Paraplegia occurred in an adolescent girl with osteogenesis imperfecta after chiropractic manipulation. The child had been able to walk freely out of doors. Complete motor paralysis with sensory sparing resulted due to anterior compression of the cord by spondyloptotic cervical vertebrae. Reconstructed computerised tomography was very helpful in demonstrating the abnormality. Anterior and then posterior decompression relieved the tethered spinal cord and were supplemented with bone grafting. Early diagnosis and surgical treatment will prevent similar neurological accidents.

The elucidation of possible involvement of the vertebral canal in familial spastic paraplegia is attempted. The sagittal and the transverse diameters of the cervical, thoracic and lumbar vertebral canal were estimated in 14 patients and 100 controls. The results showed that the vertebral canal of the patients is considerably smaller in its whole length and not only in the thoracic region. This finding is discussed in relation to the development of both the vertebral canal and spinal cord.

Aneurysmal bone cyst, a benign lesion, is difficult to diagnose radiologically, particularly in the spine, but has a characteristic microscopic appearance. Treatment of spinal lesions consists of curettage, with or without bone grafting, together with irradiation at low doses (1000 to 2000 rads). The author reports a case of aneurysmal bone cyst of the neural arch associated with neurologic complication, which illustrates some typical features of such cysts of the spine. The condition should always be considered in patients with lytic lesions of the spine, and prompt surgical intervention is r

A cerebrospinal fluid (CSF) leak developed in a 14-year-old girl and a 12-year-old boy following a diagnostic lumbar puncture. Two days and sixteen years later, respectively, paraplegia developed due to a functional disorder. Imaging revealed an extensive extradural CSF collection in both patients and digital subtraction myelography was required to pinpoint the exact site of a ventral dural puncture hole where the lumbar spinal needle had gone "through and through" the dural sac. The CSF leak was complicated by cortical vein thrombosis in one patient. Both patients underwent uneventful surgica…

A 48-year-old female with long-standing type 2 diabetes mellitus presented with acute onset of bilateral lower limb weakness. She had been previously well and denied any constitutional symptoms. Physical examinations revealed generalized lower limb weakness with bilateral lower limb hypotonia, power of 0 over 5, reduced deep tendon reflexes, and loss of peripheral sensations up to the level of T10. Upper limb functions were normal. Rectal examination showed a lax anal tone and reduced anal grip. Blood investigations showed elevated serum alkaline phosphatase, corrected serum calcium, and parat
