Cost of treating inpatient falciparum malaria on the Thai-Myanmar border.

BACKGROUND: Despite demonstrated benefits and World Health Organization (WHO) endorsement, parenteral artesunate is the recommended treatment for patients with severe Plasmodium falciparum malaria in only one fifth of endemic countries. One possible reason for this slow uptake is that a treatment course of parenteral artesunate is costlier than quinine and might, therefore, pose a substantial economic burden to health care systems. This analysis presents a detailed account of the resources used in treating falciparum malaria by either parenteral artesunate or quinine in a hospital on the Thai-

Less can be better: decomposing clinical data modalities in large language model-based healthcare applications.

OBJECTIVE: To systematically examine different clinical data modalities in large language models (LLMs) and multimodal large language models (MLLMs), and to quantify the contribution of data modalities in early inpatient risk prediction and decision support tasks. MATERIALS AND METHODS: We conducted a systematic analysis using MIMIC-IV, MIMIC-IV-Note, and MIMIC-CXR-JPG datasets to create a unified cohort of 22 254 hospital admissions containing structured electronic health records (EHRs), radiology reports (clinical notes), and chest X-ray images. We evaluated general-purpose and medical-adap

Missed Antibiotic Doses, Microbiology Diagnostic Results, and Antibiotic Prescribing Patterns at Discharge at Two Paediatric Tertiary Hospitals in Zambia: Implications for Antimicrobial Stewardship.

BACKGROUND/OBJECTIVES: Antimicrobial stewardship (AMS) is essential to optimize antibiotic use and limit antimicrobial resistance (AMR), particularly in pediatric populations where diagnostic uncertainty and system constraints complicate care. This study investigated critical AMS indicators, namely, missed antibiotic doses, microbiological diagnostics, and antibiotic prescribing at hospital discharge, which are key challenges that may affect treatment outcomes in hospitalized children under five years old. METHODS: A prospective descriptive study was conducted in two tertiary pediatric hospit

Is neonatal neurological damage in the delivery room avoidable? Experience of 33 levels I and II maternity units of a French perinatal network.

OBJECTIVE: To determine the frequency of avoidable neonatal neurological damage. STUDY DESIGN: We carried out a retrospective study from January 1st to December 31st 2003, including all children transferred from a level I or II maternity unit for suspected neurological damage (SND). Only cases confirmed by a persistent abnormality on clinical examination, EEG, transfontanelle ultrasound scan, CT scan or cerebral MRI were retained. Each case was studied in detail by an expert committee and classified as "avoidable", "unavoidable" or "of indeterminate avoidability." The management of "avoidable

Five-year vascular audit from a district hospital.

Vascular patients under the care of one surgeon over a 5-year period have been reviewed, from computer-based discharge records, with regard to in-hospital measures of outcome to determine whether or not surgery was being performed to an acceptable standard. Out of 466 procedures, 30 per cent of patients had at least one complication, 14 per cent had further surgery and 9 per cent died. Outcome was examined within the following treatment groups: aorto/iliac aneurysm, aortofemoral bypass, femoroproximal popliteal bypass, femorodistal popliteal bypass, femorocrural bypass, embolectomy and percuta

Are women with major depression in pregnancy identifiable in population health data?

BACKGROUND: Although record linkage of routinely collected health datasets is a valuable research resource, most datasets are established for administrative purposes and not for health outcomes research. In order for meaningful results to be extrapolated to specific populations, the limitations of the data and linkage methodology need to be investigated and clarified. It is the objective of this study to investigate the differences in ascertainment which may arise between a hospital admission dataset and a dispensing claims dataset, using major depression in pregnancy as an example. The safe u

Feasibility of point-of-care echocardiography by internal medicine house staff.

OBJECTIVE: To determine whether internal medicine house staff with limited training in echocardiography can use point-of-care echocardiography to make simple, clinically important diagnoses. BACKGROUND: Availability of small, portable ultrasound devices could make point-of-care echocardiography widely available. The training required to perform point-of-care echocardiography has not been established. METHODS: Medical house staff participated in a 3-hour point-of-care echocardiography training program. Patients scheduled for standard echocardiography as part of clinical care underwent point-o

Medication errors during medical emergencies in a large, tertiary care, academic medical center.

PURPOSE: Evaluate the rate, type and severity of medication errors occurring during Medical Emergency Team (MET) care at a large, tertiary-care, academic medical center. METHODS: A prospective, observational evaluation of 50 patients that required MET care was conducted. Data on medication use were collected using a direct-observation method whereby an observer documented drug information such as drug, dose, frequency, rate of administration and administration technique. Subsequently, a team of three clinicians assessed rate, type and severity of medication errors using definitions consistent

Sources and types of discrepancies between electronic medical records and actual outpatient medication use.

