Advances in Molecular Imaging for the Early Detection and Management of Alzheimer's Disease.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by impairments in memory and cognitive abilities. The development of new immunotherapies targeting beta-amyloid (Aβ) and tau protein deposition in the brain is ushering in great advances in clinical management. Advances in molecular imaging techniques, particularly positron emission tomography (PET) with amyloid and tau tracers, have facilitated the early detection of Alzheimer's pathology. We report a case of a 67-year-old patient presenting with mild cognitive impairment (MCI) who was confirmed to have AD on n…

Integration of molecular imaging in the personalized approach of patients with adrenal masses.

Adrenal masses are a frequent finding in clinical practice. Many of them are incidentally discovered with a prevalence of 4% in patients undergoing abdominal anatomic imaging and require a differential diagnosis. Biochemical tests, evaluating hormonal production of both adrenal cortex and medulla (in particular, mineralocorticoids, glucocorticoids and catecholamines), have a primary importance in distinguishing functional or non-functional lesions. Conventional imaging techniques, in particular computerized tomography (CT) and magnetic resonance imaging (MRI), are required to differentiate bet

Molecular imaging of schizophrenia: Neurochemical findings in a heterogeneous and evolving disorder.

The past four decades have seen enormous efforts placed on a search for molecular markers of schizophrenia using positron emission tomography (PET) and single photon emission computed tomography (SPECT). In this narrative review, we cast a broad net to define and summarize what researchers have learned about schizophrenia from molecular imaging studies. Some PET studies of brain energy metabolism with the glucose analogue FDGhave have shown a hypofrontality defect in patients with schizophrenia, but more generally indicate a loss of metabolic coherence between different brain regions. An early…

Prognostic value of molecular and imaging biomarkers in patients with supratentorial glioma.

PURPOSE: We evaluated the relationship between 11C-methionine PET (11C-METH PET) findings and molecular biomarkers in patients with supratentorial glioma who underwent surgery. METHODS: A consecutive series of 109 patients with pathologically proven glioma (64 men, 45 women; median age 43 years) referred to our Institution from March 2012 to January 2015 for tumour resection and who underwent preoperative 11C-METH PET were analysed. Semiquantitative evaluation of the 11C-METH PET images included SUVmax, region of interest-to-normal brain SUV ratio (SUVratio) and metabolic tumour volume (MTV).

The Latest Developments of Functional Molecular Imaging in the Diagnosis of Primary Aldosteronism.

Differentiation of unilateral from bilateral aldosterone hypersecretion is the essential step in the clinical practice of primary aldosteronism (PA). Although adrenal venous sampling (AVS) has been established as the most standard test recommended by the guideline, its invasive and technically difficult nature has facilitated the approach to develop non-invasive functioning imaging as an alternative test. Compared to the conventional adrenocortical scintigraphy with cholesterol derivatives as tracer, the first-generation imaging, both of 11C-MTO/PET and 123I-IMTO/SPECT/CT, the second-generatio

Image reconstruction for synchronous data acquisition in fluorescence molecular tomography.

The present full-angle, free-space fluorescence molecular tomography (FMT) system uses a step-by-step strategy to acquire measurements, which consumes time for both the rotation of the object and the integration of the charge-coupled device (CCD) camera. Completing the integration during the rotation is a more time-efficient strategy called synchronous data acquisition. However, the positions of sources and detectors in this strategy are not stationary, which is not taken into account in the conventional reconstruction algorithm. In this paper we propose a reconstruction algorithm based on the

Imaging molecular interaction of NO on Cu(110) with a scanning tunneling microscope.

Molecular interaction on metal surfaces is one of the central issues of surface science for the microscopic understanding of heterogeneous catalysis. In this Personal Account, I review the recent studies on NO/Cu(110) employing a scanning tunneling microscope (STM) to probe and control the molecule-molecule interaction on the surface. An individual NO molecule was observed as a characteristic dumbbell-shaped protrusion, visualizing the 2π* orbital. By manipulating the intermolecular distance with the STM, the overlap of the 2π* orbital between two NO molecules was controlled. The interaction c

[In-vivo imaging for pursuing molecular cascades in the pathogenesis of neurodegenerative diseases].

