The use of povidone-iodine in ophthalmology.

PURPOSE OF REVIEW: Povidone-iodine (PVI) is a disinfectant and antiseptic agent used for preoperative preparation of the skin and mucous membranes, as well as for the treatment of contaminated wounds. Currently regimens for prophylaxis against postsurgical endophthalmitis are being modified, including a total withdrawal of antibiotics in intravitreal injections. The aim of this study was to evaluate the use of PVI in ophthalmology. RECENT FINDINGS: As a result of its broad spectrum of microbicidal activity, PVI is routinely used in ophthalmic surgery. However, various protocols are applied wo

Longitudinal trends in volume of oculoplastic procedures without ACGME minimum requirements among United States ophthalmology residents: an ACGME case log analysis.

PURPOSE: To our knowledge, there have been no previous reports that investigate trends in ophthalmology residents' exposure to oculoplastic procedures without Accreditation Council for Graduate Medical Education (ACGME) minimum requirements. To fill this knowledge gap, we investigated publicly available ACGME ophthalmology residency case logs from 2009 to 2021. METHODS: National resident averages and standard deviations were collected for the following oculoplastic procedures without minimum ACGME requirements: eye removal and implant, lacrimal surgery, other orbital surgery (e.g. orbitotomy)

"Practicing in and out till perfection"-Use of mobile repairing microscope for home wet lab practice and as an alternative to high-end microscopes in wet lab.

Ophthalmic surgeries are microsurgeries that require fine motor skills and precision of movements. Practicing under microscope during wet lab sessions develops hand-eye coordination, removes performance anxiety, builds muscle memory, and helps attain fineness of movement. However, not every training center is equipped with a wet lab. Development of a wet lab facility is an expensive task, as the cost of operating microscopes, simulators, and phacoemulsification machines is significant. A mobile repairing tabletop, portable stereo microscope can be utilized to develop a basic, affordable wet la

Surgical capacity in ophthalmology: the unmet need for sustainable solutions.

The increasing prevalence of eye diseases is placing significant pressure on surgical and procedure-based ophthalmology services worldwide. Delays in surgical care can lead to poorer patient outcomes and reduced treatment efficacy, highlighting the urgent need for healthcare systems to address these challenges. This narrative review provides a broad overview of surgical and procedure-based ophthalmology capacity constraints across five countries (UK, Germany, Australia, Singapore, and India) to identify cross-cutting, system-level challenges that transcend individual diseases or interventions,

A case of a slipped medial rectus muscle after strabismus surgery.

BACKGROUND: A slipped muscle after strabismus surgery is a well known complication, but only a few cases have been reported in the medical literature. Here, we report a case of a slipped medial rectus muscle that occurred after strabismus surgery for esotropia. CASE REPORT: The patient was a 10 year old male with an acquired esotropia of 20 PD at distance. For treatment, 3.5 mm bilateral medial rectus recessions was performed. There were no complications during the procedure. On postoperative day 8, the eye position was orthotropic in primary position, but a right exotropia was observed in le

Global trends and practice patterns in virtual reality simulation training for ophthalmic surgery: an international survey use of virtual reality simulation training around the world.

This global survey investigated the use of virtual reality simulation (VRS) in ophthalmological surgery education. Questionnaires were distributed to authors of publications and directors of centers using VRS for surgical education in ophthalmology, then forwarded to residents and fellows of their team for completion. Out of 1845 questionnaires sent across 36 countries, 170 responses from 26 countries were analyzed, primarily from residents and fellows (75%). Mean access duration to VRS was 3.6 years, often at University Hospitals (43.5%). Notably, 12% of respondents traveled an average of 550

Management of intraocular foreign bodies: a clinical flight plan.

A systematic review of the literature on the diagnosis and management of traumatic intraocular foreign bodies (IOFBs) is presented together with a schematic "flight plan" to assist in clinical decision making when confronted with an IOFB. Several large retrospective series of IOFB have been published recently, with relevant observations regarding prognostic factors, endophthalmitis and retinal detachment incidence, timing of surgical intervention, and preventative measures. Eye trauma and IOFB, in particular, remain poorly suited to prospective study because of their variability. With few exce

The best of the best: a review of select oculoplastic case series published in 2015.

This review summarizes three case series published in the field of oculoplastic surgery in the year 2015. The first article describes the use of hedgehog pathway inhibitors for medical therapy of advanced periocular basal cell carcinoma and basal cell nevus syndrome. The second describes the use of c-reactive protein as a marker for starting treatment with steroids in children with orbital cellulitis. The third article presents an endoscopic medial orbital fat decompression technique for treatment of proptosis in thyroid eye disease.

Is blindness a realistic complication in blepharoplasty procedures?

Blindness is a rare but realistic complication in blepharoplasty procedures. Review of 55 case reports in the literature supports this view. The cause or causes of blindness are not precisely clear, but orbital hemorrhage after removal of fat seems to be the most common event associated with visual loss. Preoperative ophthalmologic assessment is highly desirable to document preexistent eye and systemic disease. Exquisite care with removal of fat is essential. Careful postoperative observation without patching is necessary. Management of acute visual loss may be medical, surgical, or both.

Nontraditional approaches to sedation for anterior segment surgery: a narrative review.

PURPOSE OF REVIEW: Surgical and anesthetic technological advancement have made both cataract and noncataract anterior segment surgery significantly less invasive and time-intensive, facilitating the transition of some of these procedures from the operating room under monitored anesthesia care (MAC) to the office-based setting without MAC. This transition has been aided by the popularization of nonintravenous approaches to achieving patient sedation for these procedures. In this review, we discuss the literature surrounding traditional and nontraditional methods of achieving patient sedation fo

Trifold technique for pupil reconstruction in large non-appositional iris defects.

Large iris defects are challenging to close due to shortage of tissue and opposite vector force. By using two pupilloplasty methods, we can close large iris defects with less tractional force or tethering. The trifold technique is a combination of trocar-assisted iris repair and single-pass four-throw (SFT) pupilloplasty for non-appositional large iris defects. The initial two trocar-assisted iris repair sutures oppose the unopposed iris ends and the subsequent SFT seals the defect. The combination procedure is less manipulative with no complications for the iris and pupil diaphragm.

[Late complications after chemical burns of the ocular surface. Surgical strategies for ocular surface reconstruction].

Severe chemical burns of the ocular surface frequently result in long-standing and ongoing disorders of the conjunctiva and the cornea including conjunctival scarring with shortening of the fornix, cicatricial entropion, complications caused by trichiasis and scarring or chronic ulceration of the cornea. Advanced destruction of limbal stem cells leads to limbal stem cell deficiency with conjunctivalization of the cornea. Surgical therapy aims at the correction of malpositioning of the lids, the reconstruction of the conjunctiva and the fornix utilizing applicable tissue grafts and the reestabl

Evolution of thyroid eye disease decompression-dysthyroid optic neuropathy.

Orbital decompression surgery and medical therapy for thyroid eye disease (TED) have evolved over the past 150 years and afforded the opportunity to restore pre-disease appearance and visual function. This manuscript explores the past 150 years of surgical innovation for the treatment of TED. The "Age of Surgical Heroism" spans the time from 1888 to 1979 during which the pioneers of orbital decompression developed lateral orbitotomy, transcranial decompression, paranasal sinus decompression, and transantral decompression despite an incomplete understanding of the pathophysiology of both TED an