BACKGROUND: Accuracy and transportability of the recorded outpatient medication list are important in the continuum of patient care. Classifying discrepancies between the electronic medical record (EMR) and actual drug use by the root cause of discrepancy (either system generated or patient generated) would guide quality improvement initiatives. OBJECTIVES: To quantify and categorize the number and type of medication discrepancies that exist between the medication lists recorded in EMRs and the comprehensive medication histories obtained through telephone interviews conducted by a team of nur

A pharmacovigilance program from laboratory signals for the detection and reporting of serious adverse drug reactions in hospitalized patients.

The detection and reporting of serious adverse drug reactions (SADRs) have become important components of monitoring and evaluation activities performed in hospitals. We present the implementation of a prospective pharmacovigilance program based on automatic laboratory signals (ALSs) at a hospital. We also report the general findings after the first year of operation of the program, which involved ALSs that indicate various SADRs: agranulocytosis, aplastic anemia, liver injury, thrombocytopenia, hyponatremia, and rhabdomyolysis. The number of hospitalizations during the year was 54,525, and 1,

Age-specific differences in patient safety incidents: focus on falls and medication using the Korean patient safety incident reporting and learning system (2016-2023).

BACKGROUND: Patient safety incidents (PSIs) pose a significant global healthcare challenge, especially for vulnerable populations such as children and young adults (CYAs, aged 0–19 years), middle-aged adults (MAs, aged 20–60 years), and older adults (OAs, aged 20–60 years). Falls and medication errors are common hospital-based incidents with serious health impacts. Targeted prevention strategies are needed due to differing age-related risks. Therefore, we aimed to analyze the characteristics and key risk factors of falls and medication incidents across different age groups using 8 years (2016–

Evaluation of a drug deactivation system for safe disposal of opioids after ambulatory surgery.

BACKGROUND: Prescription opioids are commonly prescribed postoperatively and increase the risk of diversion or misuse when left unused and accessible. Despite awareness of the risks associated with unused opioids, harm reduction strategies like safe storage and drug take-back events may be limited by inconvenience and patient-specific barriers to access. OBJECTIVE: To evaluate a quality improvement project designed to facilitate at-home disposal of unused opioids after day surgery. METHODS: An observational, prospective quality improvement project was conducted in patients undergoing electiv

Drug use inefficiency: a hidden source of wasted health care dollars.

UNLABELLED: A potential area for departmental savings is to minimize inefficient use of pharmaceuticals. We recorded drug waste data for multiple drugs for a fiscal year and surveyed providers' knowledge of departmental drug waste. Six large-cost or large-volume use drugs were chosen for study: thiopental, succinylcholine, rocuronium, atracurium, midazolam, and propofol. Amounts administered to patients were collected for one year by using a computerized anesthesia record keeper. Total drug distributed was the number of vials restocked by pharmacy for the year. An efficiency index, the percent

[The establishment and development of the Beijing-Soviet Union Red Cross Hospital (1952-1957)].

The national healthcare system was underdeveloped with weak medical technologies and outdated medical industrial norms, urgently requiring institutional reform in the early days of the New China. The Beijing-Soviet Union Red Cross Hospital built on the foundation of the Sino-Soviet friendship and alliance (1952-1957) became a model for comprehensive hospital development in China at that time, as the first landmark achievement of Sino-Soviet medical cooperation. It received full-spectrum Soviet support in medical facilities, core technologies and expert teams. It introduced scientific hospital

Sexually dimorphic effects of a modified adolescent social isolation paradigm on behavioral risk factors of alcohol use disorder in Long Evans Rats.

BACKGROUND: While rapid response systems have been widely implemented, their impact on patient outcomes remains unclear. Further understanding of their components-including medical emergency team triggers, medical emergency team member composition, additional roles in patient care beyond responding to medical emergency team events, and their involvement in "Do-Not-Resuscitate" order placement-may elucidate the relationship between rapid response systems and outcomes. OBJECTIVE: To explore how recent studies have examined rapid response system components in the context of relevant adverse pati

Development and Application of a Nurse-Led Clinical Decision Support System for Safe Intravenous Medication Administration: A Nonrandomized Controlled Trial.

BACKGROUND: Intravenous medication administration is a high-risk clinical procedure, where medication errors can lead to adverse consequences. Evidence-based clinical practice guidelines provide recommendations for the administration and monitoring of intravenous infusions. These guidelines are being increasingly integrated into clinical decision support systems (CDSS). The development of CDSS should emphasize nurses as core users, closely align with their clinical workflows, and ultimately create practical, user-friendly tools through thoughtful interface design, functional logic, and intelli
