The epidemiology of molar pregnancy and choriocarcinoma.

The available evidence suggests quite strongly that a hydatidiform mole arises as a consequence of production of a defective ovum. That the defect is maternal is implied by the risk associated with increasing maternal (but not paternal) age, by the recurrence in some women of multiple moles (even in one instance by different husbands), and by reports of familial aggregation of hydatidiform moles among sisters. The absence of maternal genetic material in most complete hydatidiform moles and the dispermic fertilization of some complete and most partial moles further suggests that the ovum is def

Partial molar pregnancy as ruptured tubal ectopic.

Tubal molar pregnancy is extremely rare, and less than 200 cases have been reported in the literature. The incidence is approximated at 1.5 per 1 000 000 pregnancies. We report a case of ruptured tubal pregnancy in a woman, whose postoperative histopathology diagnosis showed partial molar pregnancy in the ruptured fallopian tube. The presence of abnormal, non-polar trophoblast proliferation, which is circumferential with vacuolation, along with sheets of pleomorphic extravillous trophoblast, is the main diagnosing feature, and it carries the risk of malignant potential.

Molar-Incisor Hypomineralisation: Severity, caries and hypersensitivity.

OBJECTIVES: To investigate distribution of affected teeth and severity of molar-incisor hypomineralisation (MIH) in 8-9-year-old children. A second aim was to study association between severity of MIH and hypersensitivity, caries, and affection of incisors and second primary molars (SPM). METHODS: A total of 3013 children in one age cohort participated in a cross-sectional study, of which 851 children were diagnosed with MIH. A majority of these children were re-examined and MIH diagnosis based on the European Academy of Paediatric Dentistry criteria was confirmed in 538 children. The re-exam…

Decoding the Genetics of Recurrent Molar Pregnancy.

Hydatidiform mole is a condition characterised by abnormal trophoblastic hyperplasia and failure of embryonic tissue development. The risk of recurrence is seen to be associated with biallelic maternal mutations in NLRP7, KHDC3 L and PAD16 genes. Women with such mutations have a major risk of reproductive failure and normal pregnancy is seen in only 1.8%. We report the case of a 31-year-old woman with previous three molar pregnancies who on genetic testing was found to be compound heterozygous for pathogenic variants in the NLRP7 gene (c.2738A>G and c.2078G>C). Accordingly, the woman was couns…

Tubal ectopic molar pregnancy: A case report.

BACKGROUND: Ectopic molar pregnancy (EMP) is a rare form of gestational trophoblastic disease that occurs when a hydatidiform mole implants outside the uterus. CASE PRESENTATION: We describe a 35-yr-old woman with mild abdominal pain, delayed menstruation for 2 months, and high beta-human chorionic gonadotropin levels. Sonography revealed a heterogeneous hyperechoic mass in the left adnexa and fluid in the endometrial cavity, suggestive of a tubal EMP. She underwent endometrial curettage and left salpingectomy. Pathology confirmed the diagnosis of invasive hydatidiform mole/left tubal EMP. Th…

Impacted Mandibular First Molar: A Rare Riddle.

Tooth impaction incidence is in the range of 5.6 to 18.8% of the population. Eruption failure of the first permanent molar is very rare; the prevalence is 0.01% of the population. The permanent molars are of particular importance in providing adequate occlusal support as well as coordinating facial growth. Failure of the eruption of permanent molars might lead to an array of complications like a reduction in the vertical dimension, extrusion of the antagonist teeth, a posterior open bite, inclination and resorption of adjacent teeth, and cyst formation. Various treatment modalities for impacte…

Unexpected variation of human molar size patterns.

A tenet of mammalian, including primate dental evolution, is the Inhibitory Cascade Model, where first molar (M1) size predicts in a linear cline the size and onset time of the second (M2) and third (M3) molars: a larger M1 portends a progressively smaller and later-developing M2 and M3. In contemporary modern Homo sapiens, later-developing M3s are less likely to erupt properly. The Inhibitory Cascade Model is also used to predict molar sizes of extinct taxa, including fossil Homo. The extent to which Inhibitory Cascade Model predictions hold in contemporary H. sapiens molars is unclear, inclu…

Management of uterine rupture during molar pregnancy.

Gestational trophoblastic disease (GTD) is rare and encompasses several clinicopathologic forms from pre-malignant to malignant disorders. Clinical presentation is most of the time dominated by vaginal bleeding. Only few cases of uterine rupture during GTD have been reported in literature. We present the case of a female patient admitted to the hospital for hemorrhagic shock secondary to a uterine rupture due to an undiagnosed GTD. After an emergency laparoscopy, the patient underwent total hysterectomy with bilateral salpingectomy and bilateral ovarian cystectomy. Pulmonary metastasis were di

Thyrotoxicosis: a rare presentation of molar pregnancy.

A 49-year-old woman, G8P7, presented with 1 week of worsening vaginal bleeding and abdominal cramps in the setting of a recently discovered unplanned pregnancy. Vaginal ultrasound findings and a significantly elevated human chorionic gonadotropin (hCG) level were concerning for molar pregnancy. She developed signs of hyperthyroidism on the night of admission, for which the endocrinology team was consulted. Laboratory data were consistent with hyperthyroidism. The patient was believed to have thyrotoxicosis secondary to molar pregnancy with concern for impending thyroid storm. Her mental health…

Silver-Based SERS Pico-Molar Adenine Sensor.

Adenine is an important molecule for biomedical and agricultural research and applications. The detection of low concentration adenine molec … M adenine in a pH 9 aqueous solution is feasible, making the novel SERS structure a desirable pico-molar adenine sensor.

Invasive molar pregnancy in rudimentary uterine horn.

Incidence of molar pregnancy is 1-3/1000 pregnancies. Invasive mole is a local invasive form of gestational trophoblastic neoplasias which is mostly seen in reproductive age and usually follows a molar pregnancy and rarely has an initial presentation. Ectopic pregnancy in rudimentary uterine horn is extremely rare and is seen in 1/100,000 - 140,000 pregnancies. Invasive mole has seldom been reported in ectopic localizations but not in a patient with Müllerian duct anomaly. Here we represent a case of invasive mole in a reproductive age patient with unicornuate uterus and rudimentary communicat…
