[Clinical signs and diagnosis of BSE].

Bovine spongiform encephalopathy (BSE) in cattle is characterised by typical signs and an insidious course. There is gradual weight loss and decreased milk production, and the disease is distinguished by disturbances in behaviour, sensitivity and locomotion. The most common behavioural changes are apprehension and abnormal temperament. The most common changes in sensitivity are hypersensitivity to tactile, auditory and optical stimuli, kicking during milking and excessive licking of the muzzle. The most important disturbances in locomotion include ataxia, falling down and inability to get up.

Brain death/death by neurologic criteria: What you need to know.

Since the beginning of time, man has been intrigued with the question of when a person is considered dead. Traditionally, death has been considered the cessation of all cardiorespiratory function. At the end of the last century a new definition was introduced into the lexicon surrounding death in addition to cessation of cardiac and respiratory function: Brain Death/Death by Neurologic Criteria (BD/DNC). There are medical, legal, ethical, and even theological controversies that surround this diagnosis. In addition, there is no small amount of confusion among medical practitioners regarding the

Distribution of rest tremor in patients with Essential Tremor: Does it lateralize with simple kinetic, postural, or intention tremors?

BACKGROUND: Essential tremor (ET) is characterized by action tremor, although rest tremor may also occur. Despite this, rest tremor has not been studied extensively. A dearth of studies compare the relationship of rest tremor to other types of tremors in ET. METHODS: Two-hundred-and-thirteen ET cases underwent detailed neurological examination, including evaluations of rest, simple kinetic, postural, and intention tremors. A movement disorders neurologist assessed the prevalence of each type of tremor. Each instance of tremor was classified as either symmetrical (present and equally severe on

Opinions and clinical practice of functional movement disorders: a nationwide survey of clinicians in China.

BACKGROUND: There is rare reports about opinions and clinical practice of functional movement disorders (FMD) in China. The present survey aimed to investigate the views of FMD in Chinese clinicians. METHODS: The Chinese version survey of FMD were conducted in nationwide practitioners by means of an online questionnaire. RESULTS: Four hundred and thirty-four Chinese clinicians completed a 21-item questionnaire probing diagnostic and management issues in FMD. More than 80% of respondents considered that atypical movement disorder, multiple somatizations, and emotional disturbance were essenti

Electrodiagnostic Assessment of Radiculopathies.

This article discusses the electrodiagnostic assessment of radiculopathy. Relevant anatomy initially is reviewed followed by discussion surrounding the approach to nerve conduction studies and needle electrode examination when it comes to radiculopathy evaluation. Pitfalls of the electrodiagnosis versus clinical diagnosis of radiculopathy and the definitions of acute versus chronic, and active versus inactive, are reviewed.

Toxicologic Confounders of Brain Death Determination: A Narrative Review.

The aim of this narrative review is to describe the toxicologic confounders of brain death currently reported in the literature to offer guidance for physicians assessing brain death after a toxic exposure. We established an a priori definition of a "brain death mimic" as an unresponsive, intubated patient missing some, but not all brainstem reflexes. We completed a review of the literature utilizing MEDLINE and EMBASE to find case reports of patients of all ages in English, French, and Spanish meeting the criteria and hand searched the references of the results. We recorded xenobiotic dose, d

C. Miller Fisher and the Comatose Patient.

Neurologic examination of the comatose patient has gradually matured. Less than 50 years ago, neurological examination in coma became a regular part of textbooks with separate chapters devoted to the topic but many were deficient in detail. In 1969, C.M. Fisher published an extraordinary 56-page paper on the examination of the comatose patient. The paper-one of Fisher's gems-is not well known and infrequently cited. The many new observations collected in this comprehensive paper are reviewed in this vignette, which highlights not only how these contributions shaped our thinking on coma but als

Diagnosis and Differential Diagnosis of Parkinson Disease.

