[Acute idiopathic longitudinally extensive transverse myelitis with peripheral axonal motor neuropathy].

Acute longitudinally extensive transverse myelitis (LETM) is a rare disease of autoimmune, connective tissue or infectious etiology (bacterial or viral) in which spinal cord lesion extends vertically over three or more vertebral segments. The most common cause of LETM is neuromyelitis optica (Devic's syndrome). It's typically clinically characterized by a tetraparesis or paraparesis of the lower extremities, superficial and deep sensation disturbances and sphincter dysfunction. Differential diagnosis is complex and based on immunological, serological, biochemical tests. Authors present 37-year

Systemic sclerosis in aquaporin-4 antibody-positive longitudinally extensive transverse myelitis.

We report on the first patient with a relapsing, anti-aquaporin-4 (AQP-4) antibody-positive, longitudinally extensive transverse myelitis (LETM) who developed systemic sclerosis (SSc). A 62-year-old woman, who presented with bilateral, distal lower limb and perineal numbness, developed clinical manifestations and paraclinical features of SSc. Spinal cord imaging revealed lesions that were consistent with LETM. Patient's serum was positive for neuromyelitis optica (NMO)-IgG/AQP-4 antibodies. High-dose intravenous corticosteroids improved the neurological symptoms. The present case expands the l

Acute transverse myelitis in childhood: center-based analysis of 47 cases.

OBJECTIVE: To relate clinical characteristics associated with acute transverse myelitis (ATM) in children with functional outcomes at follow-up. METHODS: We identified 47 patients for whom ATM occurred under the age of 18 years. Chart analysis, clinical evaluation, and administration of functional measures were completed. RESULTS: The age at onset clustered between ages 0 to 2 and 5 to 17. Febrile illness had occurred in 47% and vaccination in 28%. Major disability at the nadir of the clinical course was noted. Eighty-nine percent were unable to walk, required assisted ventilation, or both.

Rapid recovery from acute transverse myelitis in an elite female swimmer.

Rubella is now an uncommon disease, and its neurological complications are rarely encountered. One complication, acute transverse myelitis, has been described in a few case reports and usually has an unfavourable outcome. The case is presented of an elite swimmer who developed transverse myelitis after rubella, which was treated with methylprednisolone. The recovery was so rapid and complete that two months after the acute event she broke a national swimming record.

Longitudinally Extensive Transverse Myelitis in Autoimmune Connective Tissue Diseases: A Comprehensive Review.

Longitudinally extensive transverse myelitis (LETM) is a severe inflammatory disorder of the spinal cord characterised by lesions extending across three or more vertebral segments and is associated with significant neurological morbidity. Autoimmune connective tissue diseases constitute an important noninfectious cause of this entity, reflecting complex immune-mediated and vascular mechanisms that result in extensive spinal cord injury. Despite increasing recognition, clinical presentation is heterogeneous, diagnostic differentiation remains challenging, and management strategies lack standard

Enteroviral Transverse Myelitis Presenting as Acute Ataxia in Children: A Case Series.

Background: Enteroviruses, members of the Picornaviridae family, typically cause asymptomatic or mild infections. However, they can also result in central nervous system (CNS) involvement, with transverse myelitis (TM) occurring only on rare occasions. TM is a syndrome characterized by acute or subacute spinal cord dysfunction, leading to neurological deficits below the level of the lesion. Case report: We report a case series of eight pediatric patients admitted over a three-month period, June to August 2024. All patients presented with ataxia and/or other neurological symptoms, alongside abn

Integrated Rehabilitation of Longitudinal Extensive Transverse Myelitis: Study of a Complex Case.

Longitudinal extensive transverse myelitis (LETM) is a rare neurological disorder portrayed by inflammation of the spinal cord spanning three or more vertebral segments. It can lead to severe symptoms such as weakness, sensory abnormalities, and dysfunction in various parts of the body. LETM can be idiopathic or associated with autoimmune conditions like Multiple Sclerosis (MS) and Neuromyelitis Optica (NMO). Diagnosis of LETM requires MRI imaging of the spine, and treatment often involves corticosteroids, intravenous immunoglobulins (IVIG), and plasma exchange. Physical therapy plays a crucia

Longitudinally transverse myelitis as a rare complication of pneumococcal meningitis: case report.

