The author presents an account on different causes of ophthalmological torticollis and rotational nystagmus in paresis of the trochlear nerve and abducent nerve, in Duan's syndrome I or acute Brown's syndrome or in bilateral ptosis combined with paresis of the levators. He recommends different surgical techniques and prefers combined operations performed with a single anaesthesia in pareses of the levators, the abducent nerve and trochlear nerve. For release of the horizontal direct muscles within the framework of paretic affections of the eye he recommends adjustable elongations as described

A 52-year-old woman was admitted to hospital because of sudden loss of consciousness. Neurological examination on admission revealed a comatose consciousness level, horizontal and upward palsies of both eyes and quadriparesis. The diagnosis of pontine hemorrhage was confirmed based on a brain CT scan, which showed a high density area involving the bilateral pontine tegmentum and right midbrain tegmentum at the inferior collicular level. The left eye moved downward below its primary position, and the movements usually comprised rapid downward and slow upward excursions and, on occasion, slow do

BACKGROUND: Sarcoidosis is a granulomatous inflammatory disease that may have a variety of ocular and orbital manifestations. The most common ocular manifestation is uveitis, and the most common orbital manifestation is dacryoadenitis. Extraocular muscle involvement in sarcoidosis has rarely been reported. The authors report a case of sarcoidosis involving the extraocular muscles of a 15-year-old boy with bilateral, painful, external ophthalmoplegia and enlargement of all extraocular muscles on computed tomography (CT) scan.
RESULTS: Lateral rectus muscle biopsy and transbronchial lung biopsy

The ocular motor abnormalities that commonly occur in Wallenberg's lateral medullary syndrome are often unappreciated. These abnormalities include signs of dysfunction of ocular alignment (skew deviation, ocular tilt reaction, and environmental tilt), various types of nystagmus, smooth pursuit and gaze-holding abnormalities (eye deviation, ipsipulsion or lateropulsion, and impaired contralateral pursuit), and saccadic abnormalities (ipsipulsion and torsipulsion). These impairments of ocular motor control and their proposed mechanisms are discussed.

A century ago Hess described how to map out ocular deviations on two pieces of paper in cases of diplopia. This paper discusses how to obtain useful information regarding ocular deviations from a chart that has been obtained from screen tests (Hess, Lancaster, Lees, Weiss).

Mild traumatic brain injury (mTBI) is among the most common neurological presentations worldwide, yet its diagnosis remains largely clinical and its prognosis difficult to predict. Structural imaging is typically normal and self-reported symptom scales are insensitive and confounded by effort and comorbidity. Because the generation and control of eye movements recruit an unusually large and anatomically distributed proportion of the brain-from brainstem and cerebellar circuits to parietal, frontal and prefrontal cortices linked by long white-matter tracts-ocular-motor behaviour is sensitive to

Oculomotor disorders are not typical manifestations of amyotrophic lateral sclerosis (ALS). Occasionally, this disease is associated with vertical gaze paresis, presenting a distinct type as «ALS+progressive supranuclear palsy». Studies using eye-tracking methods have revealed a variety of subclinical oculomotor disorders in this disease. These disorders can manifest as changes in reflex and voluntary saccades, antisaccades, smooth tracking eye movements, and fixations. A significant association between oculomotor disorders and clinical manifestations of ALS was reported. The occurrence of ocu

SIGNIFICANCE: Intervention strategies for post-mild traumatic brain injury (mTBI) ocular motor disorders vary across disciplines and include watchful waiting, vestibular rehabilitation, vision rehabilitation/vision therapy, and optical intervention. However, evidence supporting their effectiveness is limited, highlighting the need for high-quality randomized controlled trials with standardized testing, diagnostic criteria, and reassessment of ocular motor function after intervention.
BACKGROUND: Ocular motor disorders occur frequently after mTBI.
OBJECTIVES: This study aimed to conduct a sco

BACKGROUND: Opsoclonus is a rare disorder characterized by conjugate multidirectional, horizontal, vertical, and torsional saccadic oscillations, without intersaccadic interval, resulting from dysfunction within complex neuronal pathways in the brainstem and cerebellum. While most cases of opsoclonus are associated with autoimmune or paraneoplastic disorders, infectious agents, trauma, or remain idiopathic, opsoclonus can also be caused by medications affecting neurotransmission. This review was prompted by a case of opsoclonus occurring in a patient with Multiple System Atrophy, where amantad

When assessing the acutely dizzy patient, the HINTS 'Plus' (Head Impulse, Nystagmus, Test of Skew, 'Plus' a bedside assessment of auditory function) exam is a crucial component of the bedside exam. However, there are additional ocular motor findings that can help the clinician distinguish peripheral from central etiologies and enable accurate localization, especially when the patient has acute dizziness, vertigo and/or imbalance but without spontaneous nystagmus. We will review the literature on these findings which are 'beyond HINTS' and include saccades/ocular lateropulsion, smooth pursuit,

A primary intraventricular haemorrhage (PIVH) usually presents with non-localised neurological symptoms since the haematoma is limited to the ventricles. However, it is sometimes associated with focal neurological signs, whose pathophysiologies are not confirmed. Here, we report on a case of PIVH who showed rare manifestations in the acute stage: upward gaze palsy and convergence insufficiency. The CT and MRI showed intraventricular haematoma without evidence of parenchymal haemorrhage, local mass effect around midbrain or hydrocephalus. There had been bilateral papilloedema, and it resolved a

Traditionally, clinicians have used their experience and intuition to diagnose and treat disease states, including neurological disorders. However, the rapid increase in basic knowledge, coupled with a realization that human judgments are often flawed, has made it helpful to approach many clinical disorders by casting them in the form of models (quantitative hypotheses) that can be tested experimentally; in this way the power of the scientific method can be applied. This is especially the case in systems neuroscience, in which the experimental testing of mathematical models has proven an effec
