Single vs composite measures of pain intensity: relative sensitivity for detecting treatment effects.

Assay sensitivity remains a significant issue in pain clinical trials. One possible method for increasing assay sensitivity for detecting changes in pain intensity is to increase the reliability of pain intensity assessment by increasing the number of intensity ratings obtained, and combining these ratings into composite scores. The current study performed secondary analyses from a published clinical trial to test this possibility. The reliability and assay sensitivity pain intensity scores made up of 1 to 9 24-hour pain intensity recall ratings were compared. Although the reliability of the o

Nonneurologic hand pain versus carpal tunnel syndrome: do psychological measures differentiate?

OBJECTIVE: To determine whether psychological measures would differentiate a group of patients with physician-diagnosed nonneurologic hand pain from patients with carpal tunnel syndrome (CTS). Many patients, who also displayed symptoms of psychological distress, were referred to an electrodiagnostic clinic with a diagnosis of possible CTS; they subsequently had normal nerve conduction studies. DESIGN: Sixty patients with hand pain were referred to either of two university clinics for electrophysiologic testing, were assigned to either the CTS or nonneurologic group, and were compared on a ser

Relationship between functional evaluation measures and self-assessment in nonacute low back pain.

STUDY DESIGN: The correlations between objective biomechanical indicators of function and self-assessment scores were examined retrospectively for 91 subjects with nonacute low back pain. OBJECTIVES: To examine the correlation between self-assessment, trunk range of motion (ROM), velocity, and complex mechanical coordination patterns of the spine in nonacute low back pain. SUMMARY OF BACKGROUND DATA: In low back pain, there is often little concordance between pain, physical impairment, and disability. Use of range of motion and velocity to enhance objectivity in impairment evaluations has be

[A survey on low back pain and preventive measures among land freight transportation workers].

OBJECTIVES: Low back pain (LBP) remains a major occupational health concern in the land freight transportation industry and has yet to be effectively resolved. Contributing factors include insufficient implementation of preventive measures and misalignment with actual working conditions. In this study, we aimed to identify tasks associated with high lumbar loads, examine the current situation of LBP, and evaluate the implementation of preventive measures among workers in the land freight transportation industry through interviews and a web-based questionnaire survey. METHODS: We conducted sem

Utility of Existing Patient-Reported Outcome Measures for Identifying Non-Rhinogenic Facial Pain.

OBJECTIVE: Facial pain/pressure is often non-rhinogenic and migraine-related in etiology. However, this is frequently misdiagnosed as sinusitis, leading to inappropriate antibiotic utilization and unnecessary procedures. We assessed the utility of the 3-Item Identify Migraine (ID Migraine) and the 22-Item Sinonasal Outcomes Test (SNOT-22) in differentiating rhinogenic vs. non-rhinogenic facial pain/pressure (NRFP). METHODS: Patients presenting to the rhinology clinic with a complaint of facial pain/pressure completed the ID Migraine and SNOT-22. A diagnosis of CRS or NRFP was given based on i

Reliability and Validity Measures of the Patellofemoral Subscale KOOS-PF in Greek Patients with Patellofemoral Pain.

Background: Patellofemoral pain (PFP) is one of the most common multifactorial musculoskeletal pathologies affecting the knee joint. The prevalence of PFP in the general population ranges from 11% to 17%, with higher rates observed in specific groups such as females, runners, military personnel, and young athletes. To assess symptoms associated with PFP, the patellofemoral subscale (KOOS-PF) was developed, consisting of 11 questions that evaluate pain, stiffness, and quality of life. The KOOS-PF scale has already been validated and shown to be reliable in both its Spanish and Arabic versions.

Correlations between the Central Sensitization Inventory and measures of endogenous pain modulation in women with fibromyalgia.

BACKGROUND: The Central Sensitization Inventory (CSI) is a self-report measure to identify key symptoms of human assumed central sensitization (HACS). However, research examining how the CSI relates to experimental pain measures in a population with predominant HACS is scarce. OBJECTIVES: This study examined correlations between the CSI and experimental pain measures in 54 women with fibromyalgia. DESIGN: Cross-sectional study. METHOD: The CSI was administered along with questionnaires of illness perception (i.e., IPQ), anxiety and depression (i.e., HADS), and health-related quality of life

Preliminary development of a measure of parental behavioral responses to everyday pains in young children: the PREP.