Pneumococcal meningitis is the most severe bacterial meningitis rarely complicated by acute myelitis. We report a case of a 54-year-old female who presented with pneumococcal meningoencephalitis. After eight days of hospitalization, the patient presented a sudden onset of bilateral lower leg weakness and bladder and bowel sphincter dysfunction. A neurological examination revealed a flaccid paraplegia. A magnetic resonance imaging (MRI) of the brain and the complete spinal cord was performed and showed cervical and thoracic myelitis. The patient was successfully treated with antibiotics and int

Transverse Myelitis in a Healthy Adult Female Patient: A Rare Viral Etiology.

Transverse myelitis (TM) is an uncommon but extensively documented neurological disorder marked by acute spinal cord inflammation, resulting in a spectrum of impairments including motor, sensory, and autonomic disturbances. We outline a rare case of viral-induced TM linked to COVID-19 and Coxsackie B viral infections. A 25-year-old healthy female patient presented with acute bilateral upper and lower extremity weakness, paresthesia, and urinary retention. MRI of the cervical spine revealed abnormal marrow signal intensity. Laboratory investigation highlighted the presence of SARS-CoV-2 RNA; Co

Influenza B-induced longitudinally extensive transverse myelitis and bithalamic acute disseminated encephalomyelitis.

In the COVID-era, other viral pathogens, like influenza B, gain less attention in scientific reporting. However, influenza still is endemic, and rarely affects central nervous system (CNS). Here, we report the case of a 35-year-old male who presented with fever since 1 week, and developed acute ascending flaccid paralysis and urinary retention. The clinical presentation of paraparesis in combination with the inflammation proven by the lumbar puncture, and the MRI full spine, fulfilled the diagnostic criteria of longitudinally extensive transverse myelitis (LETM). In this case, it is most likel

Post-COVID-19 Longitudinally Extensive Transverse Myelitis with Myelin Oligodendrocyte Glycoprotein Antibodies.

BACKGROUND: Myelin oligodendrocyte glycoprotein (MOG) antibody disease most commonly presents with optic neuritis, though myelitis is also possible. It is rare in the post-infectious and particularly post-COVID-19 setting. Case Presentation. We present the case of a 57-year-old man who tested positive for COVID-19 and experienced respiratory symptoms that completely resolved within one week. About 3 weeks after testing positive, he began experiencing acute onset anuria, followed by lower extremity paresthesia and paraparesis, which progressed to bilateral lower extremity paraplegia, complete l

Two Cases of Probable Neuro-Behçet's Disease with Longitudinally Extensive Transverse Myelitis.

We report 2 cases of probable neuro-Behçet's disease (NBD) with longitudinally extensive transverse myelitis (LETM). In both cases, the patients presented paraplegia, as well as sensory, bladder, and rectal disturbances. Magnetic resonance imaging (MRI) of patient 1 showed continuous high signal intensity extending from the midbrain to the entire spinal cord in the central part of the cord on T2-weighted imaging (T2WI). Spinal MRI of patient 2 revealed high signal intensity extending from Th2 to Th10 in the central part of the cord on T2WI. Both patients received high-dose methylprednisolone.

Radiation induced delayed transverse myelitis and neurological deficit at tertiary care center.

BACKGROUND: Transverse myelitis is a rare spinal cord inflammation with absence of a compression. It varies in presentation based on the pathology location, and mainly causes a combined deficit of motor, sensory, and autonomic functions. History, physical examination, and other diagnostic tests including blood tests and an MRI are important tools to establish a diagnosis.A thorough neurological evaluation helps localize the affected region of the spinal cord. The management includes rehabilitation as any other spinal cord injury. If very severe, a multidisciplinary rehabilitation program will

A case of longitudinally extensive transverse myelitis following vaccination against Covid-19.

BACKGROUND: Longitudinally extensive transverse myelitis (LETM) is a rare subtype of transverse myelitis (TM) that potentially results in relevant disability. Apart from association to neuromyelitis optica and other chronic demyelinating diseases of the central nervous system, many other aetiologies are known. Particularly systemic infections and vaccination are considered potential triggers for immune mediated inflammation of the spinal cord. In the course of the current Covid-19 pandemic several cases of TM following Covid-19 infection have been described. Here we present a case of LETM foll

Longitudinal extensive transverse myelitis: a rare neurological complication of systemic lupus erythematosus.

We present a 47-year-old woman with recently diagnosed systemic lupus erythematosus who developed progressive numbness and tingling of her upper and lower extremities, followed by weakness and difficulty ambulating. She was diagnosed with longitudinal extensive transverse myelitis involving her entire cervical and thoracic spinal cord. Infectious workup was unrevealing. She failed to respond to pulse-dose intravenous steroids, but slowly improved with the addition of plasmapheresis and cyclophosphamide. Following maintenance treatment with mycophenolate mofetil and slow tapering of oral steroi
