Isolated sphenoid sinus mucocele: etiology and management.

Sphenoid isolated lesions are rare; they occur in 2% to 3% out of all paranasal sinus lesions. Diagnosis is based on the endoscopic examination and computed tomography scan or magnetic resonance imaging of the maxillofacial area. In this study, we present patients with sphenoid mucocele in the Maxillo-facial Surgery Department, University of Rome La Sapienza, treated with the endoscopic technique. Moreover, a literature review is performed, dealing with the evolution of approaches to the sphenoid sinus. From January 2001 to January 2007, we treated 75 patients affected by paranasal sinus mucoc…

Ethmoid mucocele presenting as an orbital mass.

This is a case of an 11-year-old male with an ethmoid sinus mucocele who presented to our emergency department with an orbital mass. This mucocele was most likely related to an ipsilateral orbital subperiosteal abscess that required endoscopic drainage 9 years ago. Mucoceles are mucus-filled, epithelial-lined sacs that slowly develop in the paranasal sinuses when sinus drainage is obstructed by inflammatory processes, trauma, or prior surgery. They are rare in children and should be suspected in patients with progressive headache or orbital complaints.

Infratemporal fossa mucocele with trigeminal nerve compression.

We present a rare case of an infratemporal fossa mucocele with facial paresthesia in a 36-year-old man. Both computerized tomography scans and magnetic resonance imaging confirmed a cystic lesion occupying the left infratemporal fossa. Marsupialization using transmaxillary and endoscopic intranasal approaches was performed to drain the mucocele into the nasal cavity and the application of mitomycin C to preserve the drainage of mucocele, which may be a good surgical option for the complete excision of infratemporal fossa cystic lesions without causing postoperative morbidity.

[Atypical frontal sinus mucocele. A case report].

CLINICAL CASE: We report a case of a 46-year-old woman who developed a tender, painful mass in the left superior eyelid over a period of about 6 months. This was a frontal mucocele with atypical clinical and histopathologic features. DISCUSSION: Most mucoceles arise from the frontal or ethmoidal sinuses. Frontal mucoceles usually cause outward and downward displacement of the globe, and are often associated with fullness in the supero-nasal and medial canthal region and a palpable mass.

Sphenoid sinus chondromyxoid fibroma mimicking a mucocele.

We report the case of a 44-year-old man who presented with a chondromyxoid fibroma (CMF) of the sphenoid sinus, which filled the sinus and clinically and radiologically resembled a mucocele. Chondromyxoid fibromas are the least common cartilaginous neoplasms of bone, typically occurring in the metaphysis of long bones. They have occasionally been described in the facial bones, usually the mandible and maxilla, and rarely involve the paranasal sinuses. Chondromyxoid fibroma displays a relatively unique histological appearance, with characteristic lobulation, with spindle to stellate cells embed…

Submandibular gland mucocele: report of two cases.

Submandibular gland mucocele should be remembered in the differential diagnosis of swelling at the submandibular triangle. In the cystic lesion of the submandibular area, the biochemical analysis of aspirated material for amylase should be performed. The cases with submandibular gland mucocele should be treated by removing the lesion with both the submandibular and the sublingual glands.

Submaxillary gland mucocele: presentation of a case.

The term mucocele is referred to two concepts: the extravasation cysts resulting from salivary glandular duct rupture, with mucin leakage into the surrounding peri-glandular soft tissue, and the retention cysts, caused by a glandular duct obstruction and resulting in a decrease or even an absence of glandular secretion. Mucocele can not be considered as a true cyst because its wall lacks an epithelial lining. These lesions are very common in the minor salivary glands (particularly in the labial glands), but are very infrequent in the major salivary glands--including the submaxillary glands. Th…

[Fronto-ethmoidal mucocele: endoscopic fronto-orbital approach].

