The treatment options in metastatic testicular germ cell cancer are based on prognostic the factor-based staging system from IGCCCG. Since 1987 (!), the optimal chemotherapy regimen has been BEP with a weekly administration of 30 mg of bleomycine, and a 3 or 5-day schedule of 500 mg/m(2) etoposide and 100 mg/m(2) cisplatin. Dose reduction of this regimen or use of carboplatin provide lower efficacy and should be abandoned. There continue to be several controversies surrounding the role for retroperitoneal lymphadenectomy (RPL) in the management of patients with germ cell cancer of the testis. The initial treatment options for those with clinical stage I disease are surveillance (orchiectomy only), RPL or chemotherapy. Survival rates are similar with RPL and surveillance. Marijuana use has been reported to spuriously elevate immunoreactive human chorionic gonadotropin (HCG) in the serum of patients with testicular germ cell tumors. To reinvestigate this finding, we measured serum HCG and delta 9-tetrahydrocannabinol (delta 9-THC) levels in 16 men known or suspected to be marijuana users. Eight of the serum samples had measurable levels of delta 9-THC, while eight did not. None contained immunoreactive HCG. A 4-year-old boy had a pulmonary blastoma removed from the site of a persistent pulmonary cystic lesion present since 6 weeks of age. Bone metastasis appeared 4 months after diagnosis. Following radiation therapy to the bone and after 15 months of VAC chemotherapy, he developed further metastases which showed little response to adriamycin or radiation therapy. He died 2 years and 2 months after the initial operation. A review of other pulmonary blastomas associated with cystic lesions in children is presented.

BACKGROUND: Testicular germ cell tumors (TGCTs), derived from primordial germ cells, are rare malignancies with high curative potential. However, the emergence of new evidence indicating that 15% of patients experience tumor progression, leading to death, underscores the need for innovative therapeutics.