Parkinsonism is one of the most common neurologic disorders in the aging population. Although Parkinson disease (PD) is the most common cause, there is a lengthy differential diagnosis. The diagnosis of PD hinges on recognizing its typical features, including bradykinesia, rest tremor, unilateral onset, cogwheel rigidity, and beneficial and sustained response to levodopa. Equally important is to be familiar with the "red flags," which are features not expected with PD and suggest an alternative diagnosis, usually a parkinsonian syndrome. In general, it is best to have the diagnosis confirmed b

Under-Recognition of Cervical Dystonia: An Essential Tremor Patient with Numerous Textbook Features of Cervical Dystonia.

BACKGROUND: Physical examination findings of dystonia are often underrecognized, especially in the setting of other movement disorders such as essential tremor (ET). PHENOMENOLOGY SHOWN: A patient with ET exhibited numerous textbook features of cervical dystonia, which were misattributed to ET by a primary care physician and two neurologists. EDUCATIONAL VALUE: To provide a clear and unmistakable visual example of the clinically significant characteristics of cervical dystonia in the setting of concomitant ET.

[Neurological Signs and Symptoms of True Neurogenic Thoracic Outlet Syndrome].

Thoracic outlet syndrome (TOS) is a well-known disorder, but many aspects of its pathology, including its definition, has been disputed. True neurogenic TOS (TN-TOS) is a rare but well-defined clinical condition. TN-TOS results from the compression of the C8/T1 roots (dominant for the T1 root) or the proximal lower trunk of the brachial plexus by a fibrous band. The band extends from the first rib to either the tip of an elongated C7 transverse process or a rudimentary cervical rib. The most common presenting symptoms of TN-TOS are insidious-onset atrophy and weakness of the intrinsic hand mus

Pediatric migraine variants: a review of epidemiology, diagnosis, treatment, and outcome.

Pediatric migraine variants, previously known as childhood periodic syndromes, migraine equivalents, or migraine precursors, are a group of periodic or paroxysmal disorders occurring in patients who also have migraine with or without aura, or who have an increased likelihood of developing migraine. They have common key clinical features including periodic or paroxysmal character, normal neurological examination between attacks, family history of migraine, and clinical evolution to classic types of migraine. This article aims to review the pathophysiology, evaluation, and management of the pedi

A pattern recognition approach to patients with a suspected myopathy.

Myopathies are a heterogeneous group of disorders that can be challenging to diagnose. This review provides a diagnostic approach based predominantly on the clinical history and neurologic examination. Laboratory testing that can be used to confirm the suspected diagnosis based on this pattern recognition approach is also discussed. Careful consideration of the distribution of muscle weakness and attention to common patterns of involvement in the context of other aspects of the neurologic examination and laboratory evaluation should assist the clinician in making a timely and accurate diagnosi

Clinical features of diabetic polyneuropathy.

Diabetic polyneuropathy (DPN) has several forms of clinical presentation. It may be asymptomatic, or present with prominent sensory symptoms, including neuropathic pain. Clinical neurologic examination is helpful in identifying patients with distal sensory deficits, loss of deep tendon reflexes, early distal motor abnormalities and skin ulceration. Other causes of polyneuropathy are important to exclude. For serial evaluations and research purposes, several clinical severity grading scales have been developed.

Twelve clinical pearls to help distinguish essential tremor from other tremors.

The features of the tremor in essential tremor are often not fully appreciated, and essential tremor is frequently mis-diagnosed. Close inspection indicates that the tremor is characterized by a specific and definable pattern of features. Recognizing these features will aid in the diagnosis. The features are as follows: (1) kinetic tremor is greater than postural tremor, for postural tremor, (2) wrist tremor is greater than metacarpal joint tremor and wrist flexion-extension tremor is greater than wrist rotation tremor, (3) tremor is regularly recurrent and without directionality, (4) arm trem

A pattern recognition approach to myopathy.

PURPOSE OF REVIEW: Myopathies are a heterogeneous group of disorders that can be challenging to diagnose. The purpose of this review is to provide a diagnostic approach based predominantly on the clinical history and neurologic examination. Laboratory testing that can subsequently be used to confirm the suspected diagnosis based on this pattern recognition approach will also be discussed. RECENT FINDINGS: Over the past decade, numerous discoveries have allowed clinicians to diagnose myopathies with genetic testing. Unfortunately, the testing is extremely expensive and frequently not covered b
