The majority of middle mediastinal cysts are benign diseases. However, some cases of middle mediastinal cysts are revealed to be malignant tumor. We herein report a case of a middle mediastinal cyst, which proved to be a right paratracheal lymph node metastasis from the thyroid papillary carcinoma. The patient was a 67-year-old lady who had underwent thyroidectomy and regional lymph node dissection for papillary thyroid carcinoma 6 years before. Follow-up computed tomography revealed a growing cyst at the right-side of the intrathoracic trachea. Metastasis from thyroid cancer was suspected fro…

We present a case of a baby girl with a congenital cystic mediastinal mass which was excised at 20 months of age and was found to be a developmental foregut cyst of the gastroenteric type. Within the cyst wall was an adrenal cortical rest. Adrenal cortical rests arising above the diaphragm are exceedingly rare and previous cases are reviewed.

Multilocular thymic cysts (MTC) are acquired multilocular cysts caused by inflammation. The rarity of such lesions and a lack of recognition make diagnosis and treatment difficult. Herein, we present our experience with a multilocular mediastinal cyst that resulted in the development of thymic cancer with metastasis over a period of 13 years. Computed tomography findings revealed an anterior mediastinal mass that was suspected to be an MTC in a 49-year-old man. The mass shrank gradually over a period of 7 years; however, growth was observed at 10 years after initial detection. At 13 years afte…

A 12-year-old adolescent boy presented with complaints of persisting dry cough and exertional dyspnea. A CT chest revealed a cystic lesion posterior to the trachea and a flexible bronchoscopy revealed extrinsic compression of the posterior trachea and partial obstruction of the left main bronchus. He underwent an Endoscopic Ultrasound with a Bronchoscope. A Hypoechoic cystic lesion was seen just in front of the esophagus. Fine needle aspiration yielded thick mucoid aspirate and cytology revealed ciliated columnar epithelium, confirming the diagnosis of a Bronchogenic cyst.

Surgical resection is usually indicated for symptomatic mediastinal tumors. However, surgery potentially increases postoperative complications and hospitalization length in patients who are elderly, in poor general condition, or have tumors located in the thoracic inlet. We present an 84-year-old female with progressive cough and dyspnea for 1 week. Simple radiogram and computed tomography scan showed a large superior mediastinal cyst, sized 8.3 × 6.1 × 4.6 cm, narrowing the trachea. Ultrasonography- and fluoroscopy-guided percutaneous sclerotherapy using alcohol and bleomycin was applied a si…

OBJECTIVE: To evaluate the feasibility of thoracoscopic surgery for congenital pulmonary airway malformation (CPAM) with ipsilateral mediastinal bronchogenic cysts in children. METHODS: From January 2019 to January 2025, our medical center performed a total of 462 surgeries on children with CPAM. Among them, 7 children who were diagnosed with CPAM with bronchogenic cysts underwent thoracoscopic surgery. A three-port approach via the lateral chest was adopted. Patients were placed in a lateral decubitus position (healthy side down). The observation hole is located at the junction of the scapula

We present a case of prenatal diagnosis of a mediastinal cyst by ultrasound, confirmed during infancy through computed tomography (CT) and magnetic resonance imaging (MRI). A 6-month-old male infant was admitted to the hospital. A mid-mediastinal cyst was detected through a province-wide fetal ultrasonography screening program. During the infancy stage, the CT scan and the MRI confirmed the finding of a mediastinal cyst. Based on the findings on prenatal ultrasonography and postnatal imaging studies (CT, MRI), a benign mediastinal cyst was suspected. The cyst was below the tracheal carina, and…

BACKGROUND: Although surgical resection of mediastinal bronchogenic cysts is considered the treatment of choice, there is increasing interest in minimally invasive approaches of management. The purpose of this study was to review the literature on the role of transbronchial needle aspiration (TBNA), either conventional or endobronchial ultrasound (EBUS)-guided, in the diagnosis and management of bronchogenic cysts.
METHODS: We systematically searched the PubMed and EmBase databases for studies (until July 2014) reporting TBNA of bronchogenic cysts. Data were recorded on a standard data extrac

INTRODUCTION: Mediastinal parathyroid cyst is a rare cystic disease that involves the parathyroid tissue within its walls. This case report is the first to document a mediastinal parathyroid cyst with recurrent laryngeal nerve paralysis and tracheal deviation that improved after surgical resection.
CASE PRESENTATION: A 47-year-old man experienced hoarseness and dyspnea upon exertion for 1 month. Computed tomography revealed a mediastinal cystic lesion with a maximum diameter of 78 mm, compressing the trachea. Laryngofiberscopy suggested long-term left recurrent laryngeal nerve paralysis. Tumo…

