[Thromboembolic disease in orthopedic surgery].

Thromboembolic complications are frequent after orthopaedic surgery of the limbs. Strategies to minimize these complications go from recognize the risk factors of the patient to introduce new antithrombotic drugs, and including general medical care, regional anaesthesia and early mobilization. Based in epidemiological studies, prophylaxis must be extended 4-6 weeks in total hip and knee arthroplasty and in surgery of the hip fracture.

[Use of bisphosphonates in orthopedic surgery].

Over the past three decades, the members of the substance group called bisphosphonates (BP) have been employed with growing success to manage osteopathies caused by increased osteoclastic activity. The following developments in BP are responsible: Modern BP are now already 20,000 times more potent than the first preparation approved for use. Their biochemical and cellular mechanisms of action have meanwhile been elucidated. They have no effect on hormones so that they are open for all patients. They are well tolerated and can be administered orally or intravenously. They have admirably been th

Selfprotective smart orthopedic implants.

In this review, we discuss current advances leading to an exciting change in implant design for orthopedic surgery. The initial biomaterial approaches in implant design are being replaced by cellular-molecular interactions and nanoscale chemistry. New designs address implant complications, particularly loosening and infection. For infection, local delivery systems are an important first step in the process. Selfprotective 'smart' devices are an example of the next generation of orthopedic implants. If proven to be effective, antibiotics or other active molecules that are tethered to the implan

Orthopedic involvement in antiphospholipid syndrome.

Antiphospholipid syndrome (APS) is a common autoimmune disease, manifested by vascular thrombosis and fetal loss in the presence of antiphospholipid antibodies. Orthopedic involvement is a relatively novel and under-recognized feature of APS. In this article we review the association of primary, secondary, and catastrophic APS with diverse orthopedic conditions, including osteonecrosis in adult and pediatric patients, bone marrow necrosis, nontraumatic fractures, and some other disorders.

[Orthopedic manifestations of the antiphospholipid syndrome].

PURPOSE: The antiphospholipid syndrome (APS) is characterized by arterial and/or venous thrombosis, and pregnancy morbidity in association with antiphospholipid antibodies. Since its classical description 22 years ago, the clinical spectrum of APS has embraced the realms of obstetrics, nephrology, cardiology, neurology, gastroenterology, angiology and now, possibly orthopaedics. This is not surprising given that this disease can affect virtually any organ system and blood vessel of any size and nature. Just as venous thrombosis may affect limbs and internal organs, arterial thrombosis has been

Spinal instability: the orthopedic approach.

The determination of spinal stability remains an important task of the spine surgeon, as treatment strategies rely heavily on this assessment. However, a clinically useful definition of spine stability remains elusive. Numerous classification systems have been proposed: White and Panjabi proposed a checklist point system to assess spinal stability for each of the cervical, thoracic, and lumbar spines. This remains one of the best recognized systems to date. Denis proposed that spinal integrity depended on the three-column system (anterior, middle, and posterior) and used it to describe thoraco

[Orthopedic interventions in patients with hemophilia].

Patients with severe hemophilia A or B and von Willebrand Syndrome have recurrent joint bleedings, which can occur spontaneously or as a result of trauma. Resorption of intraarticular blood induces an inflammation of the synovium and cartilage damage, finally leading to severe changes of the joint with complete destruction. This process is called hemophilic arthropathy. After failure of substitution and intensive conservative therapy after 3 to 6 months, a synovectomy or a synoviorthesis are indicated. In some cases the progression of hemophilic arthropathy cannot be stopped and requires total

[Thrombosis prophylaxis in orthopedic surgery].

At Trondheim University Hospital, prophylaxis against thromboembolism after orthopaedic surgery was changed from dextran 70 to low molecular weight heparin in 1992. We wanted to assess whether the frequency of thromboembolic complications has fallen after this change of procedure and to search for any indication that prophylaxis should be prolonged from two to six weeks. Our register showed 97 cases (0.76%) with thromboembolism during the first 12 weeks after 12,711 operations during the dextran 70 period; 99 cases (0.54%) after 18,368 operations during the low molecular-weight heparin period

[Lipoatrophia semicircularis--an orthopedic diagnosis?].

INTRODUCTION: Lipoatrophia semicircularis (LAS) represents an isolated fatty atrophy usually located at the lower extremity. The atrophy is circular and painless and usually resolves without therapy. The skin modifications occur within a few days or weeks. The etiology is unknown. CASE: In the following we present a case of LAS with a segmental dysfunction of the third ipsilateral lumbal root of a 43-year-old women. It was possible to provoke a referred pain into the area of atrophy by passive motion of the lumbal spine into the plane of resistant. CONCLUSION: LAS is rare and slightly known

Commonly missed orthopedic problems.

