[A unique psychopharmacologic profile of adrafinil in mice].

The following psychopharmacological effects of adrafinil have been observed in mice: increase in locomotor activity (64-256 mg.kg-1), antagonism (16-128 mg.kg-1) of the hypnotic effects of barbitone but not of pentobarbitone, reduction of immobility duration in the forced swimming test (16-256 mg.kg-1); slight antagonism (256 mg.kg-1) of electroshock-induced convulsions; no modification of rectal temperature; no stereotyped or climbing behaviour; no increase in lethality in aggregated mice (LD50 isolated = 1022 mg.kg-1, LD50 aggregated = 859 mg.kg-1); lack of effects on the provisional tests f

Preoperative topical flurbiprofen-Na+ in extracapsular lens extraction role in maintaining intraoperative pupillary dilatation.

Induction of intraoperative pupillary constriction, is predominantly a prostaglandin mediated process. The most potent antiprostaglandin NSAID, Flurbiprofen was used topically to study its efficacy against the above. In a prospective double blind clinical study, 50 brown eyes undergoing planned E.C.C.E., the pupils were dilated with 10% phenylephrine and 2% homatropine 1%/tropicamide. 25 eyes received 0.03% Flurbiprofen-Na+ eye drops 1/2 hourly starting two hours before surgery. The maintained intraoperative mydriasis in the two groups before anterior chamber entry (stage I) vs at the end of c

Severe Neurotoxicity due to Atropa belladonna Poisoning: A Case Report and Literature Review.

Atropa belladonna (A. belladonna), commonly known as deadly nightshade, is a poisonous plant belonging to the Solanaceae family. The toxic effects of A. belladonna are attributable to its alkaloid content, which possesses potent anticholinergic properties. These alkaloids are responsible for the plant's toxicity and can cause a range of adverse effects in humans and animals upon ingestion or contact. In this report, we describe two atypical cases of A. belladonna poisoning resulting from accidental ingestion of the plant's raw leaves, which were referred to the emergency room of a poisoning ce

Optometrists' examination and referral practices for patients presenting with flashes and floaters.

INTRODUCTION: Patients experiencing flashes and floaters commonly present to their optometrist. Some of these patients may have significant pathology, yet there is a great deal of variability with regard to examination technique and referral practice. METHODS: A questionnaire survey was undertaken to determine the current management of patients presenting to their optometrist with flashes and floaters. All practising community optometrists within Southern Derbyshire received a questionnaire and 74 (56.9%) completed replies were received. RESULTS: Optometrists estimated that an average of 14

Anticholinergic toxicity associated with lupin seed ingestion: case report.

We describe a case of acute poisoning in a 51-year-old female patient who presented to the Emergency Department with weakness, anxiety, dry mouth, bilateral mydriasis and lid drop. In differential diagnosis, botulism, Guillain-Barré syndrome and myasthenia gravis were considered, as well as cerebral haematoma because of a cranial injury a week before. Symptoms, which resolved within 12 h without any therapy, were instead related to the ingestion of lupin seeds.

Diplopia as a sign of third nerve palsy due to intracranial aneurysm: a case report.

Third nerve palsies that result from head injuries or intracranial aneurysms may sometimes show symptoms of aberrant regeneration and only partially heal. The usual characteristics of oculomotor nerve palsy, caused by compression of the third cranial nerve, are severe ptosis, deficiencies in elevation, depression, and adduction, and a dilated, poorly responding pupil on the afflicted side. The parasympathetic fibres are usually spared from a vasculopathic lesion and impacted by compressive lesions because they are situated in the peripheral segment of the oculomotor nerve as it leaves the brai

Neuro-Ophthalmology in Emergency Medicine.

Understanding the anatomy and physiology of the eye, the orbit, and the central connections is key to understanding neuro-ophthalmologic emergencies. Anisocoria is an important sign that requires a systematic approach to avoid misdiagnosis of serious conditions, including carotid dissection (miosis) and aneurysmal third nerve palsy (mydriasis). Ptosis may be a sign of either Horner syndrome or third nerve palsy. An explanation should be pursued for diplopia since the differential diagnosis ranges from the trivial to life-threatening causes.

Systolic hypertension as side effect of topical low dose atropine drops.

PURPOSE: To present a case of increased systemic hypertension and pupil dilation related to low dose atropine eyedrops. OBSERVATIONS: A thirteen-year-old male with progressive myopia received atropine 0.05% ophthalmic drops to slow down myopia progression. He exhibited systemic systolic hypertension, photophobia, and bilateral nonreactive mydriasis. The atropine drops were discontinued, and his blood pressure and pupillary function normalized. CONCLUSIONS AND IMPORTANCE: This case demonstrates sensitivity to low dose atropine with increased systemic blood pressure and pupillary dilation.
