A radical mastectomy was performed for a breast adenocarcinoma occurring 10 years after a subpectoral augmentation mammaplasty with silicone gel-filled prostheses. A foreign-body reaction to silicone was seen in two of the axillary lymph nodes resected.

BACKGROUND: Chemotherapy predisposes individuals to increased risk for future cardiac events.
METHODS: In order to verify the hypothesis that neoadjuvant chemotherapy may be a risk factor for perioperative cardiovascular events, 122 patients who underwent radical mastectomy with general anesthesia were separated into two groups, the N group (neoadjuvant chemotherapy group) and the C group (control group), according to neoadjuvant chemotherapy history. Augmentation mammoplasty has been said to reduce the risk for subsequent development of mammary gland carcinoma, but it may confer a worse prognosis should breast cancer develop. Nine patients who had breast cancer after augmentation mammoplasty were treated at the Ochsner Foundation Hospital and Clinic. The mean interval between augmentation and the development of a palpable mass was 6.8 years (range, 2 to 20 years). Four patients were treated with modified radical mastectomy, one was treated with radical mastectomy, and four were treated with tylectomy, axillary dissection, and radiotherapy.

The case of a 78-year-old man with Paget's disease of the breast, described herein, appears to be only the 22nd histologically proven case of this rare condition; in over 100 years less than 50 cases have been reported. Since this patient had no underlying tumour or nodal involvement the operation consisted of total mastectomy with removal of axillary nodes only, rather than the classical radical mastectomy. All eczematoid lesions of the areola of the breast that persist for more than 2 weeks or are unilateral should be considered Paget's disease until proven otherwise by biopsy.
