PURPOSE: Patellar dislocation usually occurs to the lateral side, leading to ruptures of the medial patellofemoral ligament (MPFL) in about 90 % of all cases. Reliable prognostic factors for the stability of the patellofemoral joint after MPFL surgery and satisfaction of the patient have not been established as yet.
METHODS: This multicentric study retrospectively included 40 patients with a mean age of 22.4 ± 8.1 years (range 9-48) from 5 German Trauma Departments with first-time traumatic patellar dislocation and operative treatment. Surgery was limited to soft tissue repairs, and a preoper

We describe a gene (PT-12) that is expressed in the patellar tendon and not in the anterior cruciate ligament. We used a recently developed polymerase chain reaction-based subtractive cDNA analysis to discover genes that are overexpressed in the patellar tendon but not expressed in the anterior cruciate ligament; the long-term objective was to find genes that are central to the self-repair of the patellar tendon, in contrast with the inability of the anterior cruciate ligament to launch a repair response following injury. PT-12 is a homologue of human S2 or mouse LLRep3 ribosomal genes, which

Ninety-one patients were assessed 5-9 years after an anterior cruciate ligament reconstruction (bone patella-tendon bone autograft). Forty-eight patients had been treated within 6 weeks of the injury (Group I) and 43 patients more than 3 months after the injury (Group II). 73 patients had either a normal or nearly normal final outcome. The mean Lysholm score was 82 and the mean Marshall score was 42. Eighty nine patients had normal or nearly normal stability in the operated knee when compared to the contralateral joint. In none of these results was there any significant difference between the

BACKGROUND: The influence of greater femoral anteversion on outcomes after medial patellofemoral ligament (MPFL) reconstruction is increasingly recognized, leading to the more frequent use of combined derotational femoral osteotomy. However, the indications for this additional invasive procedure remain poorly defined, particularly in patients without severe patellar maltracking.
PURPOSE: To evaluate the clinical outcomes of isolated MPFL reconstruction in patients with recurrent patellar dislocations (RPDs), increased femoral anteversion (>30°), and an absent or mild J-sign.

OBJECTIVE: To assess the biomechanical properties and failure mode of 3 different repair methods in simulated midsubstance canine patella ligament rupture on cadavers.
METHODS: The sample was paired hindlimbs of 9 adult mixed-breed dogs. The study design was an experimental study on cadavers. Patellar ligaments were transected and sutured using a Bunnell and simple interrupted epitendinous pattern. They were then randomly reinforced with 1 of 3 techniques: a circumpatellar suture loop anchored distally to the tibial tuberosity (group 1), the same loop combined with a fascia lata strip (group …

PURPOSE: To evaluate different bone-patellar tendon-bone (BPTB) plug suture configurations for pull through strength, stiffness, and elongation at failure in a biomechanical model of suspensory fixation.
METHODS: Forty nonpaired, fresh-frozen human cadaveric BPTB allografts with an average age of 65.6 years were tested. Tensile testing was performed with the use of a custom-designed fixture mounted in a dynamic tensile testing machine. A preload of 90 N was applied to the graft and held for 5 minutes. Following this, a tensile load-to-failure test was performed. The ultimate failure load, elo

PURPOSE: Patellar maltracking caused by a rupture of the medial patellofemoral ligament (MPFL) can be improved by MPFL reconstruction (MPFL-R) with a tendon graft. Nonresorbable suture tape (FiberTape®, FT) is possibly becoming an option to tendon grafts for MPFL-R. Patella-side fixation of FT can be performed with suture anchors or via soft-tissue fixation. The aim of this study was to investigate patellar tracking considering soft-tissue-based and anchor-based patella-side fixation techniques.
METHODS: In eight fresh-frozen human knee joint specimens (m/f 4/4; age 75 ± 10 years), the MPFL w

PURPOSE: Conventional press-fit technique for anterior cruciate ligament reconstruction (ACLR) is performed with extraction drilling of the femoral bone tunnel and manual shaping of the patellar bone plug. However, the disadvantages of this technique include variation in bone plug size and, thus, the strength of the press-fit fixation, bone loss with debris distribution within the knee joint, potential heat necrosis, and metal wear debris due to abrasion of the guide wire. To overcome these disadvantages, a novel technique involving punching of the femoral bone tunnel and standardized compress

BACKGROUND: Anterior cruciate ligament (ACL) reconstruction is usually performed with autologous bone-patellar tendon-bone (PT) or hamstring tendon (HT) graft. There has been only 1 randomized clinical trial examining ACL reconstruction with these grafts specifically in soccer players, and more studies comparing these graft types within a homogenous cohort such as soccer athletes may better highlight differences in outcomes.
PURPOSE: To compare the results of ACL reconstruction with PT versus HT autograft in soccer players and to evaluate objective and subjective outcomes.

BACKGROUND: Extensor apparatus rupture is a severe complication after knee arthroplasty, but there have not been many reports on how to perform knee arthroplasty after chronic patellar ligament rupture. We reported a case of total knee arthroplasty (TKA) in a patient with severe osteoarthritis (OA) complicated by chronic patellar ligament rupture.
CASE PRESENTATION: In this case, a 67-year-old male patient suffered from patellar ligament rupture due to trauma more than 20 years ago and did not undergo any formal treatment. Physical examination revealed a small amount of fluid and extension la…

UNLABELLED: Anterior cruciate ligament (ACL) reconstruction can be performed with an autograft, which is most often harvested from the patient's hamstring tendon (HT) or patellar tendon (PT). However, autograft harvesting leads to morbidity that is by no means insignificant. A systematic review of literature was performed to define the incidence of complications related to graft harvesting and the methods to prevent these complications. In March 2017, a systemic review of literature was performed using the keywords"harvesting", "harvest", "morbidity", "complication", "cruciate ligament". No ti

Tensile failure of a patella after anterior cruciate ligament (ACL) reconstruction with autogenous patellar tendon graft is an uncommon but serious complication. One factor that may affect post-harvest patella strength is the shape of the bony defect. The effect of patella defect shape on ultimate tensile strength and mode of failure of the extensor mechanism has not been studied using physiologic moments. Twelve matched pair knee specimens were randomly distributed within three groups comparing rectangular versus triangular, rectangular versus circular, and circular versus triangular shaped p

Tibial anteroposterior displacement after anterior cruciate ligament reconstruction with a patellar tendon graft was followed prospectively for 2 years in 24 patients with an arthrometer. The femoral ligament insertion location, in a lateral projection, and the change in intraarticular fixation distance, measured with an isometer, were documented intraoperatively. Two years after surgery, the overall mean injured-noninjured difference in anteroposterior displacement was 2.0 +/- 2.3 mm. All grafts were fixed during surgery at 20 degrees of knee flexion. Patients for whom this angle coincided wi

BACKGROUND: Patients with recurrent instability underwent isolated medial patellofemoral ligament (MPFL) reconstruction regardless of anatomic risk factors within the confines of strict exclusion criteria. Previous publications from this cohort have been limited in the ability to identify risk factors for failure of isolated MPFL reconstruction when recurrent instability was defined as the only mode of failure, likely because of the small patient numbers who experienced this outcome.
PURPOSE: To investigate whether persistence of postoperative apprehension and J-sign indicate suboptimal outco
