Electrophysiological study of diaphragmatic myoclonus.

This is the first reported detailed electrophysiological study of diaphragmatic myoclonus. An 86 year old woman had rapid, intermittent epigastric pulsations. Neurological examination and imaging studies of the brain and spinal cord were normal. Needle EMG showed rhythmic contractions of the diaphragm and external intercostal muscles at 4 to 5 Hz. These contractions were often associated with suppression of normal breathing and were capable of maintaining adequate ventilation. Both diaphragms were involved but showed considerable variability in their relative latencies. Automated interference …

Reflex myoclonus in olivopontocerebellar atrophy.

The presence of reflex myoclonus in response to touching and pin-pricking the wrist or stretching the fingers and to photic stimulation was assessed in 24 patients with a presumed diagnosis of olivopontocerebellar atrophy (OPCA) and in 30 age matched control subjects. Reflex myoclonus to soma-esthetic stimulation was found in 23 patients and in none of the controls. Photic myoclonus was present in 12 patients and in none of the controls. Electrophysiological study of the reflex myoclonus showed enhanced (> 10 microV) somatosensory evoked potentials and an associated reflex electromyographic di…

Spinal myoclonus resembling belly dance.

A 63-year-old man presented with an 11-month history of progressive myoclonus in the right abdominal wall. Administration of clonazepam reduced the frequency and amplitude. When the therapy was discontinued, the frequency and amplitude of the myoclonus increased, and synchronous and weak myoclonus also was observed in the left abdomen. The trunk was twisted just after the appearance of the abdominal myoclonus associated with myoclonic jerks spreading from the rostral to caudal paraspinal muscles. Later in the clinical course, the myoclonus became stimulus sensitive and was induced by tendon ta…

Posthypoxic myoclonus in intensive care.

Posthypoxic myoclonus (Lance-Adams' syndrome) is a rare complication of cardiorespiratory arrest. It has a better prognosis than other movement disorders secondary to brain ischaemia. We report a case of posthypoxic myoclonus in a 66-year-old woman after acute myocardial infarction and cardiopulmonary arrest. She had action and intention myoclonus, and these movements were also initiated by acoustic and pain stimuli. The origin of the myoclonus was probably subcortical, and it improved with clonazepam 2 mg t.i.d. We emphasize that early diagnosis is necessary in intensive care units in order t…

Cortical myoclonus in Huntington's disease.

We describe three patients with Huntington's disease, from two families, in whom myoclonus was the predominant clinical feature. The diagnosis was confirmed at autopsy in two cases and by DNA analysis in all three. These patients all presented before the age of 30 years and were the offspring of affected fathers. Neurophysiological studies documented generalised and multifocal action myoclonus of cortical origin that was strikingly stimulus sensitive, without enlargement of the cortical somatosensory evoked potential. The myoclonus improved with piracetam therapy in one patient and a combinati…

[Propriospinal myoclonus--a case report].

Propriospinal myoclonus is one type of myoclonus that is proposed to originate in the spinal cord and spread up and down via propriospinal tract. There are a few reports describing the detailed electrophysiological findings of this myoclonus. In this communication, we present the results of electrophysiological analysis of a patient with propriospinal myoclonus. A 23-year-old woman suddenly noticed abdominal pain, which spontaneously faded in a few hours. Irregular involuntary flexion jerks of the trunk appeared spontaneously without pain or hiccups after this episode. It involved the bilatera…

Cortical activity-associated negative myoclonus.

A patient with myoclonus epilepsy had 2 types of negative myoclonus as well as positive myoclonus at rest. One type followed a large EMG discharge at the end of continuous contraction and the other did not. Both types of negative myoclonus and positive myoclonus were preceded by EEG activity, predominantly distributed in the contralateral central region. It is suggested that an abnormal activity around the motor cortex suppresses a voluntary contraction as well as induces myoclonus. Clonazepam was markedly effective on both positive and negative myoclonus.

Propriospinal myoclonus: a neurophysiologic analysis.

We are reporting a neurophysiologic analysis of two patients presenting with thoracoabdominal spontaneous muscle jerks. Polymyographic recor … C-reflex, and intercostal nerve conduction were normal. These findings suggest a generator for the myoclonus in the midthoracic region of the spinal cord with up and down propagation by slowly conducting pathways, such as propriospinal fibers. This type of spinal myoclonus may thus be termed "propriospinal myoclonus," as suggested by Brown et al.

Propriospinal myoclonus in multiple sclerosis.

The clinical and electrophysiological features of segmental myoclonus affecting the right arm and upper trunk are described in a patient with multiple sclerosis. Electrophysiological studies suggested that the myoclonus was propagated from a generator site in the cervical cord, where lesions were found using MRI. The spread of electromyographic activity in each myoclonic jerk was slow and variable. These findings are characteristic of propriospinal myoclonus, which has not been associated with multiple sclerosis previously.

Myoclonus in adult Huntington's disease.

Two brothers with clinically definite adult Huntington's disease developed disabling myoclonus years after the first signs of the disease. Their electroencephalograms were consistent with a primary generalized epilepsy, although neither man had seizures. The myoclonus was controlled with valproic acid therapy.

Palatal myoclonus in Krabbe disease.

A seven-year-old girl with Krabbe disease presenting palatal myoclonus only when awake is reported. The patient was diagnosed as having Krabbe disease enzymatically at the age of eleven months. She developed rhythmical contractions of the soft palate, pharynx, larynx, lips and tongue at two years. The surface electromyography showed rhythmical 2 Hz electrical activities. The MRI disclosed markedly attenuated intensity in the midbrain, pons and medulla oblongata on T2-weighted images. Palatal myoclonus was not controlled by carbamazepine in therapeutic doses, but disappeared when the patient wa…

Moxalactam myoclonus, seizures, and encephalopathy.

Severe myoclonus, generalized seizures, and marked stupor occurred in a 39-year-old woman with chronic renal insufficiency after one week of moxalactam 2 g iv q8h. Moxalactam serum levels far exceeded the recommended therapeutic range. The drug half-life was 17 hours (normal 1.9). Excessively high levels of moxalactam, which penetrates the blood-brain-barrier relatively easily, may provoke seizures and encephalopathy.

Myoclonus as a Manifestation of Reflex Seizures.

Myoclonus is a motor symptom commonly associated with reflex seizures in people with idiopathic generalized epilepsies. The most frequently observed triggers of myoclonus are related to visual stimuli, including flashing lights or patterns; nonetheless, myoclonus can also be activated by movement, speech or reading, calculations, and praxis. Reflex myoclonic seizures may be the hallmark of a reflex epilepsy, but it may lead to the diagnosis of generalized epilepsy syndromes. In the setting of idiopathic generalized epilepsies, reflex myoclonus can persist despite optimal medical therapies and …
