Assessment of hyperbilirubinemia in full-term infants: Part I.

Nurse practitioners are in a key position to assess one of the most common problems in the full-term infant, unconjugated hyperbilirubinemia. Armed with a clear understanding of the pathophysiologic pathways that may cause and/or contribute to the development of unconjugated hyperbilirubinemia and the associated jaundice, the practitioner will be successful in helping the family understand their child's illness. A careful history and physical examination will guide the practitioner to the appropriate diagnostic studies, which will confirm that the infant has unconjugated hyperbilirubinemia and

Amplitude-Integrated Electroencephalography Interpretation During Therapeutic Hypothermia: An Educational Program and Novel Teaching Tool.

Therapeutic hypothermia (TH) is now considered a standard in tertiary NICUs. Amplitude-integrated electroencephalography (aEEG) is an important adjunct to this therapy and is gaining acceptance for use on the neonatal population. It can be easily incorporated into practice with appropriate education and training. Current publications are lacking regarding nursing care of neonatal patients undergoing th with the use of aEEG. This article presents a broad educational program as well as novel teaching tool for neonatal nurses caring for this population.

The conceptualization, measurement, and validation of transient mechanical birth trauma.

An index of transient mechanical birth trauma (TMBT), consisting of the presence or absence of molding, cephalohematoma, subconjunctival hemorrhage, body bruising, facial bruising, petechiae, forceps marks, diminished arm movements, and sensitivity to sudden position changes, was measured on a convenience sample of 196 healthy newborns. Six dimensions of the Neonatal Behavioral Assessment Scale (NBAS) and other newborn measures also were assessed. Vaginally delivered newborns had more TMBT than those delivered by cesarean section and of newborns delivered vaginally, macrosomics had more TMBT t

A nurse's guide to common mechanical ventilation techniques and modes used in infants. Nursing implications.

The need for conventional mechanical ventilation (CMV) is a common one in the neonatal intensive care unit (NICU). The goals of CMV are to facilitate adequate gas exchange, minimize the risk of lung injury/damage, decrease the patient's work of breathing, and optimize the patient's comfort. Although time-cycled, pressure-limited ventilation remains the most common CMV modality, volume-cycled ventilation, assist-control ventilation, pressure-support ventilation, and pressure-control ventilation are sometimes used in the NICU. Pressure-regulated volume control, volume-guaranteed ventilation, vol

Therapeutic hypothermia to treat hypoxic ischemic encephalopathy in newborns: implications for nurses.

Hypoxic ischemic encephalopathy (HIE) in newborns is caused by an injury to the brain following a hypoxic or an ischemic event during the peripartum, intrapartum or postpartum period. HIE may result in death or cause serious impairment in survivors, and remains a significant cause of morbidity and mortality among neonates. Mild hypothermia as a treatment for HIE is commonly used to treat moderate to severe HIE, with promising results. Nurses play an integral role in identifying newborns at risk of developing HIE.

A survey of gastric tube practices in level II and level III nurseries.

Gastric tube placement practices were identified by a telephone survey of 113 Level II and Level III nurseries in five states (Indiana, Kentucky, Michigan, Ohio, and Pennsylvania). The purpose was to examine current practice and the rationale for practice with fragile preterm infants. Twenty-one percent of the nurseries inserted only orogastric (OG) tubes, 13% used only nasogastric (NG) tubes, and 66% used both NG and OG tubes. Of those that placed an NG or OG tube, 50% did so continuously, 5% placed it intermittently, and 45% placed the tube both continuously and intermittently, for different

Home care concerns for the normal newborn.

Four of the most common problems encountered with newborns in the home include feeding difficulties, jaundice, gastrointestinal problems, and inconsolable crying. This article suggests assessment measures and intervention strategies to deal with these problems and evaluate the home environment for safety. This article also includes suggestions to promote safe infant feeding, bathing, sleeping arrangements, car safety seats, firearm and poison storage, fall prevention, concerns about pets, pest control, heating and cooling, and a nonsmoking environment.

Workshop-based participatory research design through the lens of a culture-centered approach (CCA).

PURPOSE: Nurses in the neonatal intensive care unit (NICU) play a crucial role in health communication, yet their voices are often overlooked. This study explores communication challenges from nurses' perspectives to develop a sustainable communication infrastructure. METHODS: A workshop-based participatory design (WBPD) was used, where NICU nurses worked collaboratively to identify communication barriers. Thematic analysis was conducted using the culture-centered approach (CCA) to ensure context-specific solutions, integrating both researcher insights and NICU-based communication infrastruct

National neonatal resuscitation training program in Nigeria (2008-2012): a preliminary report.

BACKGROUND: Routine institutional training of doctors and nurses on newborn resuscitation have commenced, to improve the quality of resuscitation available to high-risk babies, in Nigeria, as a means of reducing newborn deaths in the country. Perinatal asphyxia contributes to 26% of newborn deaths in Nigeria. Perinatal asphyxia results when babies have difficulty establishing spontaneous respiration after birth. MATERIALS AND METHODS: Between 2008 and 2012, doctors and nurses drawn from all the geo-political zones were trained using the Neonatal Resuscitation Training (NRT) manual of the Amer

Late preterm birth: a new nursing issue.

Late preterm infants are those born between 34 and 36 6/7 completed weeks' gestation. In the last decade, late preterm infants have become the fastest growing subset of preterm infants and now account for 74% of all preterm births. They are at greater risk for feeding problems, dehydration, hypothermia, jaundice, and hypoglycemia and are more likely to be readmitted to the hospital in the first weeks after birth and accrue greater healthcare costs as a result. Despite the alarming growth of this population and the acknowledgment of increased risk in the literature, there is limited information

NIRS Monitoring for Neonates With Mild Hypoxic-Ischemic Encephalopathy.

BACKGROUND: Despite evidence of cerebral vulnerability in the early postnatal period, neonates diagnosed with mild hypoxic-ischemic encephalopathy (HIE) may not routinely receive continuous neuromonitoring, limiting opportunities for timely neuroprotective interventions. Near-infrared spectroscopy (NIRS) offers continuous, noninvasive monitoring of cerebral oxygenation; however, standardized, nurse-driven, clinical practice guidelines (CPG) for NIRS use in this population are lacking.