OBJECTIVE: To revise the references of surgical approach of the mucocele into frontal sinus. DESIGN: A retrospective study. STUDY SUBJECTS: Three patients with mucocele frontal and orbitary encroachment were treated with endoscopic and associated external approach. The follow-up period has been between 22 and 36 months. RESULTS: Combined approach allow marsupialization and adequate repermeabilization of the nasofrontal duct. CONCLUSIONS: Endoscopy surgery is the choice method in treatment of mucoceles, but the access has to be completed by an external approach when the lateral wall of the …

Anterior clinoid mucocele masquerading as retrobulbar neuritis.

Mucoceles arising from the anterior clinoid is rare. Although intrinsically benign, mucoceles in the sphenoid sinus (anterior clinoid variant) can lead to complications due to their proximity to other structures. We present a case of a young man whose visual complaints were typical for retrobulbar optic neuritis. Results of radiologic tests for diagnosis led us to the early detection of a surgically treatable condition, a sphenoid sinus mucocele (anterior clinoid variant). Early surgery was helpful in ameliorating the symptoms, leading to full recovery of visual function. The clinician should …

Globe ptosis secondary to maxillary sinus mucocele.

Six patients were treated for gradual onset of enophthalmos, a deep superior sulcus and globe ptosis. There was no history of orbital trauma or sinusitis. CT scan showed an opacified shrunken maxillary sinus with dehiscence and depression of the orbital floor and downward displacement of the orbital contents. Pathological review of the surgical specimens showed a respiratory mucosal lining with thick mucoid secretions, new bone formation, but no purulence. The etiology is thought to be maxillary sinus mucocele. Surgical treatment with an otolaryngologist consisted of a Caldwell-Luc procedure t…

Infected congenital mucocele of the nasolacrimal duct.

The authors report their experience with an infant presenting with an infected nasolacrimal duct mucocele, emphasizing correlation of clinical, CT, and surgical findings. CT is the imaging modality of choice to demonstrate the triad of 1) a cystic medial canthal mass, 2) dilatation of the nasolacrimal duct, and 3) a submucosal nasal cavity mass; findings which are diagnostic of this entity. A brief review of the relevant embryology is also presented.

Sphenoid sinus mucocele with recurrent visual disturbance.

We present a case of sphenoid sinus mucocele with recurrent visual disturbance on the same side. A 22-year-old female showed two episodes of visual disturbance in the left eye for 3 months, and acute retrobulbar optic neuritis was diagnosed. With corticosteroid, visual disturbance improved in 1 week. MRI and CT scans showed mucocele in the left sphenoid sinus, and left optic nerve swelling with high intensity was observed in T2-weighted MRI. No destruction of the optic canal was found. The contiguous inflammation in the optic nerve rather than compression was considered as pathogenesis.

"Unusual" MRI appearance of sphenoid sinus mucocele.

The authors report a case of sphenoid sinus mucocele which exhibited unusual MRI features. However a review of the literature shows that these lesions may present with different MRI appearances probably related to the variability of the cyst content. Further series are needed for a better definition of the MRI behaviour of the lesions.

[Sphenoidal sinus mucocele. Diagnosis and endoscopic surgery].

Two cases of sphenoid sinus mucocele are described. Diagnosis was based on insidious orbital symptoms, apex orbital syndrome, followed by CT and MRI examinations. In one case the mucocele was preceded by seven years of a successfully treated undifferentiated nasopharyngeal carcinoma. Simple transnasal endoscopic drainage of the mucoceles gave good results, although a monocular blindness of long standing was irreversible. A high level of suspicion, combined with modern imaging techniques and prompt surgical intervention, are the prerequisites for avoiding persisting sequelae.

[Maxillary antrum mucocele. Comments on 3 cases].

After presenting 3 cases of maxillary sinus mucocele, met with all along 22 years, the authors bring forth some etiopathogenic, clinic and radiologic considerations, shown through the data in literature. They underline the complex, polymorphic, uncharacteristic radiological and clinical aspect. The exploratory puncture is of real help for a precise diagnosis, in this case, and is easy to perform. The treatment is always surgical, without excluding the possibility of a recurrence if the cyst caul has not been completely removed.
