Chlamydia trachomatis is responsible for urogenital infections, often minimally symptomatic, revealed by their complications. Mycoplasmas, mainly Ureaplasma urealyticum and Mycoplasma hominis, are normal commensal organisms of the genital tract, which sometimes makes it difficult to determine their pathogenicity. Mycoplasma hominis and Ureaplasma urealyticum are common inhabitants of the human genital tract. Evidence for an aetiological role in pyelonephritis, pelvic inflammatory disease, post-abortion and post-partum fever has been presented. There are sporadic reports of Mycoplasma causing serious extragenital infection such as septicemia, septic arthritis, neonatal meningitis and encephalitis. We review 38 cases of surgical infections with Mycoplasma.

In two separate herds of fattening calves a sudden-onset outbreak of ocular disease with profuse lacrimation occurred. The disease resembled the early stage of infectious bovine keratoconjunctivitis but after a few days the clinical signs of bronchopneumonia appeared. We report the case of a patient who was admitted to hospital for a pneumonia in association with a Stevens-Johnson syndrome. The association of these conditions suggested a Mycoplasma pneumoniae infection, which was confirmed on serology. There were three other extra-respiratory complications discovered: cold agglutinins, a disturbance of liver function tests, and circulating anticoagulant; both the two latter and the Stevens-Johnson syndrome are rare complications.

The core tissue microflora of 40 patients who underwent tonsillectomies were examined with cultures, DNA probe tests, enzyme immunoassays, and direct immunofluorescence antibody tests for Chlamydia and Mycoplasma. We believe this is the largest and most accurate prospective study that has examined the role of Chlamydia and Mycoplasma in the core tonsil tissue of patients with recurrent or chronic tonsillitis. The data strongly indicate that, unlike acute tonsillopharyngitis, Chlamydia and Mycoplasma are not involved in recurrent or chronic tonsillitis.

Isolates of Mycoplasma hominis collected from patients in Boston and New York between 1976 and 1989 were studied. Although prevalence of M. hominis colonization during pregnancy varies from 12-50%, its role in infections of the mother and newborn infants is unclear. Definite correlations exist with chorioamnionitis and amniotic fluid infections, but as it is rarely isolated alone during these infections, its pathogenic role is uncertain. Its association with septic abortion is similarly questioned. Prevalence and antibody titers to M. hominis increase with increasing parity. Transient bacteremia occurs in approximately 2.5% of normal deliveries. M. hominis does have a significant role in postpartum fever.

A total of 185 patients were investigated for M. hominis and mycoplasmal infection was established in 28 patients. In 2 of the patients mixed infection with E. coli was established and I with Staphylococcus aureus. Fifteen had pyelonephritis and 6--arterial hypertension, without positive data for pyelonephritis. The patients were treated with gentamycin and tetracycline according to the data of antibiograms of the isolated microorganisms. The criteria for admittance of the presence of mycoplasmal pyelonephritis are discussed.

Genital Mycoplasma and Ureaplasma have been implicated in pelvic inflammatory disease, puerperal infections, septic abortions, low birth weight, nongonococcal urethritis and prostatitis as well as spontaneous abortion and infertility. An unequivocal diagnosis of infection with these organisms can be made only after properly obtained specimens have been evaluated with the use of selective cultures.

Mycoplasma felis was the only organism recovered from the thoracic cavity of a horse with pleuritis. Large numbers of mildly degenerative neutrophils were in the pleural fluid.
