Serum visfatin levels in patients with axial spondyloarthritis and their relationship to disease activity and spinal radiographic damage: a cross-sectional study.

The purpose of this cross-sectional study was to assess the visfatin levels in patients with axial spondyloarthritis (axSpA) and to investigate the association between visfatin, disease activity and radiographic spinal damage. Serum visfatin levels were determined by enzyme-linked immunosorbent assay in 64 patients with axSpA (46 with radiographic axSpA (r-axSpA) and 18 with non-radiographic axSpA (nr-axSpA)) and 61 age-/sex-matched healthy individuals. Patients with r-axSpA were further divided into two subsets based on radiographic spinal damage using modified Stoke Ankylosing Spondylitis Sp

Advances in treatment of axial spondyloarthritis are associated with improved patient outcomes: data from the Ankylosing Spondylitis Registry of Ireland (ASRI).

The Ankylosing Spondylitis Registry of Ireland (ASRI) captures both radiographic and non-radiographic axial spondyloarthritis (axSpA) in a large, well characterised cohort. This is a valuable resource for studies in therapeutics and burden of disease, following a period of rapid change in the field of axSpA. This study aims to perform a focused analysis on patient outcomes and pattern of medication usage in axSpA. This is a cross-sectional study of registry data on 885 patients with confirmed axSpA as per the ASAS criteria for axSpA, as diagnosed by a Rheumatologist. Analysis was performed usi

JAK inhibitors, cardiovascular and thromboembolic events: what we know and what we would like to know.

JAK inhibitors (JAKinibs) have been approved for several immune-mediated inflammatory diseases (IMIDs), including rheumatoid arthritis, psoriatic arthritis, axial spondyloarthritis (axSpA), and non-radiographic axSpA. Although they have been proved to be very effective, some safety concerns have emerged. These mainly pertain to their profile regarding major adverse cardiovascular events (MACEs) as well as to thromboembolic events (VTEs). In fact, there are accumulating data showing that the concerns might be greater for the latter. Herein, we provide a critical analysis of the so far published

Dynamics of interleukin-23/interleukin-17 axis and type 1 regulatory T cells across disease stages in axial spondyloarthritis.

OBJECTIVES: The interleukin (IL)-23/IL-17 pathway is central to the pathogenesis of axial spondyloarthritis (axSpA); however, treatments targeting IL-23 have shown inconsistent effectiveness across spondyloarthritis subtypes. We hypothesised that the IL-23/IL-17 axis varies with disease stage and is modulated by its regulatory counterparts, such as type 1 regulatory T (Tr1) cells. METHODS: Serum levels of IL-17A and IL-23, along with peripheral CD4+IL-17A+ cells, monocytes (CD14+HLA-DR+), dendritic cells (DCs, CD11c+HLA-DR+), and Tr1 cells (CD4+CD49b+LAG3+FoxP3-IL-10+), were analysed in 20 pa

Heterogeneity of axial spondyloarthritis: genetics, sex and structural damage matter.

OBJECTIVE: Axial spondyloarthritis (axSpA) comprises both radiographic and non-radiographic disease. However, the paucity of specific objective measures for the disease and current classification criteria showing suboptimal specificity contribute to disease heterogeneity observed in clinical practice and research. We used a historical cohort of patients with axSpA to assess sources of heterogeneity. METHODS: The study involved 363 axSpA probands recruited from membership of the Swiss Ankylosing Spondylitis Patient Society. Participants underwent examination by a rheumatologist, completed ques

Clinical and molecular links between chronic systemic inflammation and structural damage in axial spondyloarthritis.

BACKGROUND: Structural damage in axial spondyloarthritis (axSpA) is closely linked to chronic inflammation, yet the underlying mechanisms remain incompletely understood. For that reason, we investigated the relationship between persistent systemic inflammation and radiographic damage in axSpA and characterized the associated molecular and cellular pathways. METHODS: A retrospective registry of 238 axSpA patients was stratified by C-reactive protein (CRP) trajectory over the last 5 years into non-persistent, intermediate, and persistent inflammation groups. Radiographic damage, serum bone metab

Genetics, Epigenetics, and Gender Impact in Axial-Spondyloarthritis Susceptibility: An Update on Genetic Polymorphisms and Their Sex Related Associations.

Spondyloarthritis (SpA) is a group of chronic inflammatory rheumatic disease that can be divided into predominantly axial or predominantly peripheral involvement, with or without associated psoriasis, inflammatory bowel disease or previous infection. Axial SpA (axSpA) encompasses ankylosing spondylitis (AS) with radiological sacroiliitis, and a type without radiographic sacroiliitis, called "non-radiographic axial SpA" (nr-axSpA). Males and females show large differences in their susceptibility to SpA, such as distinctions in clinical patterns, phenotypes and in therapeutical response, particu

Biomarkers in axial spondyloarthritis and low back pain: a comprehensive review.

The spectrum of axial spondyloarthritis (AxSpA) (including both non-radiographic and radiographic AxSpA), also known as ankylosing spondylitis AS, has achieved growing recognition. With the development of treatments not only effective in controlling disease activity but also in slowing radiographic progression, and given the cost and risk profiles of these novel treatments and the limitations of current clinical criteria, imaging and peripheral blood biomarkers (C-reactive protein, HLA-B27 testing), the need for better biomarkers has never been greater. The purpose of this review is to present

Certolizumab pegol for treating axial spondyloarthritis.

INTRODUCTION: Axial spondyloarthritis (axSpA) is a chronic inflammatory disease of the spine and sacroiliac (SI) joints. The spectrum of axSpA includes ankylosing spondylitis (AS) and non-radiographic axSpA (nr-axSpA). Evidence has supported the use of TNF alpha inhibitors (TNFi) in treating these diseases, with good efficacy and tolerable safety profiles. Certolizumab pegol (CZP) is an anti-TNF alpha (TNFa) agent with data to support its use in both AS and nr-axSpA. AREAS COVERED: The pharmacologic properties of CZP were reviewed. Data regarding the use and efficacy of CZP in axSpA were revi
