[Otosialorrhoea - a rare case of a spontaneous salivary fistula of the external auditory canal].

Spontaneous salivary fistulas are rare. Extremely rare are spontaneous salivary fistulas of the external auditory canal. We report a case of otorrhoea caused by such a fistula in connection with defects of the cartilagenous and the osseous external auditory canal (Fissures of Santorini, foramen of Huschke) and its successful surgical treatment.

Cryoneurolysis of auriculotemporal nerve for treatment of otalgia.

Otalgia in adults is often secondary to non-otologic pathology, particularly with normal otoscopic examination. Auriculotemporal neuralgia (ATN) is a rare and overlooked cause of secondary otalgia. We present a case of an Army veteran with chronic right otalgia and dizziness following blast-related mild traumatic brain injury who was initially diagnosed with vestibular migraine. Despite significant improvement in headache symptoms with migraine-directed preventive and abortive treatments, otalgia persisted. Auriculotemporal nerve block produced complete but short-lived pain relief, confirming

Fibroepithelial Polyp of the External Auditory Canal Extending to the Eustachian Tube: A Rare Clinical Entity.

Fibroepithelial polyps are benign mesodermal lesions that are an uncommon occurrence within the external auditory canal and the middle ear. The present case involves a 39-year-old male patient who has been experiencing hearing loss and intermittent ear discharge for a period of 10 years. Otoscopic examination revealed a smooth mass that completely filled the external auditory canal. A thorough analysis of the computed tomography results revealed the presence of soft-tissue density extending to the middle ear and Eustachian tube, with no evidence of bone destruction. Noncontrast magnetic resona

Middle Ear Neuroendocrine Tumor Mimicking As Chronic Otitis Media.

Neuroendocrine neoplasms (NEN) of the head and neck are a rare and diverse group of tumors. Here, we report a case of a 40-year-old man presenting with symptoms resembling chronic left otitis media, including left ear otorrhea, otalgia, and reduced hearing. Otoscopic examination revealed a whitish mass located behind the tympanic membrane. The patient underwent examination under anesthesia and left cortical mastoidectomy, and a histopathological examination of the middle ear biopsy indicated the presence of an epithelial tumor with neuroendocrine differentiation, suggestive of middle ear adeno

Clinical Techniques: When and how to do a myringotomy - a practical guide.

A myringotomy is a surgical incision made in the tympanic membrane (TM). This gives access to the middle ear for sampling, flushing and instilling topical therapy. It should be considered whenever the TM is intact and there is clinical evidence of otitis media, abnormal TMs and/or abnormal diagnostic imaging. Samples should be collected for cytological investigation and culture, and then the external ear should be cleaned and dried (if required). Myringotomies should be performed under general anaesthesia and, wherever possible, using a video otoscope; the enhanced view and instrument ports fa

Zygomatic root abscess: A rare entity not to be forgotten!

Zygomatic root abscess is a rare extracranial extratemporal complication of otitis media. To the best of our knowledge, there are only a few scattered cases of zygomatic root abscesses reported in the literature. We present an unusual case of a zygomatic root abscess in a 24 years old adult. He presented with one month duration of right zygomatic swelling. Otoscopic examination revealed superior and posterior external auditory canal wall sagging with an intact tympanic membrane. High Resolution Computed Tomography (HRCT) temporal bone revealed a rim enhancing lesion lateral to the zygomatic pr

Topical treatment of chronic suppurative otitis media.

Chronic suppurative otitis media (CSOM) is a chronic inflammation of the middle ear and mastoid cavity presenting with ear discharge or otorrhea through a non-intact tympanic membrane. CSOM is the most common cause of childhood hearing impairment in developing countries. Accurate diagnosis depends on a high index of suspicion, micro-otoscopic examination, and judicious use of imaging as required. CSOM can be classified into 3 types: tubotympanic, atticoantral, and post-tympanostomy tube insertion. Aerobes, anaerobes and fungi are all potential pathogens in CSOM. This review summarizes the resu

Primary malignant melanoma of the middle ear mucosa: a case report.

Primary involvement of the middle ear and mastoid mucosa by malignant melanoma (MM) has been rarely reported. We report an 81-year-old Caucasian woman presenting with otalgia, left-sided aural fullness, and hearing loss. Otoscopic examination revealed a purple-colored polyp that appeared to be vascular, occupy left external auditory canal (EAC). The computed tomography scanning showed a mass in soft tissue density, filling the tympanic and mastoid cavities, eroding the mastoid cortex, and invading medial and central parts of the EAC. A biopsy was performed with the diagnosis of MM. The patient

One-year audiologic monitoring of individuals exposed to the 1995 Oklahoma City bombing.

