Assessing fundamental clinical skills of osteopathic medical students.

CONTEXT: Clinically based performance assessments, including those employing standardized patients (SPs), have been utilized to educate and assess medical students and graduates. Starting in the 1990s, demonstrating competency on these types of assessments became part of the medical licensure pathway in some countries, and was re-added to the licensure pathway in the United States in 2004. Studies have been published to support the validity of the scores and associated pass/fail decisions obtained from SP assessments. With the onset of the COVID-19 pandemic, testing organizations were forced t

Musculoskeletal Treatments: Physical Modalities.

Chronic musculoskeletal pain disorders account for 70% to 80% of chronic pain diagnoses in the United States. T he most common conditions are low back pain, neck pain, headache disorders, and knee and hip osteoarthritis. Most evidence suggests that treatments for chronic pain are best used in combination, such as in multidisciplinary rehabilitation programs. Evaluation of individual treatments in isolation is limited by small sample sizes, high risk of bias, lack of standardization in clinician delivery and treatment dosage or intensity, and inconsistent categorization of treatment techniques.

Diagnosis and management of primary pulmonary leiomyosarcoma.

Leiomyosarcomas are cancers of smooth muscle cells that can arise from any location but occur most often in the uterus, retroperitoneum, or intraabdominal region. Primary leiomyosarcomas of the lung are extremely rare and are often diagnosed as a mass on routine chest radiography. Metastasis is uncommon and typically occurs late in the disease process, indicating the importance of early detection. Surgical excision is generally curative, offering patients optimal prognoses. The authors describe a 56-year-old asymptomatic man who was found to have a pulmonary mass on routine chest radiography.

How Much Physiotherapy, Chiropractic, and Osteopathy Care Do Compensated Australian Workers with Low Back Pain Receive? A Retrospective Cohort Study.

OBJECTIVES: To identify the prevalence and frequency of physiotherapy, chiropractic, and/or osteopathy care in Australians with workers' compensation claims for low back pain (LBP). METHODS: We included workers with accepted workers' compensation claims longer than 2 weeks from the Australian states of Victoria, Queensland, South Australia, and Western Australia. Workers were grouped by whether they attended physiotherapy, chiropractic, and/or osteopathy in the first 2 years of their claim. Descriptive statistics and logistic regression were used to describe differences between groups. Descri

The Role of Sphenobasilar Synchondrosis in Disease and Health.

BACKGROUND: The sphenobasilar synchondrosis (SBS; also, sphenobasilar synostosis or sphenooccipital synchondrosis) plays a major role in the concept of osteopathic cranial manipulative medicine (OCMM) and craniosacral therapy. Previous research suggests that many individuals, from newborns to adults, present with single or multiple SBS strain patterns as a result of the birth process and other traumatic forces throughout life. To date, it is unclear whether specific SBS lesion patterns are present in altered health states and disease and how they compare with healthy controls. OBJECTIVE: To i

Spinal manipulation for low-back pain: a treatment package agreed to by the UK chiropractic, osteopathy and physiotherapy professional associations.

Trials of manipulative treatment have been compromised by, amongst other things, different definitions of the therapeutic procedures involved. This paper describes a spinal manipulation package agreed by the UK professional bodies that represent chiropractors, osteopaths and physiotherapists. It was devised for use in the UK Back pain Exercise And Manipulation (UK BEAM) trial--a national study of physical treatments in primary care funded by the Medical Research Council and the National Health Service Research and Development Programme. Although systematic reviews have reported some beneficial

Refractory torticollis after a fall.

Though multiple medical and psychiatric causes of torticollis have been described, cervical dystonias resulting from distant somatic dysfunctions have not. This article describes the treatment of a 62-year-old woman in whom refractory retrotorticollis of surmised pelvic etiology developed after a fall. Structurally, cervical dystonias have been addressed as problems that originate in the head and neck, but this limited view of the musculoskeletal component of torticollis may prevent physicians from directing osteopathic manipulative treatment to the underlying problem.