BACKGROUND: Mediastinal cystic lesions account for approximately 15-20% of all mediastinal masses and are difficult to differentiate because of similar imaging manifestation. The aim of this study was to differentiate mediastinum cystic lesions through endoscopic ultrasound-guided-fine needle aspiration (EUS-FNA) and parameters from cyst-fluid analysis.
METHODS: Over a period of eight years, 37 patients suspected with mediastinal cystic lesions were assessed. Cyst fluid was collected via EUS-FNA and further examined using cytological and biochemical techniques. Definitive diagnosis was establ…

BACKGROUND: Anterior mediastinal cysts (AMC) are often misdiagnosed as thymomas and undergo surgical resection, which caused unnecessary treatment and medical resource waste. The purpose of this study is to explore potential possibility of computed tomography (CT)-based radiomics for the diagnosis of AMC and type B1 and B2 thymomas.
METHODS: A group of 188 patients with pathologically confirmed AMC (106 cases misdiagnosed as thymomas in CT) and thymomas (82 cases) and underwent routine chest CT from January 2010 to December 2018 were retrospectively analyzed. The lesions were manually delinea…

Bronchogenic and other duplication cysts are congenital abnormalities that can present at any age including adulthood years. They are usually asymptomatic and discovered incidentally on radiological imaging of the chest. They are commonly treated by surgical resection. Recently, endobronchial ultrasound has been used to assist in diagnosis when radiologic imaging is not definitive. Endobronchial ultrasound has been used rarely to drain infected cysts, a rare complication of the bronchogenic cyst. We present a unique case of an infected large bronchogenic cyst treated with endobronchial ultraso

We report an unusual manifestation of hydatid disease, presenting as a progressively enlarging, painless swelling in the right infraclavicular region. Imaging studies revealed a large necrotic mass extending into the superior mediastinum with the destruction of the adjacent right sternoclavicular joint; this was initially suspected to be thyroidal in origin. However, histopathological examination was consistent with a ruptured hydatid cyst, and Echinococcus serology was positive. Due to anatomical considerations, surgical intervention was not pursued, and the patient was managed with oral anth

BACKGROUND: A mediastinal air cyst is a rare computed tomography (CT) finding. Once the lesion is identified, it is difficult to diagnose and treat. Meanwhile, bronchial diverticula have been reported as a CT finding observed in certain pulmonary pathologic conditions. We encountered the case of an enlarged mediastinal air cyst accompanied with bronchial diverticula and upper lobe-dominant fibrous changes of the lung.
CASE PRESENTATION: A 69-year-old man with a chronic cough who had regularly visited a chest physician for upper lobe-dominant pulmonary fibrosis was referred to our hospital for

A 26-year-old woman presented with a history of dyspnea. The patient declined a pregnancy test before undergoing a noncontrast CT scan of the chest that showed an anterior mediastinal mass that was F-18 FDG avid on a subsequent PET/CT. Additionally, PET/CT demonstrated focal uptake in the right adnexal region. Surgical excision of the anterior mediastinal mass revealed Hodgkin lymphoma. A contrast-enhanced CT scan of the chest, abdomen, and pelvis after surgery showed no residual tumor in the mediastinum. An enhancing 1.2-cm round lesion was noted in the right adnexal region. An ultrasound exa

INTRODUCTION: Surgical resection remains the gold standard for managing mediastinal cysts, including bronchogenic cysts, whereas needle aspiration serves as an alternative option that can facilitate preoperative volume reduction or, in certain selected cases, serve as a definitive treatment. However, it may lead to rare but potentially life-threatening complications such as mediastinitis; therefore, its indication should be carefully considered. This report details a case of a delayed intracystic hemorrhage 3 days after an endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA), requirin…

BACKGROUND: Bronchogenic cysts often require surgical excision in children because of the risk of progressive enlargement and complications. Although minimally invasive surgery is preferred, pediatric thoracic procedures remain technically challenging because of limited intrathoracic working space.
CASE DESCRIPTION: A 6-year-old girl with an asymptomatic mediastinal mass demonstrated interval growth of a cystic lesion in the right superior mediastinum on serial imaging. Single-port robotic resection was performed using the da Vinci SP system through a single intercostal incision with low-pres