This longitudinal study evaluated subjective, behavioral, and objective auditory function in 83 explosion survivors. Subjects were evaluated quarterly for 1 year with conventional pure-tone and extended high-frequencies audiometry, otoscopic inspections, immittance and speech audiometry, and questionnaires. There was no obvious relationship between subject location and symptoms or test results. Tinnitus, distorted hearing, loudness sensitivity, and otalgia were common symptoms. On average, 76 percent of subjects had predominantly sensorineural hearing loss at one or more frequencies. Twenty-fo

Post-traumatic guitar-shaped deformity of the tympanic membrane.

We report a unique case of post-traumatic guitar-shaped deformity of the tympanic membrane in an 8-year-old boy. After a traffic accident, he exhibited bleeding from the ear, incomplete facial palsy and a conductive hearing loss on the left side. Although his symptoms gradually improved, the deformity of the tympanic membrane and external auditory canal persisted. The tympanic membrane appeared to be duplicated. Careful examination using an otoscope was required for accurate diagnosis. Without knowledge of the deformity, the physician could easily misinterpret the appearance of the tympanic me

Long-term effects of otitis media with effusion: otomicroscopic findings.

A cohort of 946 children who were screened for otitis media with effusion (OME) by serial tympanometry from the age of 2 to 4 years was followed up by otomicroscopy at 7.5 to 8 years of age. At school age, signs of active middle ear disease were present in 2.2% of ears that had been free from OME at preschool age, whereas 12.7% of ears that had suffered from persistent OME and 17.8% of ears that had been treated by a ventilation tube showed signs of active middle ear disease. Otopathologic sequelae (atrophy, tympanosclerosis, atelectasis, and attic retraction) were present in 13.7%, 39.8%, and

Brain stem evoked potential findings in children with otitis media.

Sixty-three children from 2 to 12 years of age were studied by means of brain stem evoked potentials (BEP) and otoscopic evaluation in a preliminary assessment of the sensitivity of BEP latency measures to middle ear abnormality. Wave I proved to be the most sensitive component; 81.25% of subjects with acute suppurative otitis media and 62.50% of subjects with secretory otitis media demonstrated latencies prolonged by more than 1 SD beyond the values observed in normal control subjects. Of the same subject groups, 86.21% and 93.31% demonstrated wave I latencies within normal limits upon retest

Otitis media in young Aboriginal children from remote communities in Northern and Central Australia: a cross-sectional survey.

BACKGROUND: Middle ear disease (otitis media) is common and frequently severe in Australian Aboriginal children. There have not been any recent large-scale surveys using clear definitions and a standardised middle ear assessment. The aim of the study was to determine the prevalence of middle ear disease (otitis media) in a high-risk population of young Aboriginal children from remote communities in Northern and Central Australia. METHODS: 709 Aboriginal children aged 6-30 months living in 29 communities from 4 health regions participated in the study between May and November 2001. Otitis medi

Functional evaluation of paratubal muscles using electromyography in patients with chronic unilateral tubal dysfunction.

Eustachian tube dysfunction is closely related to the development of otitis media and result from several factors including inflammation within the nasal cavity and nasopharynx, adenoid hypertrophy, cleft palate and nasopharyngeal carcinoma. To some extent, eustachian tube dysfunction may be related to weakness of the paratubal muscles, such as the tensor veli palatini and levator veli palatini muscles. The aim of the study is to find out myogenic factors in eustachian tube dysfunction using electromyography (EMG), and to evaluate the clinical feasibility of EMG. Ten patients with unilateral e

Capillary Hemangioma as an Unexpected Pathology in the External Auditory Canal.

INTRODUCTION: Hemangiomas are benign vascular lesions frequently observed in infancy and childhood. CASE REPORT: A 14-year-old boy was referred to the hospital with a left-sided ear canal mass and hearing impairment. Otoscopic examination revealed a mass that occluded the canal. The transcanal surgical excision was performed. The mass was dissected from the skin, no bony erosion was noted intraoperatively, and the skin was returned back properly. The histopathological report demonstrated a capillary hemangioma of the external auditory canal. CONCLUSIONS: Hemangiomas are relatively common in
