Neonatal purpura fulminans secondary to group B streptococcal infection.

Purpura fulminans is an acute and frequently fatal disorder characterized by sudden onset of progressive cutaneous hemorrhage and necrosis due to dermal vascular thrombosis and disseminated intravascular coagulation. The authors present a neonate with extensive purpura fulminans due to group B streptoccoccal septicemia and evaluated the attributable clinical mortality and morbidity of this potentially lethal syndrome. Clinicians especially neonatologists should be aware that early-onset sepsis of group B Streptococcus in the newborn infant with purpura fulminans could be a cause of maternal ca…

[Severe maternal and fetal situations during delivery].

If pregnancy is very frequently normal, severe complications can appear for fetus, mother or both. The etiologies are various but preeclampsia and its complications remain one of the leading causes. The management is discussed according to the etiology and the severity of the disease and also the level of maternity and neonatal unit which can accept the newborn. However, all situation is a case apart and any decision will not be taken without concertation between obstetricians and neonatologists. In France, a regionalization policy is taking place to improve the maternal and neonatanal managem…

Persistent pulmonary hypertension of the newborn. An update.

PPHN remains a perplexing and frustrating disorder for primary care physicians and neonatologists. The diagnosis is difficult and the optimal management controversial. ECMO has become a lifesaving technological advance for infants who do not respond to conventional medical management, and neurodevelopmental outcome of infants treated with ECMO has been favorable. The most recent therapy with potential usefulness is inhaled nitric oxide which allows local vasodilator effects without unwanted systemic effects. Inhaled nitric oxide therapy for PPHN is still in the clinical trial phase and results…

State-of-the-Art Renal Imaging in Children.

Imaging modalities for diagnosing kidney and urinary tract disorders in children have developed rapidly over the last decade largely because of advancement of modern technology. General pediatricians and neonatologists are often the front line in detecting renal anomalies. There is a lack of knowledge of the applicability, indications, and nephrotoxic risks of novel renal imaging modalities. Here we describe the clinical impact of congenital anomalies of the kidneys and urinary tract and describe pediatric-specific renal imaging techniques by providing a practical guideline for the diagnosis o…

Identification and Management of Cranial Anomalies in Perinatology.

Neonatal skull and head shape anomalies are rare. The most common cranial malformations encountered include craniosynostosis, deformational plagiocephaly, cutis aplasia, and encephalocele. Improved prenatal imaging can diagnose morphologic changes as early as the second trimester. Prenatal identification also provides perinatologists and neonatologists with valuable information that helps to optimize care during and after delivery. Cranial anomalies require a multidisciplinary team approach and occasionally a lifetime of care. Today, care begins with the perinatologist as many cranial anomalie…

Long-Term Neurodevelopmental Outcome in Survivors of Twin-to-Twin Transfusion Syndrome.

Twin-twin transfusion syndrome (TTTS) is a severe complication of monochorionic (MC) twin pregnancies associated with high perinatal mortality and morbidity rates. Management in TTTS is a major challenge for obstetricians and neonatologists. Twins with TTTS are often born prematurely after an extremely distressing and highly hazardous fetal period. Follow-up studies report varying rates of cerebral palsy (CP) and long-term neurodevelopmental impairment (NDI). This review discusses the latest findings on the long-term outcome of TTTS survivors, possible risk factors for long-term impairment, an…

Hypoxic-ischemic encephalopathy in infants: new challenges.

The subject of hypoxic-ischemic encephalopathy (HIE) is hardly new and yet remains an every day challenge to clinicians, mainly neonatologists and child neurologists. However, technological advances in neuroimaging do help in the diagnosis, but prevention of hypoxic-ischemic insults, ability to change the course of the disease, and treatment of the devastating consequences of HIE are lagging behind. With these issues in mind, we reviewed the literature on this subject and our own clinical experience in search of new developments in this area. The pathology and clinicopathologic correlates of H…

Bilateral consolidation of the lungs in a preterm infant: an unusual central venous catheter complication.

We describe a case of bilateral parenchymal consolidation with sudden respiratory distress in a preterm baby as a complication of peripherally inserted central catheter (PICC) dislocation. The X-rays showed bilateral pulmonary consolidation with the catheter tip initially located in the right, and later in the left pulmonary artery. The catheter was withdrawn. As soon as the catheter was repositioned all clinical signs and symptoms disappeared. Neonatologists should consider the possibility of dramatic respiratory distress deriving from PICC dislocation. Careful tip catheter placement and cons…

Tachydyspnea in an infant chimpanzee.

