To assess the rate and predictors of complications in patients with mitral valve prolapse (MVP), 316 subjects (mean age 42 +/- 15 years) with echocardiographic MVP were followed prospectively for a mean of 102 months: 220 (70%) were women, 225 (71%) had clinically recognized MVP, and 91 (29%) were detected in family studies. Mitral valve prolapse should be considered a benign condition as there are few complications. The natural history is not significantly different to that of a matched control population. There are potentially severe forms of the condition which are mainly observed in specialised centres and this introduces a bias in patient selection when compared with the general population. This paper briefly reviews the anatomy, epidemiology and clinical aspects of the Mitral Valve Prolapse (MVP), and focuses attention on the association between infective endocarditis and prolapse, particularly after oral surgery. It is recommended that all MVP patients, irrespective of whether they have mitral regurgitation or not, receive correct prophylactic treatment. The pharmacological approach suggested, based on the most recent advances in antibiotic pharmacodynamics, is designed in accordance with specific patient risks.

The authors describe a case of sudden death (SD) of a patient with unaltered coronary arteries, myxomatous degeneration and mitral valve prolapse (MVP). The case was marked by an isolated prolapse of the anterior cusp of the mitral valve because of its congenital structural abnormalities, which was not encountered previously in MVP patients who died suddenly. We report on 10 patients suffering from cerebral ischemic episodes in whom mitral valve prolapse was diagnosed. Neurologic assessment included computerized tomography and angiographic study. In one case, the angiographic study revealed the presence of intracranial vasculitis and one woman was taking the contraceptive pill. The diagnosis of mitral valve prolapse was established on the basis of clinical grounds and electrocardiographic, Doppler study, Holter monitoring and two-dimensional echocardiographic signs.
