From Imaging to Insight: Sarcopenia as a Marker of Venoarterial Extracorporeal Membrane Oxygenation Mortality in Postcardiotomy Shock.

Identifying risk factors for mortality in veno-arterial extracorporeal membrane oxygenation (VA-ECMO) patients with postcardiotomy shock is challenging due to numerous influencing factors. This study investigates the role of sarcopenia in outcomes for these patients. We retrospectively analyzed 433 patients who underwent ECMO implantation at our institution between 2012 and 2023. Among those with VA-ECMO for postcardiotomy shock, 99 had preoperative computed tomography (CT) scans. We measured the psoas muscle area at the L3-L4 vertebra and the pectoralis muscle area at the level of the left co

Anosognosia for hemiplegia: an electrophysiologic investigation of the feed-forward hypothesis.

The cause of anosognosia for hemiplegia (AHP) remains unclear. Weakness is detected when there is a mismatch between the expectancy of movement and the sensory perception of movement. The feed-forward hypothesis of AHP posits that there is a failure of detection because there is a loss of motor intention and expectancy of movement. We tested motor intention by measuring the activation of proximal muscles (pectoralis majoris) while subjects squeezed a dynamometer with each hand. We tested a group of normal controls, a group of patients with hemiparesis, a patient with neglect, a patient with re

Poland syndrome.

Poland syndrome is characterized by hypoplasia or absence of the breast or nipple, hypoplasia of subcutaneous tissue, absence of the costosternal portion of the pectoralis major muscle, absence of the pectoralis minor muscle, and absence of costal cartilages or ribs 2, 3, and 4 or 3, 4, and 5. The chest wall defect is often associated with a lung hernia. Clinical manifestations are extremely variable and rarely are all the features recognized in 1 individual. Fortunately it is invariably unilateral, allowing for an easier reconstruction. Single-stage reconstruction of the chest wall combined w

Rotation of the clavicular portion of the pectoralis major for soft-tissue coverage of the clavicle. An anatomical study and case report.

BACKGROUND: The purposes of this study were to describe the anatomical features of a rotational flap consisting of the clavicular portion of the pectoralis major and to report the surgical technique and the outcome of use of this flap in a patient with poor soft-tissue coverage following multiple operations for a clavicular fracture complicated by nonunion and infection. METHODS: Ten shoulders from five cadavera were dissected to isolate the clavicular portion of the pectoralis major. The vascular pedicle, thoracoacromial artery, and axillary artery were identified, and the length of the vasc

Suxamethonium fasciculations: a topographical study.

It has been found in a group of 455 gynaecological patients that not all the voluntary muscles of the body respond to suxamethonium in the same way. One group of muscles (e.g. biceps) which is sensitive to suxamethonium, responds by fasciculating in almost every individual. A second, resistant, group of muscles (e.g. pectoralis major) is almost totally unresponsive to suxamethonium. A third muscle group (e.g. triceps) shows fasciculations in about 60% of cases and appears intimately associated with the development of suxamethonium pains, for if fasciculations do occur then suxamethonium pains

Evaluation of catheter loops in central venous port systems.

OBJECTIVES: We sought to evaluate the cause for catheter loops of central venous ports. MATERIAL AND METHODS: A total of 241 patients who received a central venous port via subclavian vein access were included in this retrospective study. Eighty of these patients had undergone a computed tomography of the chest, allowing retrospective evaluation of the course of central venous catheters. Complications were evaluated by review of the clinical data charts. RESULTS: In 49 patients, the catheter penetrated only the major pectoralis muscle. Only one of these patients (2.0%) showed a loop. In 31 c

Subpectoral implantation of the hypoglossal nerve stimulator: An effective technical modification.

Upper airway stimulation is now a well-established treatment option for selected patients with obstructive sleep apnea. The implanted pulse generator of this system activates the hypoglossal nerve and is routinely placed in a subcutaneous pocket overlying the pectoralis muscle. This case report describes a patient with a history of bilateral mastectomy and radiation for breast cancer who required explantation due to device exposure and infection. The patient was successfully reimplanted by placing the implantable pulse generator deep to the pectoralis major muscle. Clinical circumstances invol

[Poland syndrome: about a case and review of the literature].

