Grading of complications and risk factor evaluation in laparoscopic colorectal surgery.

BACKGROUND: A grading system for postoperative complications is important for quality control and comparison among investigations. The objective of the current study was to evaluate complications associated with laparoscopic colorectal surgery according to a standardized grading system, and to examine risk factors associated with different complication grades. METHODS: Data of all patients who underwent elective laparoscopic colorectal surgery at two medical centers between September 2003 and January 2011 were collected prospectively. Complications were graded retrospectively into five catego

Are All Periprosthetic Joint Infections the Same? Evaluating Major vs Minor Criteria.

BACKGROUND: The diagnosis of periprosthetic joint infection (PJI) can be made when 1 major criterion or 3 of 5 minor criteria are present. However, the outcomes of patients with a major vs minor criteria for diagnosis have not been studied. The objective of this study was to evaluate if a difference in outcome of surgical intervention existed between patients with PJI who were diagnosed with a major criterion or a combination of minor criteria. METHODS: A retrospective chart review identified 277 primary total hip or knee arthroplasty patients who had developed PJI based on the International

Epidermal Inclusion Cyst after Carpal Tunnel Release: A Case Report.

Background Carpal tunnel release is one of most commonly performed procedures in upper extremity surgery, with low complication rates. Case Description We report a case of epidermal inclusion cyst that occurred in a 44-year old man after carpal tunnel release. The epidermal inclusion cyst formed beneath the previous operation scar and induced recurrent carpal tunnel syndrome. Symptoms resolved completely after excision of the cyst. Literature Review Epidermal inclusion cysts are the third most common type of hand tumors. Although repeated minor trauma is the most frequent cause, surgical proce

[Early postoperative complications in carotid surgery].

Carotid endarterectomy has a documented stroke preventing effect in symptomatic high-grade carotid stenosis when the rate of serious complications is below 6%. In the period 1986-96, 160 procedures were performed on 150 patients at the Department of Neurosurgery, Ullevål Hospital. Oslo. The indication for surgery was hemispheric transient ischemic attack and/or amaurosis fugax in 137 patients and/or minor stroke in 49 in addition to a high-grade stenosis. Mean age was 61 years (42-74 years). One patient (0.6%) died four hours after surgery. Three patients (1.9%) suffered disabling postoperativ

[Gynecologic interventions in the 10th decade of life].

We report on 28 surgical procedures performed between 1978 and 1992 performed on patients aged 90 years or older. 18 of them had a malignant tumour (11 mamma, 2 corpus, 3 vulva and 2 ovarian cancer), one had an ovarian cystoma. Three patients suffered from pelvic relaxation, four had postmenopausal bleeding, one vulvar dystrophy and one patient suffered from pyometra. Four laparotomies, three vaginal hysterectomies with colporrhaphy, four minor operations of the vulva, six curettages and eleven mammary operations were performed. During the surgical procedures, none of the patients died, wherea

Updated experience with the Monti catheterizable channel.

OBJECTIVES: The Monti catheterizable channel is used as an integral part of continent bladder reconstruction in children. We have updated our ongoing experience at Riley Children's Hospital with 199 patients. METHODS: We identified 199 patients for retrospective review, including all patients for whom a Monti ileovesicostomy was created from January 1997 to August 2004. We assessed the complications, surgical procedures, and stomal continence. RESULTS: At mean follow-up of 28 months, we found that 194 of 199 patients (97.5%) continued to use their Monti catheterizable channel for bladder dra

Perioperative Management of Patients Taking Direct Oral Anticoagulants: A Review.

IMPORTANCE: Direct oral anticoagulants (DOACs), comprising apixaban, rivaroxaban, edoxaban, and dabigatran, are commonly used medications to treat patients with atrial fibrillation and venous thromboembolism. Decisions about how to manage DOACs in patients undergoing a surgical or nonsurgical procedure are important to decrease the risks of bleeding and thromboembolism. OBSERVATIONS: For elective surgical or nonsurgical procedures, a standardized approach to perioperative DOAC management involves classifying the risk of procedure-related bleeding as minimal (eg, minor dental or skin procedure

Fatal group A streptococcal infection with toxic shock syndrome: complicating minor orthopedic trauma.

Since 1987, reports have appeared of a streptococcal toxic shock syndrome in various clinical settings. None have appeared in the orthopaedic literature. Between 1989 and 1991 at our institution three patients with relatively minor orthopaedic injuries or procedures died of group A streptococcal infections complicated by toxic shock syndrome. The manifestations of this syndrome included rapid progression of systemic sepsis, necrotizing soft-tissue infections, acute renal failure, adult respiratory distress syndrome, and coagulopathy. All three patients died despite aggressive resuscitative mea

Insights Into the Assessment of WHO-Recommended Practices Based on Surgical Operations in Tertiary Healthcare Settings.