Effects of osteopathic treatment on pulmonary function and chronic thoracic pain after coronary artery bypass graft surgery (OstinCaRe): study protocol for a randomised controlled trial.

BACKGROUND: Coronary artery bypass graft surgery (CABG) is an effective and widespread coronary revascularisation technique, nevertheless there are a number of long-term postoperative complications from which patients can suffer. One year after CABG surgery pulmonary function is decreased by 12% and 30% of the patients suffer from chronic thoracic pain. To date and to our knowledge there are no effective treatments for these conditions. The aim of the present clinical trial is to explore the effectiveness of osteopathic treatment on these conditions. METHODS: The study is designed as a random

A pilot study on spatial changes in the maxilla caused by osteopathic therapy.

OBJECTIVES: A variety of theories on the pathogenesis of temporomandibular disorders (TMD) exists resulting in treatment approaches ranging from the fabrication of occlusal splints to alternative treatment modalities such as osteopathy. The goal of this pilot study was to investigate whether osteopathic treatment causes spatial changes in the maxilla. METHOD AND MATERIALS: Following ethics commission approval and informed patient consent, three patients diagnosed with TMD participated in this investigation. In addition to regular treatment, an individualized mandibular occlusal splint was fab

Blunt laryngeal trauma resulting in arytenoid dislocation and dysphonia.

We present a case of left arytenoid dislocation due to blunt laryngeal trauma causing a subsequent large granuloma formation resulting in dysphonia and stridor. The patient underwent emergency excision of the obstructive granuloma and speech therapy was started post-operatively. A few weeks after surgery, the granuloma started to recur and laryngeal manipulation by a specialist osteopath was performed. A few weeks after the conservative management, the recurred granuloma resolved completely and patient's voice improved remarkably. Dislocation of the arytenoid cartilage due to blunt trauma is r

Manipulative Therapies: What Works.

Manipulative therapies include osteopathic manipulative treatment and many other forms of manual therapies used to manage a variety of conditions in adults and children. Spinal manipulative therapy may provide short-term improvement in patients with acute or chronic low back pain, comparable with other standard treatments. When compared with oral analgesics, cervical manipulation and/or mobilization appears to provide better short-term pain relief and improved function in patients with neck pain. Manipulative therapies may be as effective as amitriptyline for treating migraine headaches and ca

Preventing progression from chronic to widespread pain and its impact on health-related quality of life: a historical cohort study of osteopathic medical care.

CONTEXT: It is generally acknowledged that osteopathic physicians take a holistic approach to patient care. This style may help prevent the progression of painful musculoskeletal conditions, particularly if combined with osteopathic manipulative treatment (OMT). OBJECTIVES: The study aimed to determine if osteopathic medical care lowers the risk of progression from localized chronic low back pain to widespread pain and lessens the impact of pain on health-related quality of life. METHODS: A historical cohort study was conducted within the Pain Registry for Epidemiological, Clinical, and Inte

Management of suppurative cervical lymphadenitis in a healthy 24-year-old man.

Unilateral suppurative cervical lymphadenitis is characterized by acute onset of 1 or more tender cervical lymph nodes and can lead to fever, cellulitis, abscess formation, and bacteremia. This form of lymphadenitis is usually caused by gram-positive bacteria. The present case details the treatment of a previously healthy 24-year-old man who presented with an acutely inflamed cervical lymph node. The patient did not respond to antibiotic monotherapy or combination antibiotics but recovered rapidly after methylprednisolone and osteopathic manipulative treatment were added to his care.

Osteopathic treatment of patients with shoulder pain. A pragmatic randomized controlled trial.

BACKGROUND: Shoulder complaints are common in the general population. Typically, the diagnosis of a specific pathology is lacking. The objective of this trial was to evaluate the effectiveness of an osteopathic treatment in patients suffering from shoulder pain. METHODS: A pragmatic randomized controlled trial was conducted in patients with a history of shoulder pain of 6 weeks to 12 months, and a pain intensity level of at least 40% on the visual analogue scale (VAS). Participants were identified from the general population in Germany and allocated by means of external randomization to an in
