only been a few previously documented cases of macrolide treatment failure in community-acquired pneumonia and there is controversy as to whether macrolide resistance in S. pneumoniae is clinically important. Reported here are two cases of community-acquired pneumonia caused by S. pneumoniae, one of which was fatal, that failed to respond to clarithromycin.

Macrolide resistance in adults with bacteremic pneumococcal pneumonia.

We conducted a case-control study of adults with bacteremic pneumococcal pneumonia to identify factors associated with macrolide resistance. Study participants were identified through population-based surveillance in a 5-county region surrounding Philadelphia. Forty-three hospitals contributed 444 patients, who were interviewed by telephone regarding potential risk factors. In multivariable analyses, prior exposure to a macrolide antimicrobial agent (odds ratio [OR] 2.8), prior flu vaccination (OR 2.0), and Hispanic ethnicity (OR 4.1) were independently associated with an increased probability

Prevalence of macrolide resistance in Mycoplasma pneumoniae in The Netherlands.

BACKGROUND: Macrolide resistance in Mycoplasma pneumoniae is an increasing concern worldwide. OBJECTIVES: To investigate the prevalence of … d in 7 of 235 specimens (3%). CONCLUSIONS: The low resistance rate of 3% still supports the use of macrolides for empirical treatment of M. pneumoniae infections in The Netherlands, but macrolide non-responsive infections should lead to the suspicion of resistant M. pneumoniae. Regular surveillance studies are important as clonal spread may cause rapid chan …

Research and Application of the Polyene Macrolide Antibiotic Nystatin.

Nystatin is a polyene macrolide antibiotic with broad-spectrum antifungal activity and serves as a key therapeutic agent for superficial fungal infections. This review systematically elaborates on its multicomponent chemical nature, its mechanism of action targeting ergosterol, and highlights the potential adverse effects, such as cardiotoxicity, associated with impurities like RT6 (albonoursin). The fundamental analytical techniques for quality control are outlined. Furthermore, the clinical applications and combination therapy strategies of nystatin in treating oral diseases, vaginitis, and

Macrolide antibiotics (including azithromycin) for cystic fibrosis.

BACKGROUND: Cystic fibrosis (CF) is a life-limiting genetic condition, affecting over 90,000 people worldwide. CF affects several organs in the body, but airway damage has the most profound impact on quality of life (QoL) and survival. Causes of lower airway infection in people with CF are, most notably, Staphylococcus aureus in the early course of the disease and Pseudomonas aeruginosa at a later stage. Macrolide antibiotics, e.g. azithromycin and clarithromycin, are usually taken orally, have a broad spectrum of action against gram-positive (e.g. S aureus) and some gram-negative bacteria (e.

Administration of macrolide antibiotics increases cardiovascular risk.

BACKGROUND: The increased risk of cardiovascular events in patients prescribed macrolides has been subject to debate for decades. METHODS: Medline, EMBASE databases and ClinicalTrials.gov were searched from inception until August 31, 2022 for studies investigating the link between macrolides and cardiovascular risk. A meta-analysis was performed using a random-effects model. RESULTS: A total of 80 studies involving 39,374,874 patients were included. No association was found between macrolides and all-cause death. However, compared with the non-macrolide group, macrolides were associated with…

What is the evidence for macrolide therapy in chronic rhinosinusitis?

PURPOSE OF REVIEW: To assess the most recent evidence for macrolide therapy in chronic rhinosinusitis (CRS). RECENT FINDINGS: Macrolides play a significant role in a select group of patients with CRS. Low-serum and tissue eosinophilia in patients who do not respond to corticosteroid therapy appeared to be an effective predictor of a CRS phenotype suitable for a trial of long-term macrolide therapy. Therapies using half a dose for longer than 12 weeks have noted good outcomes. SUMMARY: The anti-inflammatory and immunomodulatory effects of macrolides have been demonstrated in several studies. …

Palstimolide A: A Complex Polyhydroxy Macrolide with Antiparasitic Activity.

Marine Cyanobacteria (blue-green algae) have been shown to possess an enormous potential to produce structurally diverse natural products that exhibit a broad spectrum of potent biological activities, including cytotoxic, antifungal, antiparasitic, antiviral, and antibacterial activities. Here, we report the isolation and structure determination of palstimolide A, a complex polyhydroxy macrolide with a 40-membered ring that was isolated from a tropical marine cyanobacterium collected at Palmyra Atoll. NMR-guided fractionation in combination with MS2-based molecular networking and isolation via

Macrolide and Nonmacrolide Resistance with Mass Azithromycin Distribution.

