Ophthalmologists' and patients' perspectives on treatments for diabetic retinopathy and maculopathy in Vietnam: a descriptive qualitative study.

INTRODUCTION: Globally, diabetic retinopathy (DR) is the leading cause of blindness in working-aged adults. Early detection and treatment of DR is essential for preventing sight loss. Services must be available, accessible and acceptable to patients if we are to ensure they seek such care. OBJECTIVES: To understand patients' knowledge and attitudes towards laser versus antivascular endothelial growth factor (VEGF) injections to treat DR in Vietnam, and to identify factors Vietnamese ophthalmologists consider when making treatment decisions. METHODS: This is a descriptive qualitative study ba…

Urbach-wiethe syndrome and the ophthalmologist: review of the literature and introduction of the first instance of bilateral uveitis.

Patients suffering from Urbach-Wiethe syndrome (UWS), also known as lipoid proteinosis or hyalinosis cutis et mucosae, may have an ophthalmo … . This case was thought to be the first instance of bilateral uveitis associated with UWS. Overall, ophthalmologists may encounter diverse ocular complications accompanying this syndrome. They should be familiar with well-established ophthalmologic manifestations leading them to cooperate with other specialists in diagnosis and management of the disease.

Clostridium botulinum and the ophthalmologist: a review of botulism, including biological warfare ramifications of botulinum toxin.

The anaerobic bacterium Clostridium botulinum causes disease by elaborating an extremely potent neurotoxin that inhibits release of acetylcholine at presynaptic nerve endings, thereby resulting in a descending flaccid paralysis and autonomic nervous system dysfunction. Possible ophthalmological effects of this neurotoxin are many and typically constitute the earliest manifestations of botulism. This review summarizes the medical literature on botulism with regard to historical perspective, epidemiology, clinical manifestations, and treatment. Ophthalmological findings of botulism are tabulated

Glaucoma care and conformance with preferred practice patterns. Examination of the private, community-based ophthalmologist.

BACKGROUND: Practice guidelines are becoming more prominent in the provision of medical care. A previous study demonstrated a high rate of c … up within PPP-recommended time intervals. CONCLUSIONS: Chart reviews from private, community-based ophthalmologists show that some initial examination steps such as gonioscopy and pupil examination are performed to widely varying degrees. While patients are generally likely to be scheduled for follow-up within PPP-recommended intervals, patients with unstable gla …

A survey of color discrimination in German ophthalmologists. Changes associated with the use of lasers and operating microscopes.

Color vision tests were performed on 211 German ophthalmologists during their annual meeting at Essen. The subjects also answered detailed questionnaires about their use of lasers and operating microscopes, and their ocular and general health. It was found that 33% of doctors who use lasers or operating microscopes have decreased color discrimination for colors in a tritan color-confusion axis (greater than 2 standard deviations above normal). There is a relationship between number of patients treated and the degree of threshold elevation. Thirty hours of using the operating microscope produce…

[Traumatology of the midface--diagnostic and therapeutic guidelines for the practicing ENT physician from the viewpoint of the ophthalmologist].

Fractures of the midface combined with orbital injuries endanger vision, binocular vision, and the lacrimal system. The best results of primary surgical management are advised if the surgeon adheres to a strict time schedule. For example, a perforating injury of the eye must be diagnosed as early as possible, and be repaired immediately. Otherwise, all manipulations necessary for reconstruction of the bone and soft tissue of the face worsen the condition of the opened eyeball, including loss of its contents and function. On the other hand, competent repair of the lacrimal system may be done at

Ophthalmological care delivery in East Kazakhstan: a qualitative study of ophthalmologists on system barriers and service organization.

BACKGROUND: Access to ophthalmologic care is essential for maintaining eye health and preventing vision loss. However, barriers within health systems may limit timely access to services. Understanding these barriers from the perspective of healthcare providers may help improve the organization and accessibility of ophthalmological care. METHODS: A qualitative study was conducted in the East Kazakhstan region using semi-structured interviews with 22 ophthalmologists working in primary healthcare and hospital settings. Interviews explored public awareness, workforce capacity, service provision,…

Ophthalmologists' Perceptions of the Impact of Patient Mental Status on Clinical Decision-Making: A Cross-Sectional Survey.

PURPOSE: To investigate ophthalmologists' perceptions of how patients' mental status influences ophthalmic clinical practice and to identify ocular diseases perceived by ophthalmologists as having the greatest psychological impact. SUBJECTS AND METHODS: This exploratory cross-sectional questionnaire survey included 61 ophthalmologists from a regional ophthalmologists' association in Japan (41 men and 20 women; mean clinical experience, 26 ± 14 years). Participants rated the extent to which they considered patients' mental status during treatment selection using a 10-point scale (1 = not consi…

FunduScope: a human-centered, machine learning-based interactive tool for training junior ophthalmologists in diabetic retinopathy detection.

Interpreting fundus images is an essential skill for detecting eye diseases, such as diabetic retinopathy (DR), one of the leading causes of visual impairment. However, the training of junior doctors relies on experienced ophthalmologists, who often lack the time for teaching, or on printed training materials that lack variability in examples. In this work, we present FunduScope, an interactive human-centered learning tool for training junior ophthalmologists, which is based on a pre-trained ML model for classifying DR. In a qualitative pre-study, we investigated the needs of junior doctors an…

Impact of the COVID-19 Pandemic on Ophthalmologists Residents Well-being and Training Programs: A Systematic Review.

BACKGROUND/AIM: The COVID-19 pandemic significantly impacted residents' well-being and training programs, with potentially severe consequences for specialties like ophthalmology, where residents were often reassigned to non-specialty-related units and tasks. This study aimed to systematically review the pandemic's effects on ophthalmology residents' well-being and training. PATIENTS AND METHODS: A systematic review of the literature was carried out using MEDLINE, Scopus, and EBSCO databases. After removing duplicates, 173 unique records were identified, and 30 were considered as potentially r

Microbiological alterations in the conjunctiva of hot tub-soaking ophthalmologists (MACHO): a randomized double-blind clinical trial.

BACKGROUND: To determine if there is a difference in the quantity of microbial flora of the conjunctiva in individuals practicing head submersion ("dunk") versus no head submersion ("no-dunk") during hot tub use. METHODS: In this double-blind randomized clinical trial, healthy volunteers aged ≥ 18 years were recruited. Participants were randomized to head submersion versus no head submersion during a 15-minute hot tub soak. Study personnel, masked to the dunk or no-dunk group assignment, obtained conjunctival cultures before and immediately after hot tub use. De-identified specimens were subm