INTRODUCTION: Everyday pains are experienced frequently by young children. Parent responses shape how young children learn about and experience pain. However, research on everyday pains in toddlers and preschoolers is scarce, and no self-report measures of parent responses to their child's pain exist for this age group. OBJECTIVES: The objective of this study was to develop a preliminary self-report measure of parent behavioral responses to everyday pains in the toddler and preschool years (the PREP) and examine its relationship with child age, sex, and parent and child distress. METHODS: It

Objective wearable measures and subjective questionnaires for predicting response to neurostimulation in people with chronic pain.

BackgroundNeurostimulation is an effective therapy for treating and management of refractory chronic pain. However, the complex nature of pain and infrequent in-clinic visits, determining subject's long-term response to the therapy remains difficult. Frequent measurement of pain in this population can help with early diagnosis, disease progression monitoring, and evaluating long-term therapeutic efficacy. This paper compares the utilization of the common subjective patient-reported outcomes with objective measures captured through a wearable device for predicting the response to neurostimulati

Autonomic measures identify stress, pain, and instability associated with retinopathy of prematurity ophthalmologic examinations.

BACKGROUND: Retinopathy of prematurity (ROP) ophthalmologic examinations cause stress and pain. Infants' stress and pain can be measured non-invasively using skin conductance (SC) and high frequency heart rate variability (HF-HRV), reflecting sympathetic-mediated sweating and parasympathetic activity, respectively. OBJECTIVES: To test the utility of SC to detect sympathetic activation during ROP examination, and the contribution of HF-HRV to predict stability post-examination. METHODS: In this prospective, single center study, we measured SC continuously pre-, during, and post-examination, a

Initial Content Validation and Roadmap for a New Patient-Reported Outcome Measure of Pain Intensity.

Measures of pain intensity (eg, numeric rating scales [NRS]) are widely used in clinical research and practice. While these measures have evidence for validity and reliability, poor standardization of instructions, and response options limits precision of pain assessment, allows for inconsistency in interpretation, and presents a challenge for comparison and aggregation of study results. Despite these pitfalls, the 0 to 10 NRS remains the most commonly used primary outcome measure in clinical trials of pain treatments and is the core measure recommended by regulatory agencies. The purpose of t

Clinical Utility of CAT Administered PROMIS Measures to Track Change for Pediatric Chronic Pain.

Patient Reported Outcomes (PROs) are utilized in clinical registries and trials, necessitating development of benchmarks to enhance interpretability. This study aimed to 1) examine if PROMIS measures administered via computer adaptive testing (CAT) were responsive to change, and 2) highlight one method of assessing clinically significant change for youth seen in a tertiary pain clinic. Clinically significant change was achieved if patients had significantly reliable pre-to-post-changes greater than Reliable Change Index (RCI) value and reported decreased symptoms by at least one severity level

Physiological Measures of Acute and Chronic Pain within Different Subject Groups: A Systematic Review.

RESULTS: The methods' heart rate variability and electroencephalogram show clear and consistent results as acute pain assessment. Magnetic resonance imaging can measure chronic pain. Ordered by invasiveness and vulnerability, a trend shows that the invasive methods are used more with less vulnerable subjects. Only instruments used for skin conductance and automatic facial recognition have a lower-than-average technological maturity. CONCLUSIONS: Some pain assessment methods show good and consistent results and have high technological maturity; however, using them as pain assessment for person

The CALI-9: A brief measure for assessing activity limitations in children and adolescents with chronic pain.

Pain-related functional limitations represent an important outcome domain to assess in children and adolescents with chronic pain. The aim of this study was to extend the empirical support of the 21-item Child Activity Limitations Interview (CALI-21), a well-validated measure of activity limitations, using a large, multisite sample and to develop a brief form of the measure with more interpretable scoring. A sample of 1616 youth and 1614 parents completed the CALI-21 at an initial appointment in 1 of 3 pain specialty clinics in the Midwest or Northwest United States, or as part of a research s