Reports on intensive care and invasive treatments of primates are scarce. Generally, there is little knowledge and experience in regard to resuscitation, cardiac support and ventilation support especially in small infants of primate species. We therefore report on our experience with respect to the successful treatment of a former small-for-date chimpanzee infant with severe cardiorespiratory distress due to pneumonia inflicted by an unknown infective agent. Treatment was primarily with analgosedation, oxygen application and dobutamine infusions. Cooperation of neonatologists and veterinarians…

Genetic skin disorders.

Neonatologists do not require a detailed knowledge of all genetic skin disorders but need to recognize one if they see it. The unique accessibility of the skin makes it possible to observe the physical signs and deduce the child's immediate needs from first principles. The morphological classification given here will help the nondermatologist establish a clinical diagnosis. Tremendous advances over the last 10 years in understanding the molecular basis of skin disease make it possible, in many cases, to confirm the diagnosis and to counsel the family accurately.

Recognition of Undiagnosed Neonatal Heart Disease.

Heart defects are the most common congenital malformation. Approximately 8000 infants per year in the United States require diagnosis in the newborn period to avoid severe injury or death. It is incumbent on the neonatologist and pediatrician to expeditiously detect the presence of symptomatic heart disease so that infants can be stabilized before cardiovascular decompensation. Evaluating infants and further categorizing them into the particular pathophysiology are necessary to stabilize them in anticipation of more definitive care by the pediatric cardiac team.

Differences in attitudes to feeding post repair of Gastroschisis and development of a standardized feeding protocol.

BACKGROUND: The purpose of this study was to examine differences in attitudes to feeding in neonates with Gastroschisis between clinical groups and to develop a standardized feeding protocol. Confusion, inconsistencies in practice and lack of evidence could be contributing to avoidable delays in the establishment of enteral feeds resulting in lengthy requirements for central venous access, dependence on total parenteral nutrition (TPN), increased risk of sepsis, TPN related cholestasis and prolongation in length of hospital stay. METHODS: A national survey of clinicians (neonatologists, neona…

Acute heart failure in pregnancy: importance of a timely multidisciplinary approach to recognition and management.

Increased cardiovascular demands of pregnancy may unmask occult diseases, such as dilated cardiomyopathy or valvular stenosis, or precipitate peripartum cardiomyopathy. We report a case of the emergency management and delivery of a young pregnant woman who presented with acute decompensated heart failure that was not immediately recognized. An emergency transfer to a tertiary care institution was arranged. Once diagnosed, the patient received multidisciplinary care shared between cardiologists, obstetricians, cardiac anesthetists, a neonatologist, and a midwife, resulting in good maternal and

[Neonatal hyperthyroidism: A sometimes challenging diagnosis].

Graves disease complicates two pregnancies out of 1000 and when it is known before pregnancy, it warrants careful monitoring of the fetus and the newborn. We report on a case of neonatal hyperthyroidism, which revealed a previously unknown maternal thyroid disease. In this situation, neonatal signs can be misinterpreted, delaying the diagnosis. Neonatal hyperthyroidism is, however, a therapeutic emergency because of the risk of cardiac and neurological complications. The neonatologist must identify thyroid disease in the absence of a maternal history in order to promptly start therapy.

[The clinical picture of the fetal alcohol syndrome].

Six new cases of alcoholic embryopathy on newborn children are described. We found out that the mother suffers from chronicle alcoholism. The diagnosis fetal alcohol syndrome is made because there are the minimal diagnostically syndromes: prenatalis hypotrophia, microcephalia, mental retardation, face dysmorphism, cardinal anomalies. We have observed an unfolded clinical picture with added hypertrophia on the clitoris and anomalies of the kidneys on one of the children. With the description of the cases we direct obstetrician's and neonatologist's attention to this current in this moment medic

Standardization of nitric oxide inhalation in extremely preterm infants in Japan.

BACKGROUND: In perinatal medicine, inhaled nitric oxide (iNO) has been an important tool for the treatment of full-term and late-preterm infants with persistent pulmonary hypertension of the newborn (PPHN) and hypoxemic respiratory failure (HRF). Its use in more premature infants, however, is controversial. To evaluate the current clinical practices regarding use of acute iNO in extremely preterm infants, a nationwide survey was conducted in Japan. METHODS: A questionnaire survey was conducted from May to September, 2015. Questionnaires about PPHN and iNO treatment were sent to the doctor in

Growth of low birth weight preterm children.

Very low birth weight premature (VLBWPT) infants demonstrate growth patterns in the early years of life which differ from those of term and large low birth weight preterm (LBWPT) peers. Optimal post natal growth of VLBWPT children is associated with more positive later health and neurodevelopmental outcomes. The neonatologist engaged in the follow of care of VLBWPT infants after discharge from the Neonatal Intensive Care Unit should monitor over time the velocity of weight, length, head circumference and weight/length ratio utilizing appropriate growth references. VLBW children who demonstrate
