Pulsatile parathyroid hormone secretion in health and disease.

In humans plasma parathyroid hormone (PTH) fluctuates episodically at a frequency of 6-7 bursts per hour. Approximately 30% of circulating PTH is attributable to pulsatile secretion and 70% to tonic secretion. PTH release is tightly controlled by Ca2+. Acute hypocalcaemia elicits a biphasic wave of PTH release, with an initial selective amplification and acceleration of the pulsatile component followed by proportionate stimulation of pulsatile and tonic secretion. Acute hypercalcaemia submaximally suppresses the frequency and mass of PTH bursts as well as tonic PTH release. Patients with prima…

Serum parathyroid hormone-related protein levels during lactation.

We measured parathyroid hormone-related protein (PTHrP) in the serum of 15 healthy women within three days of beginning breast-feeding and within one hour of completing nursing. A sensitive immunoradiometric assay that measures N-terminal PTHrP containing at least the first 74 amino acids was used. We found normal PTHrP values in all patients. Values one day before and two days after delivery in a hypoparathyroid woman who chose not to breast-feed were also normal. Since N-terminal PTHrP fragments of 1-36 amino acids or more are biologically active, we believe additional studies using sensitiv…

[Parathyroid hormone related peptide (PTHrP) and bone metabolism].

The discovery of PTHrP was the result of research on the mechanisms by which some cancers cause hypercalcemia (humoral hypercalcemia of malignancy) without necessarily metastasizing to bone. PTHrP is also present in various normal adult and fetal tissues. Its concentration is normally very low (picomolar) in blood, but it is more abundant in milk (nanomolar concentration). PTHrP seems able to exert autocrine/paracrine as well as endocrine effects on bone metabolism. A major role for PTHrP in regulation of fetal bone metabolism has been demonstrated in mice. Homologous recombination has been us

Parathyroid hormone secretion after operation for primary hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism (pHPT) is associated with a defective regulation of the secretion of parathyroid hormone (PTH). Thus in pHPT, higher than normal calcium concentrations are required to inhibit PTH release. However, it is not known if this defective regulation is normalized by removal of the parathyroid adenoma (i.e., whether the regulation of PTH secretion is normal in the remaining glands). In this study we therefore investigated the PTH secretion in patients operated on for parathyroid adenoma 1 year after operation. METHODS: Na2 ethylenediamine tetraacetic acid and C…

Increasing parathyroid hormone concentrations in untreated primary hyperparathyroidism.

Twenty-four patients with mild to moderate primary hyperparathyroidism were followed for an average of 2.45 years with serial determinations of serum ionized calcium and intact parathyroid hormone (PTH). For the entire group serum ionized calcium remained stable, whereas serum PTH increased significantly. Eleven patients (group 1) demonstrated a significant increase in PTH with time. The remaining 13 patients formed group 2. Comparison of the changes (%) in each subgroup showed a small but significant increase in serum ionized calcium of 2.6% with time in group 1, while serum PTH increased by …

Parathyroid hormone-related protein: a novel gene product.

Many factors, such as interleukin 1, transforming growth factor alpha, tumour necrosis factor alpha and beta, and prostaglandins, have been implicated in the pathogenesis of the humoral hypercalcaemia of malignancy (Mundy and Martin, 1982; Martin and Mundy, 1987; Mundy et al, 1984). Much interest in the past has also centred upon the likelihood of ectopic secretion of PTH in this condition. We have purified a protein (PTHrP) implicated in HHM from a human lung cancer cell line (BEN). Full-length cDNA clones have been isolated and found to encode a pre-pro-peptide of 36 amino acids and a mature

Parathyrin (parathyroid hormone): metabolism and methods for assay.

Parathyrin (parathyroid hormone) radioimmunoassay is a commonly ordered endocrine assay. I discuss here of the metabolism of the hormone and the effect information on this metabolism has had on the development of radioimmunoassays for parathyrin. Although radioimmunoassay is the most widely accepted technique for the routine assay for parathyrin, "high-performance" liquid chromatography, cytochemical bioassay, and homologous bioassay have also been developed for this analyte. Accordingly, I briefly review the clinical utility of these assays in terms of their ability to assess the functional a…

Parathyroid gland morphology in nonparathyroid hormone-mediated hypercalcemia.

Occasionally, patients with other causes of hypercalcemia undergo parathyroidectomy for suspected primary hyperparathyroidism. We studied the parathyroid glands from 12 persons with nonparathyroid hypercalcemia. Glands from these individuals were similar to normal glands in size and fat:parenchymal ratio, showing neither atrophy nor hyperplasia. A distinctive histologic feature, also seen in hyperparathyroid hypercalcemia, is the presence of pale, vacuolated cells arranged in a trabecular pattern. Recognition of normal parathyroid parenchymal weight and the nonspecific nature of these vacuolat

Parathyroid hormone infusion test in children and adolescents.

