Keeping healthcare afloat: a protocol for a 5-year multi-sited interdisciplinary research project into preparedness of healthcare for floods in the Netherlands.

INTRODUCTION: The 2021 European floods in Germany, Belgium, and the Netherlands significantly impacted healthcare. With climate change increasing flood risks, healthcare preparedness is essential. Floods affect healthcare directly and indirectly by disrupting patient access, damaging infrastructure and impeding care continuity. Our interdisciplinary research in the Netherlands systematically assesses flood impacts on healthcare, optimises disaster preparedness, patient logistics, and continuity and explores crisis governance, incorporating lessons from coronavirus disease-2019 (COVID-19). The importance of getting the ethics right in a pandemic treaty.

The COVID-19 pandemic revealed numerous weaknesses in pandemic preparedness and response, including underfunding, inadequate surveillance, and inequitable distribution of countermeasures. To overcome these weaknesses for future pandemics, WHO released a zero draft of a pandemic treaty in February, 2023, and subsequently a revised bureau's text in May, 2023. COVID-19 made clear that pandemic prevention, preparedness, and response reflect choices and value judgements. These decisions are therefore not a purely scientific or technical exercise, but are fundamentally grounded in ethics. The latest

Confronting the known unknown: historical lessons and future strategies for Disease X.

The 2026 outbreak of Nipah virus in West Bengal, India, a known WHO priority pathogen, serves as a timely reminder of the constant threat from high-consequence emerging diseases and underscores the critical relevance of Disease X-a future pandemic caused by an unknown pathogen. The profound global health and economic impact of COVID-19 has made the potential devastation of Disease X tangible. However, despite the insights gained from COVID-19, significant gaps remain in translating this awareness into an effective, coordinated, and pre-emptive preparedness strategy. This review advances an int

Avian influenza H5N1: A warning signal for the next influenza pandemic.

Highly pathogenic avian influenza A (H5N1) remains one of the most significant zoonotic threats of the 21st century due to its wide host range, high case-fatality rates in humans, and ability to cause devastating losses in poultry and wildlife. Since its first detection in humans in 1997, H5N1 has diversified into multiple genetic clades, with clade 2.3.4.4b now driving a true global panzootic affecting birds, mammals, and livestock across continents. This review synthesizes current knowledge on the virology, genomic evolution, transmission dynamics, and ecological spread of H5N1, with particu

From SARS to Avian Influenza Preparedness in Hong Kong.

The first human H5N1 case was diagnosed in Hong Kong in 1997. Since then, experience in effective preparedness strategies that target novel influenza viruses has expanded. Here, we report on avian influenza preparedness in public hospitals in Hong Kong to illustrate policies and practices associated with control of emerging infectious diseases. The Hong Kong government's risk-based preparedness plan for influenza pandemics includes 3 response levels for command, control, and coordination frameworks for territory-wide responses. The tiered levels of alert, serious, and emergency response enable

Co-learning in crisis: A comparative analysis of digital preparedness during COVID-19 in Taiwan and the United Kingdom.

This article provides a comparative analysis of digital interventions implemented in Taiwan and the UK during the COVID-19 pandemic to examine how national contexts shaped the rollout and reception of these technologies. Our analysis challenges the simplistic East/West divide often invoked in pandemic response analyses, which arose during and in the wake of the pandemic, arguing that successes or failures of digital health interventions must exceed assumed cultural traits in East or West, and consider sociohistorical factors, legal frameworks, and specific political contexts. By advancing inte

"By the Skin of Our Teeth": U.S. Hospital, Regional, and State Experiences of Scarcity During the COVID-19 Pandemic.

BACKGROUND: The COVID-19 pandemic presented unprecedented challenges to hospital system and critical care resources, leading to significant changes to operations and patient care. There are limited national data on these changes and instances of unsanctioned deviations from patient care, yet understanding the COVID response is key to future preparedness efforts. We sought to understand how hospitals and states navigated scarcity during COVID-19, particularly in the absence of a declaration of crisis standards of care. METHODS: Between February 2022 and September 2022 we conducted 34 interview

Africa's Critical Role in Shaping and Implementing the Pandemic Agreement's PABS Annex in an Era of Fragmentation.

The adoption of the World Health Organization Pandemic Agreement in May 2025 marked a political commitment to stronger global pandemic preparedness. Its credibility, however, depends on the successful negotiation and implementation of the Pathogen Access and Benefit Sharing annex under Article 12, to be finalized by April 2026. The COVID 19 pandemic exposed the failures of voluntary global health mechanisms, as African countries rapidly shared pathogen samples and sequence data yet faced severe delays in access to vaccines, diagnostics, and therapeutics. The PABS annex therefore represents the

Ebola in the rear-view mirror: how epidemic pasts shaped Sierra Leone's preparedness and response to Covid-19.

This paper examines how past experience and legacies of epidemics shaped Sierra Leone's response to COVID-19 and how these influences evolved over time. COVID-19 unfolded in the wake of the West African Ebola epidemic (2013-2016), a crisis which was unprecedented in scale. Despite differing markedly in both transmission patterns and clinical outcomes, the Sierra Leonean government repeatedly invoked Ebola when responding to COVID-19, framing the new outbreak through the lens of the old. Drawing on ethnographic fieldwork and interviews with policymakers, response personnel, health workers, and

Efficient vaccine against pandemic influenza: combining DNA vaccination and targeted delivery to MHC class II molecules.

There are two major limitations to vaccine preparedness in the event of devastating influenza pandemics: the time needed to generate a vaccine and rapid generation of sufficient amounts. DNA vaccination could represent a solution to these problems, but efficacy needs to be enhanced. In a separate line of research, it has been established that targeting of vaccine molecules to antigen-presenting cells enhances immune responses. We have combined the two principles by constructing DNA vaccines that encode bivalent fusion proteins; these target hemagglutinin to MHC class II molecules on antigen-pr
