Correct orthopedic therapy for traumas of the cervical rachis requires perfect knowledge of the spatial balance of the fracture focus. The authors believe Computed Tomography (CT) to be the most suitable, and often indispensable, method for this purpose. Sublaminar wiring provides strong and effective fixation of the scoliotic or unstable spine, but its long-term effects on the spinal canal remain unknown. An animal model was developed to observe these effects on the growth and development of the immature spine over a two-year period. Laminar overgrowth occurred both longitudinally to produce a kyphoscoliosis and in the transverse plane to cause significant laminar thickening and growth into the spinal canal. However, the cross-sectional area of the spinal canal was not significantly compromised.

An analysis has been made of the records of 23 cases of resection-reconstruction of the shafts of long bones for a variety of tumours of bone. Four were in the humerus, four in the forearm bones, five in the femur and ten in the tibia. From January 1978 until March 1985 more than 500 patients were operated on by posterolateral distraction spondylodesis using Zielke's twofold sacral bar for stabilization of the lumbosacral joint. A significant abduction contracture of the hip followed closed reduction and pinning of an acute slip of the proximal femoral epiphysis in a 13-year-old boy. The contracture was corrected by removal of this bony prominence. Over-reduction of the slipped epiphysis into valgus is thought to have stimulated bony overgrowth at the posteromedial aspect of the capital femoral epiphyseal-neck junction. This causes a block to adduction by abutting against the inferior lip of the acetabulum. To our knowledge, this complication has not been previously reported.

Experiences in management of paralytic foot deformities in 30 children and adolescents with myelomeningoceles are reported. Suitable reconstruction of foot form can be achieved regularly with the help of early manipulation, walking plasters and orthopaedic surgery. However, the impairment of sensation remains unchanged. The polypropylene solid ankle orthosis has proved particularly useful for maintaining good weight-bearing abilities of the foot combined with partial load transfer to sensitive skin regions below the knee joint.