BACKGROUND: Mass distribution of azithromycin to preschool children twice yearly for 2 years has been shown to reduce childhood mortality in sub-Saharan Africa but at the cost of amplifying macrolide resistance. The effects on the gut resistome, a reservoir of antimicrobial resistance genes in the body, of twice-yearly administration of azithromycin for a longer period are unclear. METHODS: We investigated the gut resistome of children after they received twice-yearly distributions of azithromycin for 4 years. In the Niger site of the MORDOR trial, we enrolled 30 villages in a concurrent tria

The tendency to the formation of macrolide resistance in campylobacteriosis pathogens is considered as a serious threat to public health due … resistant to azithromycin. The search and selection for gene markers of Campylobacter resistance to macrolides was performed. It was found that the resistance of C. jejuni to erythromycin is realized mainly via synthesis of proteins that protect ribosomes (the presence of coding sequences was detected in 45% of the studied strains) and the transmembrane pump …

[Maintenance treatment with macrolide antibiotics in COPD: not for everyone].

Several studies and meta-analyses have recently shown a positive effect of maintenance treatment with macrolide antibiotics in patients with chronic obstructive pulmonary disease (COPD). The next step is not to implement this therapy on a large scale, but to restrict the use of antibiotics as much as possible. In a recent meta-analysis, maintenance therapy with macrolides reduced exacerbation frequency and improved quality of life (Wang et al., 2018). Comparable effects have been observed in patients with rare inflammatory pulmonary diseases. However, it is arguable to restrict maintenance the…

Differentiating the Pharmacodynamics and Toxicology of Macrolide and Ketolide Antibiotics.

This is a review of the macrolide and ketolide field focusing on differentiating the pharmacodynamics and especially the toxicology of the macrolides and ketolides. We emphasize the diversity in pharmacodynamics and toxicity of the macrolides and ketolides, resulting from even small structural changes, which makes it important to consider the various different compounds separately, not necessarily as a class. The ketolide, telithromycin, was developed because of rising bacterial macrolide resistance but was withdrawn postapproval after visual disturbances, syncope, myasthenia gravis, and hepat…

The Nargenicin Family of Oxa-Bridged Macrolide Antibiotics.

The nargenicin family of antibiotic macrolides comprise a group of bacterial natural products with a rare ether bridged cis-decalin moiety and a narrow spectrum of activity. Most family members were identified almost four decades ago and were placed on the shelf due to the numbers of broad-spectrum compounds available at the time. However, in light of rising rates of antimicrobial resistance, there has been a renewed interest in the use of narrow-spectrum antimicrobials. Here, we review the history of this family of compounds, including synthetic approaches, and highlight the recently uncovere…

Look and Outlook on Enzyme-Mediated Macrolide Resistance.

Since their discovery in the early 1950s, macrolide antibiotics have been used in both agriculture and medicine. Specifically, macrolides such as erythromycin and azithromycin have found use as substitutes for β-lactam antibiotics in patients with penicillin allergies. Given the extensive use of this class of antibiotics it is no surprise that resistance has spread among pathogenic bacteria. In these bacteria different mechanisms of resistance have been observed. Frequently observed are alterations in the target of macrolides, i.e., the ribosome, as well as upregulation of efflux pumps. Howeve…

Increasing Macrolide and Fluoroquinolone Resistance in Mycoplasma genitalium.

Escalating resistance to azithromycin and moxifloxacin is being reported for Mycoplasma genitalium in the Asia-Pacific region. Analyzing 140 infections, we found pretreatment fluoroquinolone-resistance mutations in parC (13.6%) and gyrA (5%). ParC S83 changes were associated with moxifloxacin failure. Combined macrolide/fluoroquinolone-resistance mutations were in 8.6% of specimens, for which recommended therapies would be ineffective.

Resistance to Macrolide Antibiotics in Public Health Pathogens.

Macrolide resistance mechanisms can be target-based with a change in a 23S ribosomal RNA (rRNA) residue or a mutation in ribosomal protein L … tutively expressed. The induction of gene expression is cleverly tied to the mechanism of action of macrolides, relying on antibiotic-bound ribosomes stalled at specific sequences of nascent polypeptides to promote transcription or translation of downstream sequences.
