Treatment of superficial mycoses: review. Part II.

Superficial fungal infections of the hair, skin and nails are a major cause of morbidity in the world. Choosing the right treatment is not always simple because of the possibility of drug interactions and side effects. The first part of the article discusses the main treatments for superficial mycoses - keratophytoses, dermatophytosis, candidiasis, with a practical approach to the most commonly-used topical and systemic drugs , referring also to their dosage and duration of use. Promising new, antifungal therapeutic alternatives are also highlighted, as well as available options on the Brazili…

[Cutaneous mycoses in Japan originating from animals].

Human cases of dermatophytoses are occasionally transmitted from animals, and suffered from tinea corporis and sometimes Kerion celsi. The most frequent causative agent of these diseases is Microsporum canis. The other dermatophyte, Arthroderma benhamiae is now prevailing in rabbits, rodents and hedgehogs that are popular household pets in Japan. Therefore, some human cases of A. benhamiae infection were reported and the transmission of this infection from rabbits and rodents was confirmed.Cryptococcosis is regarded as dangerous zoonosis, but its transmission from animal to peoples has not bee

Immune reconstitution syndrome associated with opportunistic mycoses.

Host immunity is essential in facilitating the eradication of infection. However, immunological recovery and an imbalance characterised by either suboptimum or excessive expression of immune responses can also be harmful to the host. Inflammatory responses triggered by rapid resolution of immunosuppression can lead to a series of localised and systemic reactions, termed immune reconstitution syndrome (IRS), that are often misconstrued as failure of specific antifungal therapy to eliminate the offending fungal pathogen. Recognition of IRS has become increasingly relevant in the context of our c

[Internal mycoses of exogenous origin (author's transl)].

The increase of both visceral mycoses and designations used for them requires, among other matters an accord on a better non-ambiguous terminology. The disease designation "blastomycosis" is quite obsolete and should be replaced by proper terms such as candida mycosis, cryptococcus mycosis, histoplasma mycosis etc. The frequently used term "systemic mycosis" should be restricted to exogenous mycotic infections of internal organs caused by obligatory pathogenic fungi such as Coccidioides immitis, Histoplasma capsulatum and others. The common term "endomycosis" is considered appropriate for fung…

Endemic Mycoses in Solid Organ Transplant Recipients.

The endemic mycoses are a group of thermally dimorphic fungal pathogens occupying a specific geographic range. In North America, the chief endemic mycoses are histoplasmosis, coccidioidomycosis, and blastomycosis. Endemic fungi can cause serious infections in solid organ transplant recipients from primary infection, reactivation of latent disease, or donor-derived infection.

Angioinvasive opportunistic filamentous mycoses in immunocompromised patients.

Immunocompromised individuals are at greater risk for disseminated fungal infections. Immunocompromised individuals in the community have increased because of medical advances, thereby increasing the incidence and prevalence of opportunistic mycoses. The following case series illustrates the importance of having a high clinical suspicion for skin manifestations concerning for deep fungal infections.

[Invasive mycoses during hematopoietic stem cell transplantation].

AIM: To define the frequency, etiology, and risk factors of invasive mycoses (IM) in patients with allogeneic (allo) and autologous (auto) hematopoietic stem cell transplantation (HSCT) and to evaluate the impact of IM on overall survival (OS). MATERIALS AND METHODS: Data on 356 patients after allo-HSCT (n = 237) and auto-HSCT (n = 119) from 2000 to 2010 were analyzed. The diagnosis of IM was established according to the EORTC/MSG 2008 criteria. RESULTS: The incidence of myocardial infarction (MI) was 19.1%; that was 23.2 and 10.9% in allo-HSCT and auto-HSCT recipients, respectively. The inc…

[Blood coagulation in domestic deep-seated mycoses].

Activation of blood coagulation to a varying extent affect the course of domestic invasive mycoses. Upon invasion of blood vessels by Candida or aspergilli, occasionally thrombi are formed, which may cause septic embolism. In the course of mucormycosis (syn. zygomycosis) thrombotic occlusion of afflicted blood vessels and subsequent necrosis of dependent tissue regularly occurs. Coagulation during candidosis or aspergillosis may be triggered by secreted aspartic proteinases which are able to activate factor X as has been shown previously [1, 2]. During mucormycosis, severe blood coagulation ap…

Pulmonary mycoses. New concepts and new therapy.

The pathogenic and opportunistic fungi that commonly infect the human respiratory system cause diseases such as histoplasmosis, blastomycosis, aspergillosis, and invasive candidiasis. The increase in the incidence and severity of mycotic infections due to more aggressive immunosuppressive therapy and the spread of acquired immunodeficiency syndrome (AIDS) has had an impact on diagnosis and treatment of these conditions. Several therapeutic agents are currently available or undergoing clinical trials in the treatment of the pulmonary mycoses; among the agents with the greatest potential usefuln…

Endemic mycoses - are we making progress in management?

PURPOSE OF REVIEW: The endemic fungi are a significant cause of morbidity and mortality in effected patients. The range of endemicity for th … erapy over itraconazole in the treatment of talaromycosis. SUMMARY: The increased range of endemic mycoses coupled with the growing immunocompromised patient population mandates continued investigation of improved diagnostic and therapeutic options. Advances in these areas have led to more rapid diagnosis and more efficacious antifungal therapy.

Epidemiology of Systemic Mycoses in the COVID-19 Pandemic.

The physiopathologic characteristics of COVID-19 (high levels of inflammatory cytokines and T-cell reduction) promote fungal colonization and infection, which can go unnoticed because the symptoms in both diseases are very similar. The objective of this work was to study the current epidemiology of systemic mycosis in COVID-19 times. A literature search on the subject (January 2020-February 2021) was performed in PubMed, Embase, Cochrane Library, and LILACS without language restrictions. Demographic data, etiological agent, risk factors, diagnostic methods, antifungal treatment, and fatality r

[Epidemiology of invasive mycoses: A landscape in continuous change].

The landscape of invasive mycoses is in a continuous evolution with important implications for their diagnosis and treatment. The overall burden remains high, particularly in neonates and the elderly, patients admitted to intensive care units, using prostheses, catheters or other intravenous devices, those receiving different immunosuppressant treatments or antineoplastic chemotherapy, or transplant recipients. In addition, opportunistic mycoses can be associated with HIV infection. Many fungal infections are acquired by inhalation, direct contact or ingestion, but fungi can also enter into th…

Subcutaneous mycoses in coastal Karnataka in south India.

BACKGROUND: Subcutaneous mycoses are chronic, localized infections of the skin and subcutaneous tissue which occur following traumatic implantation of the etiological agent. The causative organisms are soil saprophytes of regional epidemiology with varying ability to adapt to the tissue environment and elicit disease. OBJECTIVES: This study was conducted to evaluate the various types of subcutaneous mycoses, including actinomycotic mycetomas, in south coastal Karnataka, India. METHODS: Between January 2005 and January 2013, a total of 25 patients were diagnosed with subcutaneous mycoses base…
