Blood donor screening for parvovirus B19 in Germany and Austria.

BACKGROUND: Although the main transmission pathway of parvovirus B19 (B19) is typically via the respiratory route, several transfusion-transmitted infections have been reported. To increase blood safety, all blood donations to our blood donor service have been screened by a B19 minipool real-time nucleic acid testing (NAT) since April 2000. Additional customers have been screened since the summer of 2003. STUDY DESIGN AND METHODS: In total, 2.8 million donations from Germany and Austria were screened for B19 by real-time minipool NAT. A subgroup of 50 B19 DNA-positive donors was screened for …

Concurrent infection of a cat with cowpox virus and feline parvovirus.

Concurrent infection with cowpox and feline parvovirus was diagnosed in a 5-month-old male European Short Hair cat. Microscopical examination of the facial skin, ears and foot pads revealed multifocal to coalescing, ulcerative to necrotizing dermatitis and panniculitis with ballooning epidermal degeneration and eosinophilic cytoplasmic inclusion bodies. Immunohistochemistry, polymerase chain reaction testing and virus isolation confirmed infection with a strain of cowpox virus similar to that isolated from a cat in Germany 5 years previously. Lymphoid tissues were depleted and there was catarr…

Occurrence of canine parvovirus type 2c in the United States.

Canine parvovirus (CPV) type 2 (CPV-2) emerged around 1978 as a major pathogen of dogs worldwide. In the mid-1980s, the original CPV-2 had evolved and was completely replaced by 2 variants, CPV-2a and CPV-2b. In 2000, a new variant of CPV (named CPV-2c) was detected in Italy and now cocirculates with types 2a and 2b in that country. The CPV-2c has also been reported from single outbreaks in Vietnam and Spain. This study was conducted to determine if CPV-2c occurs in the United States. Thirty-three fecal samples were collected from dogs in 16 states between April 2006 and April 2007 and were te…

Development of an improved method of detection of infectious parvovirus B19.

BACKGROUND: Parvovirus B19, the only known pathogenic human parvovirus is the aetiologic agent of erythema infectiosum, transient aplastic crisis, pure red cell aplasia, and hydrops fetalis. Transmission is either by respiratory secretions or, as it can be present at high titre in plasma, by blood and blood products. B19 is only cultured with difficulty in vitro, and there is no readily available assay for detecting B19 infectivity or neutralizing antibodies. OBJECTIVES: In this study, we evaluated different methods to detect viral infection for the purpose of developing automated methods for…

Focal parvovirus B19 myocarditis in a patient with Brugada syndrome.

The Brugada syndrome is characterized by a distinct ECG pattern consisting of ST segment elevation in the right precordial leads and right b … ation, in-situ hybridization, and PCR revealed evidence of a locally restricted inflammation due to parvovirus B19. Brugada syndrome is regarded as a primary electrical disease due to dysfunction of distinctive ion channels, but focal myocarditis may serve as a trigger for ventricular arrhythmias in this patient. Further morphologic studies will be helpful t …

Evidence for persistence of parvovirus B19 DNA in livers of adults.

Recent studies have suggested a pathogenic role of human parvovirus B19 (B19) in the development of acute fulminant liver failure in children. The hypothesis was based on the detection of B19 DNA in 8 of 10 explanted livers of children requiring liver transplantation. In the present study, explanted livers from 43 adults selected at random undergoing orthotopic liver transplantation for various reasons were examined. Pre-transplant sera were available from 40 patients of whom 35 (88%) were anti-B19 IgG-seropositive. All but one serum were negative for anti-B19 IgM antibody. By polymerase chain…

Human parvovirus B19 as a causative agent for rheumatoid arthritis.

Human parvovirus B19 (B19) DNA was detected in the synovial tissues in 30 of 39 patients with rheumatoid arthritis (RA), and infrequently in those with osteoarthritis and traumatic joints. On the other hand, the expression of the B19 antigen VP-1 was specific (27/27) in RA synovium with active synovial lesions, but not in osteoarthritis and controls. The target cells of B19 were macrophages, follicular dendritic cells, T cells, and B cells, but not synovial lining cells in the synovium. B19-negative bone marrow cells, tonsil cells, and macrophage cell line U-937 cells became positive for the e…

The prevalence of antibody to parvovirus B19 in hemophiliacs and in the general population.

The prevalence of antibodies to parvovirus B19 (B19) was measured in the sera of 566 hemophiliacs and 524 individuals of the general population by immunofluorescence assays, using antigen expressed by the baculovirus system. In the general population, anti-B19 IgG seroprevalence was found to continuously decline from 64 percent at birth to 0 percent in the age of 9-11 months and thereupon to increase to 61 percent in the age of 12 years. In younger adults and older people, IgG seroprevalence only slowly increased with age, reaching 77 percent in people aged 60 and above. In contrast, in hemoph…

Modulation of lethal and persistent rat parvovirus infection by antibody.

