[Experience with titanium mesh as a temporary foreign material implant for the reconstruction of mandibular defects].

Restoration of a loadable mandible after extensive resections, defects or comminuted fractures, serious osteomyelitis and pseudarthrosis will be successful only if sufficiently stable fixation of the fragments and immobilization beyond the physiological period of bone healing are guaranteed. By means of various types of plates and extroral appliances for osteosynthesis the conventional methods have been improved, but failures and complications are still frequent. The treatment itself is a great strain for the patient. The titanium mesh implant system developed by Ph. Boyne and his group for re

Head and neck reconstruction in vessel-depleted necks: Case report of a labio-mental and mandibular reconstruction using an arteriovenous loop.

Major defects of the facial structures cause severe functional and esthetic impairment. Difficulty in head and neck reconstruction lies in cases of secondary, tertiary, or further reconstruction. This is not a rare situation for patients who had cancer of the upper airways, since the rate of recurrence, second location, or osteoradionecrosis is high. Multiple surgeries and radiation therapy cause significant fibrosis and scar tissues, making any further reconstruction a major challenge for the surgeon when operating patients with vessel- depleted neck. We report our experience with a clinical

Reconstruction of mandibular defects using free fibula flaps with a spliced surgical template system.

BACKGROUND: Mandibular defect reconstruction after tumor resection poses significant challenges in oral and maxillofacial surgery. The free … 0.05). CONCLUSION: The spliced surgical template system provides higher accuracy and stability in mandibular reconstruction with the free fibula flap. It reduces errors in the positioning of fibula segments, resulting in improved reconstruction precision and potentially contributing to better functional outcomes compared with conventional methods.

Biomechanical evaluation of reconstruction plates in bone-grafted mandibular defects: a finite element analysis.

OBJECTIVE: To evaluate, through three-dimensional finite element analysis (FEA), the stress and strain distribution in reconstruction plates of different thicknesses used for fixation of bone-grafted segmental mandibular defects. METHOD: A dentate mandibular model was obtained from computed tomography data and digitally processed for FEA using ANSYS 7.0. Three reconstruction plate models (2.0, 2.4, and 3.0 mm thick) were applied to simulate defects in the symphyseal, body, and angle regions of the mandible, each associated with block bone grafts and fixed using 2.4-mm locking screws. A 300-N

Algorithm-Based Reconstruction for Pediatric Mandibular Desmoid Tumors: Two Decades From a Single Institution.

Pediatric mandibular desmoid tumors are rare, locally aggressive lesions that can create composite defects in a growing craniofacial skeleton. We retrospectively reviewed biopsy-proven pediatric desmoid tumors involving the mandible region treated at a tertiary children's hospital (2004-2024) and mapped each case to an institutional, algorithm-based reconstructive pathway (A-D) incorporating a mandibular resection index (RI; threshold 32%) and 2 modifiers (modifier 1: systemic therapy/radiotherapy; modifier 2: staged bony reconstruction for inadequate bone stock). We summarized pathway assignm

Differential fibular bone mineral density changes following maxillary versus mandibular reconstruction: a cohort study.

The aim of this study was to compare the changes in fibular bone mineral density (BMD) in flaps used for maxillary and mandibular reconstruction, using quantitative computed tomography (QCT). Patients who underwent jaw reconstruction with a fibula flap at Peking University Hospital of Stomatology between September 2021 and September 2024 were included retrospectively. QCT performed at 1 week, 3, 6, and 12 months postoperatively was used to measure and assess BMD, and corresponding resorption amounts were calculated (percentage decrease in BMD relative to the value at 1 week after surgery). Cha…

Towards patient-specific optimization for mandibular reconstruction planning based on predicted bone-union propensity.

Mandibular reconstruction with vascularized bone grafts is complicated by donor-host nonunion, and current virtual surgical planning produces a geometric plan rather than optimizing for bone-union propensity at the donor-host interface. We present OsteoOpt++, an image-to-decision planning loop for patient-specific mandibular reconstruction. A pre-operative computed tomography (CT) is converted into a personalized digital twin through template-to-patient registration and CT-derived updates of the muscle and temporomandibular-joint parameters. Bayesian optimization with an expected-improvement-p…

The Implant-borne Articulation Splint in Fibula Free Flap Mandibular Reconstruction: A Technical Note.

Computer-aided design and computer-aided manufacturing and digitally simulated surgeries have revolutionized maxillomandibular reconstruction. In particular, this technology has increased the accuracy and facilitated the process of dental implantation in fibula free flaps. Despite the efficacy of virtual planning, there is a minor degree of translational difference between digital and intraoperative measurements, which may affect the precision of implant and fibula orientations. This is especially concerning during the last stage of fibula insetting, where the graft segments have the potential…

Novel and accurate 3D-Printed surgical guide for mandibular reconstruction with integrated dental implants.

PURPOSE: Patients with mandibular defects due to trauma or infiltrated disease are in a need of functional mandibular implants that will completely restore the function of their lower jaw. One of the most important roles of well-functioning jaw is mastication, a complex mechanism. A conventional approach used in oral and maxillofacial surgery accomplish this aim via two major surgeries- mandibular reconstruction and surgical placement of dental implants. Little work has been done on combining the two surgeries into with using Additive Manufacturing (AM) and digital planning. MATERIAL AND METH…

Endoscopically Assisted Middle Cranial Fossa Reconstruction Following Traumatic Intracranial Intrusion of the Mandibular Condyle.

Traumatic intracranial intrusions of the mandibular condyle are a rare injury pattern that carries significant morbid outcomes due to potential for neurological, otological, temporomandibular joint damage, or joint ankylosis. Treatment modalities involve either closed reduction, open approaches via transcranial or subtemporal approaches, or delayed total joint replacement. This paper presents a clinical report of a traumatic intracranial intrusion of the mandibular condyle treated with an endoscopically assisted preauricular approach to reconstruct the middle cranial fossa and temporomandibula

Unusual mandibular condylar pathology: Aneurysmal bone cyst, a case report and review on reconstruction.

Aneurysmal bone cysts (ABCs) involving condyle are unusual clinical presentation affecting younger group of society with only 12 cases reported till today in English literature. In this case of ABC condyle following resection, reconstruction of temporomandibular joint was done by alloplastic condylar prosthesis as a primary choice. Monthly basis follow-up was done for 1-year with a successful result. Reviewing the reconstructive options used in ABC condyle cases we found that the condylar head add-on system has shown a satisfactory result in comparison to costochondral graft with no donor site

Reconstruction of large mandibular and surrounding soft-tissue defects using distraction with bone transport.

Reconstruction of large bone and soft-tissue defects of the inferior third of the face is possible using various surgical techniques. Patients who require these procedures need to be in good general health, may have sequelae linked to donor sites, and require several interventions to achieve good aesthetic and functional results. The aim of this study was to report outcomes in patients with large mandibular and soft-tissue defects treated using osteogenic distraction with bone transport. Between 2001 and 2008, 14 patients had distraction with bone transport. Most patients were men (92.1%). The

Reconstruction of large mandibular bone and soft-tissue defect using bone transport distraction osteogenesis.

Reconstruction of large anterior mandibular defects is a challenging task. The condition can become even more complex if primary reconstruction fails, leading to loss of the entire midline portion of the lower face with massive scarring of the remaining tissues. Bone transport distraction osteogenesis can provide a viable treatment option for these patients. One of such cases will be presented, followed by a discussion of the advantages, disadvantages, and limitations of the technique.
