Lessons Learned from CNV Analysis of Major Birth Defects.

The treatment of major birth defects are key concerns for child health. Hitherto, for the majority of birth defects, the underlying cause re … findings of CNV analysis in non-isolated complex congenital malformations, associated with central nervous system malformations or intellectual disabilities, compared to isolated single organ-system malformations. We propose that the more complex an organ system is, and the more genes involved during embryonic development, the more likely it is that mutational de novo events …

Cervicothoracic spinal epidural cavernous hemangioma: case report and review of the literature.

Cavernous hemangiomas are vascular malformations that effect the central nervous system. This pathology is frequently encountered in the cerebral hemispheres, cerebellum, and brainstem. Cavernous hemangiomas, although usually solitary, may be associated with the same lesion in other parts of the central nervous system or other organ systems and may appear as sporadic or familial cases. They are infrequently found at the spinal level; in particular, pure cervicothoracic epidural location is rare. We report a case with a purely spinal epidural cavernous hemangioma from C6 to T1 and discuss the c

Supratentorial arteriovenous malformations.

Arteriovenous malformations are a heterogeneous group of intra-axial central nervous system vascular lesions consisting of tangles of abnormal arteriovenous connections without intervening capillary beds. The heterogeneity of arteriovenous malformations is described by the Spetzler-Martin grading scale, a scale that also forms the basis for clinical decision making. The microsurgical treatment of appropriately selected supratentorial arteriovenous malformations is based on the tenets of circumferential isolation and transection of arterial feeders, preservation of vessels en passant and surrou

Major diagnostic and pathological features of iniencephaly based on twenty-four cases.

Iniencephaly is quite a rare malformation the etiology of which is still not fully understood. In the majority of cases it is a grave and lethal condition. It is often complicated by other abnormalities affecting the central nervous system (spina bifida, anencephaly), but malformations involving other organs and systems may also be observed. Based on 24 cases the authors have surveyed the diagnostics of iniencephaly with special regard to the disorders affecting the central and non-central nervous systems. In addition, they have compared the results of prenatal diagnostics and pathological inv

Triple spinal dural arteriovenous fistula.

Spinal dural arteriovenous fistulas (SDAVFs) are the most common type of spinal vascular malformations. Multiple SDAVFs are unusual and can occur either synchronously or metachronously, as reported in the literature. We report on the unusual case of a woman with three separate SDAVFs, which were surgically treated within the same setting; the postoperative course was characterised by unexplained repeated haematoma formation within the surgical sites. A review of the literature was performed to assess the outcomes of patients with multiple distinct SDAVFs: the data of 19 patients were collected

[Spinal dural arteriovenous malformations].

Vascular malformations of the spinal cord are a rare and complex clinical entity and can lead to severe morbidity with progressive spinal cord symptoms if not treated properly. In early stages the disease is characterized by slowly progressive, nonspecific symptoms, such as gait disturbance, paresthesia, diffuse sensory symptoms and radicular pain. In the late stages, bowel and bladder incontinence, erectile dysfunction, and urinary retention may develop. In recent decades the understanding and treatment of spinal vascular malformations have improved with the evolution of diagnostic tools and

Cognard Type Ⅴ Torcular Dural Arteriovenous Fistula: A Rare Entity.

Tentorial dural arteriovenous fistulas (DAVFs) are a dangerous subtype of fistula. Lawton et al classified this type of fistula into 6 categories based on specific operative strategies. Clinical manifestations of 6 types of tentorial DAVFs vary to some extent. Cognard type V DAVFs in the tentorial region are usually petrosal/superior petrosal sinus DAVFs. DAVFs with perimedullary venous drainage around the torcular herophili are rare. The veins of the posterior fossa are intricate and variable. There are anatomic communications between the perimedullary vein and cerebellar vein or veins of the

Developmental genetic malformations of the cerebral cortex.

Cortical malformations give rise to severe clinical manifestations such as epilepsy and mental retardation, but sometimes to more subtle problems like dyslexia. From a clinical standpoint, such structural abnormalities are diagnosed by radiographic and histologic findings, with disease classifications often based on these observations. Using this categorization, many of the responsible genes have been determined and now provide a means of understanding the molecular basis of the neurologic disorders. This review discusses the known genetic developmental syndromes in the context of the observed

[Devices for Transvenous Embolization of Dural Arteriovenous Fistulas].

When approaching an occluded sinus during transvenous embolization of dural arteriovenous fistulas, it is important to employ a combination of appropriate guiding and distal access catheters to ensure adequate support and effective navigation. When navigating a long and tortuous venous route, careful attention must be given to the backup support and lengths of the coaxial catheters and connectors. Effective embolization can be achieved by selecting coils according to the stage of the embolization procedure and using liquid embolic agents when indicated.

[Foix-Alajouanine syndrome as a variant of a spinal dural arteriovenous fistula].

The epidemiology of Foix-Alajouanine syndrome (FAS) remains poorly characterized. Accurate data on the prevalence of FAS In Russia are not available; in Germany, only 5-10 cases per 1 million population are reported. The article reviews data from the current literature on FAS and presents a clinical case of a patient with a dural arteriovenous fistula at the level of the craniovertebral junction with severe circulatory disorders at the upper cervical level of the spinal cord, with progressive severe focal neurological deficit, despite endovascular surgery. The disease-associated syndromes are

Intracranial dural A-V fistulae.

Dural Arteriovenous Malformations (DAVFs) are rare vascular malformations involving the dura and cerebral veins. They occur in a variety of locations but in general the cavernous sinus lesion is more frequent among Asian patients while the transverse sinus is more usual among western patients. Treatments include microsurgery, endovascular embolization and GKNS. The dangerous concomitant of DAVFs is retrograde flow into the cerebral veins (CVR). Patients with CVR are at risk for bleeding, neurological deficit and in some cases death. They require treatment with the aim of occluding the fistula.