Two day-old athymic (rnu/rnu) and euthymic (rnu/+) rat pups nursing immune or non-immune dams were inoculated oronasally with the Yale strain of rat virus (RV-Y). All athymic and euthymic pups (57/57) from immune dams remained clinically normal, whereas 51 of 66 athymic and euthymic pups from non-immune dams died within 30 days. Infectious RV was detected by explant culture in 12 of 15 surviving pups of both genotypes from non-immune dams 30 days after inoculation, but in none of the 57 surviving pups from immune dams. RV-Y DNA was detected by Southern blotting in kidneys of surviving athymic

Cordocentesis for the diagnosis and treatment of human fetal parvovirus infection.

Human parvovirus B-19 infection was diagnosed by DNA hybridization in blood obtained by cordocentesis from two hydropic fetuses at 22 and 26 weeks' gestation; B-19-specific immunoglobulin M (IgM) in fetal blood was negative in both cases. Hematologic studies demonstrated severe anemia, which was treated by intravascular fetal blood transfusions. The hydrops resolved and healthy infants were delivered at term. The pathophysiology of hydrops in fetal parvovirus infection is discussed.

Embryonic malformations in a case of intrauterine parvovirus B19 infection.

An incomplete embryo of 9 weeks development from a woman infected by human parvovirus B19 during early pregnancy was histologically examined. B19-DNA was detected in both embryonic and placental tissue by dot-blot hybridization. Focal vascular endothelial damage was found throughout the entire embryo and placenta together with mononuclear infiltrations around the vessels. In the placenta these mononuclear cells belonged for the greater part to the cytotoxic and/or suppressor T-cell group. One eye showed lens abnormalities and retinal folds. The other eye was microphthalmic and aphakic and show…

Detection of parvovirus in a patient with "reactive arthritis" by in situ hybridization.

We used in situ hybridization to search for the presence of viruses in synovial fluid cell preparations obtained from patients with various forms of knee arthropathies. One patient, presenting with acute reactive arthritis, was found to replicate parvovirus DNA in cells from synovial fluid whereas six other patients with various other forms of arthritis were negative for parvovirus infection. Five patients with osteoarthritis, constituting a control group in which an infectious etiology would not be expected, were consistently negative. In addition, no hybridization was found when synovial cel…

Pathogenicity of a skin isolate of porcine parvovirus in swine fetuses.

The pathogenic properties of a skin isolate of porcine parvovirus (PPV), designated Kresse isolate, were compared with NADL-8 isolate, a prototype isolate of PPV, by in utero inoculation of mid-term and late-term gestation swine fetuses. Fetuses from pregnant sows of mid-gestation were inoculated with either NADL-8 or Kresse virus. Both isolates were highly pathogenic to mid-gestation fetuses. In contrast, dramatic differences in pathogenicity between these 2 isolates were observed in fetuses inoculated late in gestation. Such fetuses from each of 4 sows were inoculated with NADL-8 or Kresse v…

Experimental in utero infection of fetal pigs with a porcine parvovirus.

In utero infection of fetuses of six specific-pathogen-free large white sows at 35, 48, 55, 72, 99, and 105 days was studied. The fetuses were infected by direct inoculation of porcine parvovirus into the amniotic sac. The inoculation consisted of 0.25 ml of tissue culture fluid containing 10(5.5) mean tissue culture infective doses per ml of porcine parvovirus strain G10/1. Fetuses of one uterus horn were infected, whereas fetuses in the opposite horn were given 0.25 ml of noninfected cell culture material. No clinical signs of infection were observed; however, all sows developed antibodies 7…

Canine parvovirus in Africa: Explaining persistent disease despite vaccination.

Canine parvovirus (CPV) remains one of the most important causes of infectious gastroenteritis and mortality in puppies worldwide despite the widespread availability of effective vaccines. Although vaccination has substantially reduced disease incidence since the introduction of modified-live vaccines, CPV continues to circulate globally, and cases of parvoviral enteritis are still reported in both vaccinated and unvaccinated dogs. These observations have generated ongoing debate regarding the relative contribution of viral, host, and management factors to vaccination failure. This narrative r…

Parvovirus B19 Infection Presenting as Acute Anemia and Polyarthropathy.

Human parvovirus B19 is a common viral pathogen in humans with increasing prevalence in the United States. While it is classically taught as an important cause of transient aplastic crisis in patients with sickle cell disease, it is usually a mild, self-limited illness in healthy hosts. We report a 46-year-old woman who presented with normocytic anemia, rash, and arthropathy after a flu-like syndrome and was found to have acute parvovirus B19 infection. She improved with supportive care. This case highlights the varied manifestations of parvovirus B19 infection and the importance of recognizin…