Nonclinical and clinical evidences have supported the view that accumulations of neurotoxic amyloid components initiate chain reactions of molecular and cellular pathologies, eventually leading to neuronal death and symptomatic onsets of neurodegenerative diseases. As this amyloid-triggered cascade is virtually composed of bidirectional causalities between upstream and downstream events, it is of critical significance to monitor all key processes, including amyloidosis, neuroinflammation, disrupted calcium homeostasis and impaired neurotransmissions, in living brains toward therapeutic regulat

Molecular imaging (SPECT and PET) in the evaluation of patients with movement disorders.

In this article the role of molecular imaging with SPECT and PET in patients with movement disorders is reviewed. It is mentioned that SPECT and PET imaging with cocaine analogues ((123)I-beta-CIT,(123)I-FP-CIT, (18)F-DOPA), radioligands labeling the presynaptic dopamine transporters, is of value for the differentiation of patients with PD or Parkinson-plus syndromes with individuals with essential tremor. In addition the clinical impact of this procedure, the role of molecular imaging in the preclinical diagnosis and in the follow-up of patients with PD, as well as, in the differential diagno…

Molecular imaging for evaluation of synovitis associated with osteoarthritis: a narrative review.

Recent evidence highlights the role of low-grade synovial inflammation in the progression of osteoarthritis (OA). Inflamed synovium of OA joints detected by imaging modalities are associated with subsequent progression of OA. In this sense, detecting and quantifying synovitis of OA by imaging modalities may be valuable in predicting OA progressors as well as in improving our understanding of OA progression. Of the several imaging modalities, molecular imaging such as positron emission tomography (PET) and single-photon emission computed tomography (SPECT) has an advantage of visualizing the ce…

PET detectives: Molecular imaging for phaeochromocytomas and paragangliomas in the genomics era.

Phaeochromocytomas and paragangliomas (PPGLs) are rare tumours that arise from the adrenal medulla or extra-adrenal sympathetic or parasympathetic paraganglia. Recent advances in genetics have greatly enhanced understanding of the pathogenesis and molecular physiology of PPGL. Concomitantly, advances in molecular imaging mean four techniques are now available for use in PPGLs: [123 I]-MIBG coupled with SPECT/CT; [18 F]- FDG, [68 Ga]-DOTATATE and [18 F]-FDOPA coupled with PET/CT. Each modality relies on unique cellular uptake mechanisms that are contingent upon the tumour's molecular behaviour-…

The role of molecular imaging in assessing degenerative parkinsonism - an updated review.

Diagnosing degenerative forms of parkinsonism still relies on a thorough clinical assessment, which in Parkinson's disease involves the presence of an asymmetric bradykinesia with rest tremor and/or rigidity that respond substantially to levodopa. Conversely, atypical forms, including multiple system atrophy, progressive supranuclear palsy and corticobasal degeneration, exhibit additional features (cerebellar or pyramidal signs, early postural instability), a poor response to dopamine replacement therapy and a bad prognosis. Consensus diagnostic criteria have excellent specificity, but lack se

Molecular imaging of inflammation in the ApoE -/- mouse model of atherosclerosis with IodoDPA.

BACKGROUND: Atherosclerosis is a common and serious vascular disease predisposing individuals to myocardial infarction and stroke. Intravascular plaques, the pathologic lesions of atherosclerosis, are largely composed of cholesterol-laden luminal macrophage-rich infiltrates within a fibrous cap. The ability to detect those macrophages non-invasively within the aorta, carotid artery and other vessels would allow physicians to determine plaque burden, aiding management of patients with atherosclerosis. METHODS AND RESULTS: We previously developed a low-molecular-weight imaging agent, [(125)I]io

[Molecular PET imaging for in vivo detection of amyloid in the human brain].

Neocortical deposition of amyloid plaques is one of the pathological hallmarks of Alzheimer's disease (AD). In vivo detection of amyloid plaques in the brain enables early identification of AD patients. Many beta-sheet binding agents have been developed as amyloid-binding radiotracers for PET. Currently, the most successful amyloid-binding agent is 11C-PIB. PET amyloid-imaging studies in human subjects have shown a robust difference between the retention pattern in AD patients and healthy controls. In vivo amyloid-imaging enables early and accurate detection of AD patients in the stage of MCI.