Poland syndrome is a rare congenital malformation associated with various degrees of thoracic and homolateral upper limb abnormalities. We report the case of a 7-year old girl who underwent exploration for depression of the left hemithorax associated with homolateral subclavicular mass. CT scan showed that the deformation of the thoracic wall was related to the absence of left pectoralis major muscle sterno-costal heads insertion associated with agenesis of the pectoralis minor muscle and hypoplasia of the anterior arches of the first six corresponding ribs. These abnormalities were related to

Pectoralis Major Injury During Basic Airborne Training.

Injuries involving rupture of the pectoralis major are relatively rare. When they do occur, it is mostly frequently in a young, athletic man. The most common cause is weight lifting that results in eccentric muscle contraction (muscle contraction against an overbearing force, leading to muscle lengthening)-specifically, the bench press. Other mechanisms for this injury include forceful abduction and external rotation of the arm. Injury can occur anywhere along the pectoralis major from its medial origin on the sternum and clavicle to its lateral tendinous insertion on the humerus. At the time

Recurrent neurogenic thoracic outlet syndrome stressing the importance of pectoralis minor syndrome.

BACKGROUND: Although first described over 60 years ago, neurogenic pectoralis minor syndrome (NPMS) has only recently been noted to be present in over half the patients with a clinical diagnosis of neurogenic thoracic outlet syndrome (NTOS). Similarly, NPMS has also been observed in the majority of patients with recurrent NTOS. This study explores the role of pectoralis minor tenotomy (PMT) in patients with recurrent NTOS. METHODS: A total of 86 patients with symptoms of recurrent NTOS, who had failed to improve on conservative management, were evaluated by history and physical examination fo

Use of sonography in thoracic outlet syndrome due to a dystonic pectoralis minor.

OBJECTIVE: For patients with thoracic outlet syndrome (TOS), it is important to determine the location of the neurovascular compression to achieve effective intervention. METHODS: The diagnostic workup for a 39-year-old man with TOS included a selective anesthetic block of the pectoralis minor muscle and duplex sonography before and after the block. RESULTS: The subclavian artery peak systolic flow velocity decreased after the block from 208 to 63 cm/s when the arm was in the abduction and external rotation position, indicating a reduction in the severity of focal arterial compression. Also,

Pectoralis Major Tendon Tears During Airborne Operations: Are These Injuries Isolated?

INTRODUCTION: Pectoralis major tendon tears are an injury pattern often treated in military populations. Although the majority of pectoralis major tendon tears occur during eccentric loading as in bench press weightlifting, military service members may also experience this injury from a blunt injury and traction force produced by static line entanglement during airborne operations. Although these injuries rarely occur in isolation, associated injury patterns have not been investigated previously. MATERIALS AND METHODS: After obtaining institutional review board approval, medical records were

Denaturation of myofibrillar proteins from chickens as affected by pH, temperature, and adenosine triphosphate concentration.

The susceptibility to denaturation of myofibrillar protein from chicken muscles was investigated and compared with denaturation of myofibrillar protein from pork. Immediately postmortem, the Pectoralis profundus (white muscle) and the Pubo-ishio femorale (red muscle) of six Arbor Acres chickens were collected. The Semimembranosus (white muscle) and Psoas major (red muscle) of three Yorkshire x Landrace and three Yorkshire x Landrace x Duroc pigs were collected at 45 min postmortem. Protein denaturation was prevented by keeping the muscles at 0 to 2 C in a buffer (pH 7.2) containing ethylene gl

Neck pain in rounded shoulder posture: Clinico-radiologic correlation by shear wave elastography.

AIM: The frequency of neck pain increases in adolescents and young adults. Using smart phones and computers for a long time causes posture disorders such as rounded shoulder (RSP) and forward head postures, and these posture disorders cause neck pain. The aim of the study is to analyse the correlation between shoulder and neck angles and the stiffness of the upper trapezius and pectoralis minor muscles in patients with RSP. METHOD: Patients with neck pain examined by shear wave elastography were screened retrospectively. Patients with RSP were determined. Of these patients, whose had evaluate

Increased Expression of Lipid Metabolism Genes in Early Stages of Wooden Breast Links Myopathy of Broilers to Metabolic Syndrome in Humans.

Wooden breast is a muscle disorder affecting modern commercial broiler chickens that causes a palpably firm pectoralis major muscle and severe reduction in meat quality. Most studies have focused on advanced stages of wooden breast apparent at market age, resulting in limited insights into the etiology and early pathogenesis of the myopathy. Therefore, the objective of this study was to identify early molecular signals in the wooden breast transcriptional cascade by performing gene expression analysis on the pectoralis major muscle of two-week-old birds that may later exhibit the wooden breast
