Meningothelial hamartoma associated with nevus sebaceus.

Meningothelial hamartoma is a rare lesion of ectopic meningothelial elements that usually presents on the scalp at birth or in early childhood. The characteristic spindle cells infiltrate through collagen and stain positively for epithelial membrane antigen and vimentin but are negative for CD31 and CD34. This distinguishes meningothelial hamartoma from vascular tumors. Nevus sebaceus is a fairly common lesion that has been associated with several different benign tumors, including trichoblastoma and syringocystadenoma papilliferum. We describe the case of a 17-month-old girl who presented wit…

Hairy nevus spilus: a case series.

Nevus spilus, also known as speckled lentiginous nevus, is a nevoid disorder characterized by hyperpigmented macules or papules scattered over a background of tan pigmentation. Although nevus spilus is mainly of cosmetic concern, malignant melanoma may rarely develop in the lesions. Although classically not mentioned as a hairy nevus, a few reports in literature mention overlying hypertrichosis in lesions of nevus spilus. We hereby report four cases of nevus spilus without malignant change with overlying terminal hairs that arose mainly from the background pigmented area.

Malignant melanoma arising within nevus spilus.

A 68-year-old Caucasian man presented with a suspicious lesion near the left axilla during a full skin examination that was performed for a … he patient have a wide local excision of the biopsy site and the adjacent remaining portions of the nevus spilus. A sentinel lymph node biopsy and an oncologic evaluation were also performed. The sentinel lymph node biopsies, as well as a computed tomographic scan performed by oncology, showed no evidence of metastatic disease. Since the procedure, the …

Extensive intraoral blue nevus--case report.

Nevus is a congenital pigmented malformation rarely found in the oral mucosa. Around one third of cases located in this anatomical region are of the blue type, a histological variant with considerable tendency to malignancy. This study reports the case of a male patient, chronic smoker, with a blue nevus measuring 5 cm in diameter on the hard palate. Since controversy exists in the literature regarding the incisional biopsy of pigmented lesions with malignant or malignant potential, excision without previous biopsy of the lesion was the therapy of choice for this case. The patient was followed…

Verruciform xanthoma: a special epidermal nevus.

Congenital hemidysplasia with ichthyosiform nevus and limb defects (CHILD) syndrome is a rare X-linked hereditary disorder. Presentation varies from ichthyosiform nevus to complete limb amelia. We present a 17-year-old adolescent girl who presented with a 16-cm exophytic mass of the right foot that had been growing for 7 years as well as knee contracture. Deformed nails with onychorrhexis were noted bilaterally. History of multiple nonlinear erythematous skin lesions covered by dry waxy scales involving multiple body folds with sharp midline demarcation was obtained. The patient's family histo…

Congenital panfollicular nevus associated with polydactyly.

A 2-year-old girl presented with ulnar-sided duplication of the left thumb distal to the interphalangeal joint and syndactyly of the first web space. She also had several asymptomatic pink-tan cutaneous papules, involving the first and second ray of the left hand and wrist, clinically resembling a linear epidermal nevus. Microscopically, the papules were composed of well-circumscribed aggregates of basaloid epithelium within the dermis. No normal hair follicles were identified. Follicular germ and papillae were identified, representing abortive attempts at hair follicle formation. The features…

Congenital Spitz nevus clinically mimicking melanoma.

The differentiation between atypical variants of Spitz nevus and melanoma is often difficult given the many clinical and histopathologic similarities between the two. We report a case of an infant with a congenital scalp lesion exhibiting clinical features of melanoma, including variegation and regression of pigmentation and a rapidly changing appearance. Histologic examination of the excised lesion revealed a benign congenital Spitz nevus. This case emphasizes the need for clinical and histologic correlation in determining the benign or malignant nature of atypical pigmented lesions in infant…

New type of epidermal nevus syndrome.

An uncommon type of epidermal nevus characterized by systematized bands of non-epidermolytic hyperkeratosis with increased hairiness and follicular hyperkeratosis was observed in a 16-year-old boy who showed, in addition, hemihypoplasia of limbs, brachydactyly, clinodactyly and onychodystrophy. This case cannot be categorized within the group of presently established epidermal nevus syndromes. Most likely, this combination of anomalies represents a new type of epidermal nevus syndrome.

Epidermal nevus syndrome with maxillary involvement.

A female patient with epidermal nevus syndrome is reported. There were linear epidermal nevi, hemihyperplasia of the limbs and tongue, macrocephaly, several ophthalmic malformations, and multiple radiolucent lesions in the limbs and sacroiliac region. At age 14 years, she developed a giant cell granuloma of the maxilla.

Quadrant distribution of dysplastic nevus syndrome.

A 59-year-old man with numerous aggregated pigmented lesions confined to the left upper quadrant of his body is described. The lesions consist of lentigines, common acquired nevi, and dysplastic nevi. In addition, within this quadrant two malignant melanomas at different stages of progression developed from dysplastic nevi. This patient presents a hitherto undescribed quadrant type of dysplastic nevus syndrome. As quadrant syndromes are due to single mutations, our observation strongly supports the concept of an autosomal-dominant trait in dysplastic nevus syndrome.

Epidermal nevus misinterpreted as child abuse.

Determining what constitutes true child abuse is not always easy. A child with epidermal nevus and multiple café-au-lait spots was erroneously thought to be a victim of abuse. This report details the features of epidermal nevi. We present these findings with the hope that such misdiagnoses can be avoided in the future.

Regional odontodysplasia in epidermal nevus syndrome.

Epidermal nevus syndrome is defined as a combination of nevoid skin alterations, epileptic seizures, and psychomotoric retardation. Involvement of other organs, especially the eyes, may also occur, but is not obligatory. The present report is concerned with a case that combined regional odontodysplasia with nevoid skin alterations in the overlying skin area of the face. This observation suggests that odontodysplasia may be the result of aberrations in the development and migration of cells of the neural crest occurring early in embryonic life.

[Incontinentia pigmenti achromians (systematized depigmented nevus)].

Two girls of 1 1/2 und 4 years age and described with linear, arch-like depigementations which appeared in early childhood. In both children no additional abnormalities could be found. The problem of terminology is briefly discussed, and it is proposed to interpret this syndrome as a variant of a systematized naevus depigmentosus. There seems to be no justification for turning this clinical entity into a new syndrome, when the only clinical signs are the characteristic depigmentations without any additional abnormalities.

[Superficial cutaneous nevus lipomatosus (Hoffmann-Zurhelle)].

Neviform hand-sized yellowish or skin colored plaques or linear lesions with flattened papules or folded surface in the gluteal regions, especially in females, point out to nevus lipomatosus superficialis (Hoffmann-Zurhelle). These nevi consisting of mature fat cells among the dermal collagen are rare lesions. The case report of two female patients should remind of this clinical and histological picture.
