[The effect of passive plates on the arrangement of the alveolar arch segments in unilateral total cheilognathopalatoschisis].

Maxillary alveolar arch morphology was studied from birth to palate repair at the age of 19 months in 24 children with complete unilateral cleft lip and palate (UCLP) treated with passive orthopaedic plates (treated children). 18 children with the same defect, who received no orthopaedic treatment, served as controls. The surgical procedures and the age at lip and palate repair were similar. Maxillary alveolar arch morphology was described by 15 linear and five angular measurements. No significant difference was found in total alveolar crest length neither between the cleft and the non-cleft m

Speech therapy for compensatory articulations and velopharyngeal function: a case report.

The objective of this study was to describe the process of intensive speech therapy for a 6-year-old child using compensatory articulations while presenting with velopharyngeal insufficiency (VPI) and a history of cleft lip and palate. The correction of VPI was temporarily done with a pharyngeal obturator since the child presented with very little movement of the pharyngeal walls during speech, compromising the outcome of a possible pharyngeal flap procedure (pharyngoplasty). The program of intensive speech therapy involved 3 phases, each for duration of 2 weeks incorporating 2 daily sessions

A Review of and Algorithmic Approach to Soft Palate Reconstruction.

IMPORTANCE: The soft palate contributes to deglutition, articulation, and respiration. Current reconstructive techniques focus on restoration of both form and function. The unique challenges of soft palate reconstruction include maintenance of complex upper aerodigestive tract function, with minimal local or donor site morbidity. OBJECTIVE: To review the literature on soft palate reconstruction and present an algorithm on how to approach soft palate defects based on this review. EVIDENCE REVIEW: A review of the literature for articles reporting studies on and that described concepts related

Paediatric intraoral mucoepidermoid carcinoma mimicking a bone lesion.

BACKGROUND: Mucoepidermoid carcinoma of the salivary glands occurs most frequently in adults during the fifth and sixth decades of life. Although uncommon, it is the main malignant salivary gland tumour in children, particularly adolescents. CASE REPORT: A 14-year-old girl presented with a mass in her palate that had had a duration of one year. On panoramic X-ray, a well-delimited radiolucent area was observed on the left maxillary sinus region. The initial clinical diagnostic hypothesis was that this was a central giant cell granuloma. An incisional biopsy was performed and the final microsc

Speech results of the Zürich approach in the treatment of unilateral cleft lip and palate.

The purpose of this study was to describe perceptually the speech articulation, voice quality, and velopharyngeal competency of subjects with complete unilateral cleft lip and palate treated by the Zürich approach. The mean age of the 37 subjects was 10.5 years. Although only one subject had had secondary palatal management, no subject was rated as exhibiting a severe articulation or nasality problem. Subjects were rated as exhibiting adequate to marginal velopharyngeal competency 94.5 percent of the time, and the incidence of compensatory articulation errors was low. In comparison with other

Pharyngeal obturator using a combined conventional and digital approach: A clinical report.

The prosthodontic management of a 62-year-old edentulous woman with a velopharyngeal defect following maxillectomy and radiation therapy is described. She presented with concerns about obturator stability, hypernasality, and fluid leakage from her pharyngeal obturator prosthesis. A new pharyngeal obturator and mandibular paralleling prosthesis were fabricated using a digital workflow by incorporating computer-aided design and computer-aided manufacturing (CAD-CAM) technology and conventional processing techniques. This approach reduced clinical visits while ensuring optimal fit, retention, and

Quality of life in patients with cancer-related Brown IIb maxillary defect: A comparison between conventional obturation rehabilitation and submental flap reconstruction.

OBJECTIVES: The purpose of this retrospective study was to compare the differences in quality of life (QOL) outcomes between the conventional obturator prostheses (COP) and the pedicled submental artery island flap (SAIF) in the reconstruction of Brown IIb maxillary defects. MATERIALS AND METHODS: The QOL of 116 eligible patients who had a lapse ≥ 12 months after the cancer-related maxilla ablation was evaluated by the University of Washington quality of life scale (UW-QOL), Performance Status Scale for Head and Neck (PSS-HN), and Obturator Functioning Scale (OFS). RESULTS: Patients in the S

Nutritional assessment in a maxillectomy patient from the preoperative period to definitive obturator insertion: A case report.

PATIENT: A 69-year-old Japanese male with squamous cell carcinoma of the right maxilla (T4M0N0) was referred to our department for a preoperative examination. An immediate surgical obturator was fabricated before surgery. He underwent surgical treatment, which included right subtotal maxillectomy and reconstruction with a split skin graft. One week postoperatively, the immediate surgical obturator was modified to expand the nasal cavity for obturator prosthesis. Oral intake was started 12 days postoperatively with an immediate surgical obturator in situ. The definitive obturator was fabricated

Aerodynamic assessment of prosthetic speech aids.

The primary function of a speech aid prosthesis is to provide adequate palatopharyngeal function by preventing nasal emission and hypernasality during oral speech production and permitting sufficient nasal air escape during nasal consonant production. The adequacy of speech aids is often judged subjectively by speech-language pathologists and prosthodontists. However, when oral and laryngeal function are also affected, additional information may be needed for accurate assessment of palatopharyngeal function and optimal prosthetic management. In these instances, aerodynamic measurements can pro

Endodontic Management of Dilacerated Maxillary Central Incisor fused to a Supernumerary Tooth using Cone Beam Computed Tomography: An Unusual Clinical Presentation.

AIM: The purpose of this report is to present a case of endodontic management of a dilacerated maxillary central incisor fused to supernumerary tooth unusually appearing as a labial tubercle using cone beam computed tomography (CBCT) as a diagnostic aid. BACKGROUND: Anterior teeth are commonly encountered with aberrant anatomical variations in the crown, number of roots, and root canals. Fusion is an infrequent developmental abnormality in shape of the tooth caused by the union of two adjacent tooth germs. A complete knowledge of such complex anatomies is mandatory for a successful root canal

Simple reconstruction with titanium mesh and radial forearm flap after globe-sparing total maxillectomy: a 5-year follow-up study.

BACKGROUND: Reconstruction of eye globe-sparing total maxillectomy defects is one of the major challenges to reconstructive surgeons. In 1994, the authors developed an uncomplicated and easy reconstructive method, where a titanium mesh is applied for the support of orbital contents, a radial forearm free flap for covering the mesh and the cheek lining, and an obturator prosthesis for palatal and dental rehabilitation. METHODS: Five patients who underwent primary reconstruction with the authors' method after globe-sparing maxillectomy with loss of the orbital floor from 1994 to 1999 and who we

Algorithm for the Treatment of Oronasal Communications: Introducing a Novel Classification.

INTRODUCTION: Breach of the palate leading to a pathological communication between the oral cavity and the nose is known as oronasal communication/fistula (ONC). Various methods for the closure of communications have been reported in the literature, such as obturators, local flaps, and microvascular surgery. Due to the lack of a classification system for ONC, the purpose of this study was to trace an operative algorithm in a group of patients affected by ONC who underwent surgical reconstruction of the oronasal communication. METHODS: A retrospective study was conducted for all patients affec

Presurgical management of unilateral cleft lip and palate in a neonate: a clinical report.

A cleft lip and palate consists of fissures of the upper lip and/or palate, and is the most commonly seen orofacial anomaly that involves the middle third of the face. Early treatment of patients with a cleft lip and palate is important because of esthetic, functional, and psychological concerns. Nasoalveolar molding provides excellent results when started immediately after birth. This clinical report describes the presurgical management of an infant with a complete unilateral cleft of the soft palate, hard palate, alveolar ridge, and lip.