BACKGROUND: The protection of surgical safety comprises two major elements that produce the best outcomes for patients. The World Health Organization (WHO) created a surgical practice-based assessment for operative procedure-related risks. Medical professionals have proven the effectiveness of this checklist to eliminate both adverse outcomes and medical complications caused by surgical negligence. METHODOLOGY: A study was conducted on 250 surgeries (major and minor) in a tertiary healthcare setting based on a qualitative questionnaire, adapted from the WHO checklist, operated through Google

Not Just Another Trigger Finger.

BACKGROUND: Open A1 pulley release for trigger finger has generally been considered a minor procedure with infrequent complications. Most reported complications are minor, including scar pain and tenderness, mild extension lag, and recurrence of triggering. Rates of major complications, such as bowstringing, neurovascular bundle injury, and infection requiring reoperation, are less than 1% to 4%. We aimed to describe the potentially devastating sequelae of these major complications and the subsequent consequences. METHODS: Three patients underwent open trigger finger release, which were all c

Epistaxis.

Epistaxis is a common occurrence. The estimated lifetime incidence of epistaxis is approximately 60% and it ranges in severity from a minor nuisance to a life-threatening hemorrhage. Evaluation of recurrent or severe cases includes a search for underlying causes, such as bleeding disorders and neoplasia. Many techniques, materials, and procedures treat nasal bleeding effectively and sometimes more than one treatment must be used. Otolaryngologists must be prepared to deal with severe or refractory bleeding through the use of medications, packing materials, and radiologic or surgical interventi

Minimally Invasive Surgical Techniques for the Treatment of Dogs with Urinary Incontinence Due to Ureteral Ectopia.

Ectopic ureters are uncommon congenital abnormalities in dogs, leading to urinary incontinence. Ectopic ureters can be intramural or extramural. Intramural is usually the more prevalent form. Female dogs are affected more often than male dogs. This disease requires the surgical correction of the ureters. Three surgical techniques have been described in this regard: neoureterostomy, nephroureterectomy, and ureteroneocystostomy. In this study, we present minimally invasive surgical procedures for treating extra- and intramural ectopic ureters. Sixteen client-owned dogs with clinical signs of uri

Value and relevance of routine postoperative blood sample analysis after general surgery-a single centre analysis of 1198 patients.

BACKGROUND: Routine blood sample analysis is performed in most surgical institutions after general surgery on the first postoperative day. Substantial economical impact is thereby generated in addition to patient distress caused by venous puncture. AIM: The presented study was designed to analyse the relevance of postoperative routine blood sampling with special focus on patient safety. METHODS: 1198 patients undergoing minor general surgical procedures (appendectomy, cholecystectomy, groin hernia repair) at our institution were retrospectively analysed. Data was gathered and forwarded to st

Management of severe hypospadias.

Many classifications of hypospadias have been published, mainly based on the position of the ectopic meatus, which is an insufficient criterion to define the severity of this malformation. What really marks the proximal landmark of this malformation is the level of division of the corpus spongiosum, which is always proximal to the ectopic meatus. In this article, we will focus on the most severe forms of hypospadias which include those with a proximal division of corpus spongiosum (below the midshaft), important chordee and a poor development of the ventral radius, reflecting a marked hypoviri

Toe Amputation After Minor Surgery in a Patient with Behçet's Disease: A Case Report.

Behçet's disease is a systemic autoimmune vasculitis. Although various clinical findings can be observed depending on the pathologic features caused by the blood vessels involved, the classic triad of the disease includes oral aphthae, genital ulcers, and uveitis. Although complications involving the aorta or the vena cava inferior can prove fatal, thrombophlebitis in the superficial veins of the lower extremities are more commonly observed. Some patients can remain asymptomatic for a long period after the diagnosis. In patients with positive pathergy test findings, trauma can trigger the infl

[Hemostasis in patients at high hemorrhagic risk].

Surgical treatment in patients with heavy coagulation disorders needs measures that minimize immediate and post-surgical hemorrhagic risk. Besides the subjects afflicted with constitutional coagulation pathologies such as haemophilia A, B, C, AB, Von Willebrand disease, platelet disorders and others, too many are the patients that need pharmacological therapy to impede the creation of intra-vascular coagula. Today, surgical treatment of these patients is possible with the use of human fibrin glue (Tissucol) that permits excellent surgical and post-surgical hemostasis without therapy suspension
