Personalized and typical concurrent risk of limitations in social activity and mobility in older persons with multiple chronic conditions and polypharmacy.

PURPOSE: We define personalized concurrent risk (PCR) as the subject-specific probability of an index outcome within a defined interval of time, while currently at risk for a separate outcome, where the outcomes are not mutually exclusive and can be jointly modeled with a shared random intercept. We further define typical concurrent risk as the risk obtained by setting the random intercept to null. METHODS: Drawing data from the Medical Expenditure Panel Survey (cohorts 2008-2013), we jointly model limitations in social activity and mobility over two years among older community-dwelling perso

Body mass index and mobility limitations: An analysis of middle-aged and older Black, Hispanic, and White women in the U.S.

INTRODUCTION: While the Body Mass Index (BMI) did not change significantly for men from 2005 to 2014 in the United States, women exhibited an upward linear trend. Hispanic and Black women, in particular, showed a dramatic increase. Therefore, the objective of this study was to examine the association between BMI and mobility limitations for non-institutionalised middle-aged and older Black, Hispanic, and White women. METHODS: The International Classification of Functioning, Disability, and Health model was applied to a sample of 2865 Black, 1846 Hispanic, and 9721 White women categorised as m…

Functional mobility limitations and falls in assisted living residents with dementia: physical performance assessment and quantitative gait analysis.

BACKGROUND AND PURPOSE: The prevalence of functional mobility limitations and falls is higher in people with dementia compared with cognitively healthy older adults, and both are associated with gait and motor impairments. The aims of this study were to examine concurrent validity of physical performance assessments and spatiotemporal gait measures in older adults with advanced dementia and to prospectively examine their relationship to functional mobility limitations and falls over a 4-month period. METHODS: Thirty-one older adults living in dementia-specific assisted living residences parti…

Fatigue, fear of being mobilized and residual limb pain limit independent basic mobility and physiotherapy for patients early after major dysvascular lower extremity amputation: A prospective cohort study.

AIM: Early mobilization of patients with a major lower extremity amputation (LEA) is often a challenge because of lack of compliance. Therefore, we investigated factors limiting independent mobility and physiotherapy on the first day with physiotherapy (PTDay1) and the following 2 days after LEA. METHODS: A total of 60 consecutive patients, mean age 73.7 years (SD 12.1 years), undergoing LEA were included over a period of 7 months. The Basic Amputee Mobility Score was used to assess basic mobility. Predefined limitations for not achieving independent mobility or not completing physiotherapy w

Mobility Limitations and Self-Perceived Unmet Dental Needs Among Korean Adults: A Nationwide Multilevel Analysis for Integrated Care.

INTRODUCTION AND AIMS: Self-perceived unmet dental needs reflect access barriers and inequities. In rapidly aging Korea, mobility limitations may exacerbate these barriers, yet evidence disentangling individual, household, and district-level influences is scarce. We examined the association between mobility limitations and self-perceived unmet dental needs and assessed contextual variation. METHODS: We analyzed nationally representative data from the 2024 Korea Community Health Survey (KCHS) for adults aged ≥45 years (n = 167,474). Multilevel logistic regression with district random intercept…

Dimethyl carbonate as a green solvent in ternary solvent systems for liquid chromatography: Investigation of miscibility limits and mobile phase viscosities.

The extensive use of organic solvents in high-performance liquid chromatography (HPLC) contributes to a considerable environmental impact, driving the search for more sustainable solvents. In this context, dimethyl carbonate (DMC) has recently emerged as a promising, greener organic modifier due to its low viscosity and high elution strength. However, its limited miscibility with water requires the use of a ternary solvent. As comprehensive physicochemical data on such systems remain limited, the present study investigates the miscibility and viscosity behaviour of ternary mobile phase systems

A Theory-Based Approach to Adapting a Task-Oriented Community Exercise Programme for Virtual Delivery for People With Balance and Mobility Limitations.

Purpose: Despite the health benefits for people with mobility limitations of participating in community-based exercise programmes (CBEPs), accessing and implementing in-person CBEPs can be challenging. We describe a theory-based approach to adapting a group task-oriented, in-person CBEP targeting balance and mobility to a virtual format. Method: We used the Knowledge-to-Action and Medical Research Council frameworks to guide adaptation of an established CBEP for virtual delivery. We undertook consultations with knowledge users (i.e., CBEP providers, health care professionals, managers, policy-…

Digital limited interaction efficacy trial of LifeSkills Mobile to reduce HIV incidence in young transgender women: study protocol.

BACKGROUND: Young transgender women are disproportionately impacted by HIV in comparison to other risk groups in the United States. Despite research documenting this vulnerability, few interventions have been developed and tested to reduce their risk and none to evaluate effects on HIV incidence. The LifeSkills Mobile app addresses the specific structural, developmental, and interpersonal challenges to HIV prevention among young transgender women ages 16-29 and is adapted from LifeSkills, a group-based intervention with evidence of efficacy to reduce sexual risk for HIV. The LifeSkills Mobile

Neighborhood Social Cohesion and Mobility Limitations Among Community-dwelling Older Americans: The Mediating Roles of Depressive Symptoms and Mastery.

Neighborhood environment plays an important role in late-life health; yet, the social aspect of neighborhood environment and its impact on mobility limitations have rarely been examined. This nonexperimental, cross-sectional study examines the relationship between neighborhood social cohesion and mobility limitations and the potential mediators (i.e., depressive symptoms, mastery) of this relationship. A total of 8,317 Americans aged 65 years and older were selected from the Health and Retirement Study. Using ordinary least squares regressions, this study shows that neighborhood social cohesio…

Spinal PA movements behave 'as if' there are limitations of local segmental mobility and are large enough to be perceivable by manual palpation: A synthesis of the literature.

BACKGROUND: Posterio-anterior (PA) movements are one type of passive intervertebral movement used to assess and treat perceived deficits in localized segmental mobility. OBJECTIVES: To describe: 1) The specific effects that reductions in segmental mobility would be expected to have on PA movements; 2) How differences in PA movements in clinical situations compare to what would be expected with reduced segmental mobility; and 3) Whether such differences in PA movements are likely to be perceivable by manual palpation. METHODS: Multiple modelling studies and in vivo measurements of PA movement

The Role of Neighborhood Safety in Recovery from Mobility Limitations: Findings from a National Sample of Older Americans (1996-2008).

Prior research has documented a link between perceived neighborhood safety and functional limitations including incident mobility limitation, yet no research has explored the association between perceived neighborhood safety and recovery from functional limitations. This study investigates whether perceived neighborhood safety independently predicts recovery. Using longitudinal data from the Health and Retirement Study (HRS) (1996-2008), discrete-time event history models with multiple competing events were estimated using multinomial logistic regression. Robust standard errors were used to ad…

Design and development of a telerehabilitation self-management program for persons with chronic lower limb swelling and mobility limitations: preliminary evidence.

This paper describes design and development of a self-management program, delivered by telerehabilitation (TR), to address the problem of chronic lower limb swelling in persons with limited mobility. The 18.6 million persons with limited mobility in the USA are at increased risk for chronic lower limb swelling and related secondary complications, including cellulitis and skin ulcers. Over time, chronic swelling often progresses to lymphedema, an incurable condition requiring lifelong care. Without successful self-management, lymphedema and its related complications inevitably worsen. Access an