10 children with short stature were infused with parathyroid hormone (PTH). The results were compared to those obtained from a study of 28 normal adult volunteers. As expected, the children had higher basal serum phosphorus levels than the adults. PTH administration increased urinary excretion of cyclic AMP, sodium, potassium, bicarbonate, phosphate clearance, and reabsorption of calcium. The responses of the children were indistinguishable from those of the adults. Infusion of PTH to 3 children with hypoparathyroidism produced exaggerated cyclic AMP, phosphate, calcium, potassium, and bicarbo…

Homologous radioimmunoassay for human mid-regional parathyroid hormone.

A radioimmunoassay, selective for the clinically important mid region of human parathyroid hormone (hPTH), is reported. The synthetic 44-68 amino acid sequence (h44-68PTH) was used as both the standard and tracer material. This eliminated many of the undesirable characteristics associated with PTH assays the employ hormone of biological origin. These components allowed the detection of mid regional fragments present in both intact and fragmented hPTH, with a working range between 50 and 2500 pg/ml h44-68PTH. There was no interference from serum proteins and no significant cross reactivity to a…

Multiple sampling for parathyroid hormone by subclavian approach.

Selective venous catheterization with parathyroid hormone assay is a valuable procedure in the diagnosis and treatment of parathyroid tumors. The multiple sampling via the femoral vein is sometimes frustratingly uncertain and time consuming. The technique herein described is simpler, quicker, and less hazardous. It is accomplished via the subclavian approach, a relatively easier anatomic way, and utilizes a short single-curve catheter that gives a high degree of torsion control.

Hypo-hyperparathyroidism: evidence for a defective parathyroid hormone.

Biochemical evidence for hypoparathyroidism and roentgenographic evidence for hyperparathyroidism were present in a 7-year-old girl with seizures and tetany. She was hypocalcemic (4.7 mg/dl), hyperphosphatemic (11 mg/dl), and normomagnesemic, with elevated parathyroid hormone level (2,603 pg/dl and 3,693 pg/dl in immunoassays utilizing two different antisera). Somatic features of pseudohypoparathyroidism were absent. Increased serum alkaline phosphatase activity (335 IU/liter) with evidence of subperiosteal bone resorption suggested parathyroid hormone activity on bone. Intramuscular administr…

Simultaneous ectopic production of parathyroid hormone and calcitonin.

Two patients with cancer were evaluated in whom there was evidence for the simultaneous ectopic production of parathyroid hormone (PTH) and calcitonin (CT). One patient had a gastric carcinoid and the other had a pancreatic islet cell carcinoma. Abnormal concentrations of parathyroid hormone and calcitonin were demonstrated by radioimmunoassay in the peripheral blood of each patient and in the gastric tumor. In the pancreatic tumor, immunohistologic studies also demonstrated the presence of CT and PTH and suggested that each hormone was produced by different cells of the tumor. Plasma concentr…

Impaired release of parathyroid hormone in magnesium deficiency.

Parathyroid hormone release and end-organ responsiveness to parathyroid extract (PTE) were evaluated in a 25-year-old woman with magnesium deficiency associated with hypocalcemia and inappropriately low levels of serum immunoreactive parathyroid hormone (iPTH). End-organ responsiveness to PTE was demonstrated by increases in serum calcium and in urinary phosphorus, cyclic AMP, and hydroxyproline. When the serum calcium was increased from a baseline of 6.9 mg/100 ml to levels of 8.0 mg/100 ml and higher by calcium infusion, the serum iPTH decreased from the low normal range to below the limits …

Novel insights into parathyroid hormone: report of The Parathyroid Day in Chronic Kidney Disease.

Chronic kidney disease (CKD) is often associated with a mineral and bone disorder globally described as CKD-Mineral and Bone Disease (MBD), including renal osteodystrophy, the latter ranging from high bone turnover, as in case of secondary hyperparathyroidism (SHPT), to low bone turnover. The present article summarizes the important subjects that were covered during 'The Parathyroid Day in Chronic Kidney Disease' CME course organized in Paris in September 2017. It includes the latest insights on parathyroid gland growth, parathyroid hormone (PTH) synthesis, secretion and regulation by the calc…

Calcium-sensing receptor expression and parathyroid hormone secretion in hyperplastic parathyroid glands from humans.

In uremic patients, severe parathyroid hyperplasia is associated with reduced parathyroid calcium-sensing receptor (CaR) expression. Thus, in these patients, a high serum Ca concentration may be required to inhibit parathyroid hormone (PTH) secretion. This study compares the magnitude of reduction in CaR expression and the degree of the abnormality in Ca-regulated PTH release in vitro. A total of 50 glands from 23 hemodialysis patients with refractory hyperparathyroidism were studied. Tissue slices were incubated in vitro to evaluate (1) the PTH secretory output in a normal Ca concentration (1…