When not diagnosed early and managed appropriately, common musculoskeletal injuries may result in long-term disabling conditions. Anterior cruciate ligament tears are some of the most common knee ligament injuries. Slipped capital femoral epiphysis may present with little or no hip pain, and subtle or absent physical and radiographic findings. Femoral neck stress fractures, if left untreated, may result in avascular necrosis, refractures and pseudoarthrosis. A delay in diagnosis of scaphoid fractures may cause early wrist arthrosis if nonunion results. Ulnar collateral ligament tears are a fre

[Orthopedic therapy in rheumatic diseases].

The treatment of rheumatic disease requires an interdisciplinary approach in which the orthopedic surgeon is also involved. His contribution includes both conservative and operative procedures. Early synovectomy applied to joints and tendons is considered a preventive measure. Arthroscopic synovectomy belongs in the hands of an experienced operator. Joints that are not directly involved in weight-bearing can be treated by means of arthroplastic procedures, provided the cooperation of the patient is ensured. Arthrodesis is used preferentially to treat articular destruction of the ankle joint, t

The management of complex orthopedic injuries.

As advancements are made in the prevention of automobile fatalities, an increase in the incidence of pelvic and lower extremity injuries has occurred. These remain the leading causes of impairment and loss of years of productive life. Pelvic trauma has a high initial mortality rate when severe. However, with early resuscitation and transport, more survivors arrive in our trauma centers harboring these injuries. Owing to early stabilization and mobilization of the traumatized patient, a decrease in complications in these patients has been noted. Both the trauma surgeon and the orthopedic trauma

Orthopedic aspects of myelodysplasia in children.

A review of new developments in spina bifida shows that many cases are preventable by administration of periconceptual folic acid, reducing the relative risk by up to 86%. Currently, however, 50% of women of reproductive age have no folic acid in their usual diet. The Centers for Disease Control as recommended folic acid at 0.4 mg/d for all women anticipating pregnancy. The long-term outcomes of sacral level patients show a surprising decline in adult ambulatory status. Adult ambulatory status in spina bifida is significantly predictive (89% accurate) by two variables: motor level and sitting

[Pain and tourniquet in orthopedic surgery].

The use of a pneumatic tourniquet to provide a bloodless field in orthopedic surgery is often complicated by tourniquet pain. The mechanism of this pain remains incompletely understood, but it is probably multifactorial. Nerve compression is a common etiologic feature. The use of local anaesthetics may be considered the best choice for avoiding tourniquet pain. Superficial (skin) compression and deep components compression like blood vessels and muscles can both induce tourniquet pain. Central nervous system can also interfere. Release of tourniquet can increase the pain by post-ischaemic oede

Unusual orthopedic manifestations of neurofibromatosis.

Forty-four children with neurofibromatosis were found to have three previously underdescribed conditions: thoracic lordoscoliosis, protrusio acetabuli, and monomelia neurofibromatosis. Thirty-four patients (77%) had scoliosis; of these, nine had dystrophic scoliosis (Group I, 26%), six had dystrophic lordoscoliosis (Group II, 18%), eight had dystrophic kyphoscoliosis (Group III, 24%), and 11 had nondystrophic scoliosis (Group IV, 32%). Group III curves were severe and progressive, rendering them difficult to stabilize even with anterior and posterior spinal fusions. Group II curves were progre

[Imaging of the shoulder in orthopedic pathology].

Radiological examination of an unstable shoulder relies on straight X-ray images which are better than CT scans to detect bone lesions consecutive to anterior destabilizing accidents. The indications of CT arthrography are very limited. Radiological studies of the rotator cuff must necessarily include a standard radiological examination. Contrast arthrography is used either to find out whether or not the cuff is damaged, or when the tear is thought to be small. Arthropneumotomography is useful in the preoperative assessment of severe postero-superior tears. CT arthrography is very good to expl

The orthopedic management of Rett syndrome.

Musculoskeletal deformity sufficiently severe to require orthopedic surgery is a significant problem in Rett syndrome. Preliminary results from the study of 16 patients suggest deformity in nearly all patients. Eight patients in stage III and seven patients in stage IV showed clinical evidence of scoliosis. Radiographic studies confirmed a structural curve in nine of ten patients studied. Heel cord tightening was seen in nine of 16 patients. Hip instability was identified as an area of potential concern in the Rett patient.
