Determinants for Perinatal Mortality in South China: A Prospective Cohort Study.

OBJECTIVE: To estimate the association of selected maternal and fetal characteristics with the risk of perinatal mortality in South China. METHODS: A prospective cohort study was conducted from March 2013 to December 2019. The exposures of interest were maternal sociodemographic characteristics, lifestyle and habits during early pregnancy, and complications of pregnancy. Their effects on the development of perinatal death were analyzed in our study. RESULTS: A total of 44,048 eligible pregnant women were included in the analysis. Of these, 596 fetuses were perinatal deaths (perinatal mortali

Lack of Change in Perinatal Mortality in the United States, 2014-2016.

Perinatal mortality (late fetal death at 28 weeks or more and early neonatal death under age 7 days) can be an indicator of the quality of health care before, during, and after delivery (1,2). The U.S. perinatal mortality rate based on the date of the last normal menses (LMP) declined 30% from 1990-2011, but was stable from 2011-2013 (1,3). In 2014, National Center for Health Statistics (NCHS) transitioned to the use of the obstetric estimate of gestational age (OE), introducing a discontinuity in perinatal measures for earlier years (4,5). This report presents trends in perinatal mortality, a

Perinatal Mortality in a Northwestern Nigerian City: A Wake up Call.

BACKGROUND: In Nigeria, of the over 900,000 children under the age of 5 years that die every year, perinatal mortality is responsible for a little over 20%. Previous reports are largely from the southern part of the country. This is the first report of perinatal data from the northwest of Nigeria. METHODS: A case control study of perinatal deaths in the three major public hospitals in Katsina metropolis was carried out to determine the pattern of perinatal deaths in the metropolis. Data were collected over a 6 week period on maternal socio-demographic, antenatal, and delivery variables. Data

Are maternal and fetal parameters related to perinatal mortality in HELLP syndrome?

PURPOSE: We designed this retrospective study to evaluate the association between maternal and fetal parameters and perinatal mortality in hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome. METHODS: In this retrospective study, 152 pregnancies complicated with HELLP syndrome were evaluated. Risk factors recorded were maternal age, gravidity, gestational age (weeks), systolic and diastolic blood pressure, platelet count, alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase (LDH), fetal weight, and Apgar scores. The association of thes

Is single umbilical artery an independent risk factor for perinatal mortality?

OBJECTIVE: To evaluate perinatal outcome of fetuses with isolated single umbilical artery (SUA), and specifically to examine whether an isolated SUA is an independent risk factor for perinatal mortality. METHODS: A population-based study was conducted, comparing pregnancies of women with and without SUA. Deliveries occurred between the years 1988-2006, in a tertiary medical center. Multiple gestations, chromosomal abnormalities and malformations were excluded from the analysis. Stratified analysis was performed using multiple logistic regression models to evaluate the association between SUA

Lesions in the central nervous system associated with perinatal lamb mortality.

OBJECTIVE: To identify and describe the occurrence of neurological lesions that could have an effect on lamb mortality. PROCEDURE: The central nervous system was investigated macroscopically (n = 92) and microscopically (n = 72) in lambs dying in the perinatal period during 3 years in flocks of adult Corriedale ewes. The central nervous system was removed intact and samples of cerebral cortex, basal ganglia, thalamus, hippocampus, mesencephalon, cerebellar cortex, medulla oblongata, and cervical spinal cord were scored microscopically for the severity of neuronal dead, cytotoxic and perivascu

[No correlation between rates of caesarean section and perinatal mortality in Iceland].

INTRODUCTION: Caesarean section rates have increased over the past decades without a concomitant decrease in perinatal mortality. In Iceland the same trend has been seen while at the same time perinatal mortality rate has remained low. Most caesarean sections are done at term. Crude perinatal mortality rates give limited information about whether the increase in section rates leads to a lower perinatal death rate among term non-malformed singleton infants. The relation between caesarean section and perinatal mortality rates in singleton, non-malformed infants of birthweight > or =2500 g in Ice

Mortality risk associated with perinatal drug and alcohol use in California.

OBJECTIVE: To analyze the relationship between perinatal drug/alcohol use and maternal, fetal, neonatal, and postneonatal mortality. STUDY DESIGN: Linked California discharge, birth and death certificate data from 1991-1998 were used to identify drug/alcohol-diagnosed births. Mortality relative risk (RR) ratios were calculated and logistic models were generated for mortality outcomes. RESULTS: Among 4,536,701 birth records, 1.20% contained drug/alcohol discharge diagnostic codes (n=54,290). The unadjusted RRs for maternal (RR=2.7), fetal (RR=1.3), neonatal (RR=2.4), and postneonatal (RR=4.3)

[Perinatal morbidity and mortality in children born to mothers with gestational hypertension].

Gestational hypertension is a severe pathology leading to important maternal and neonatal effects. It represents one of the most important causes of maternal morbidity and contributes to a high percentage of perinatal mortality, determined by fetal hypoxia and especially by prematurity and low birth weight. To-date the only treatment offered in gestational hypertension remains delivery, which has to be scheduled regarding timing and method on the basis of the appearance of hypertension, of its severeness and maternal and fetal complications. To evaluate the clinical course and the presence of

Population density and perinatal mortality in Norway and Sweden 1975-1988.

Data from the medical birth registries in Norway and Sweden were used to study geographical variations in perinatal mortality in the two countries. The study population comprised 1.4 million single births during 1975-79 and 1985-88. Perinatal death (n=9,834) for infants with a birthweight of 1,000 g or more was the main measure of outcome. In both periods and in both countries, perinatal mortality was inversely associated with population density and annual number of births, with relative risks amounting to 1.4 in Norway and 1.2 in Sweden. Among live births, these gradients were observed in dea

The effect of placental malaria infection on perinatal mortality in rural Malawi.

Perinatal deaths (fetal or infant deaths from the 28th week of pregnancy up to the seventh day after birth) occur as a result of adverse conditions during pregnancy, labor, and delivery, or in the first few days of life. Placental malaria infection is known to increase the risk of delivery of a low birth weight infant, thus, potentially increasing the risk of perinatal and infant mortality. To better understand the relationship among the adverse events in pregnancy, including placental malaria infection, adverse conditions in labor, and birth weight to perinatal mortality, we investigated the

[Perinatal mortality and preeclampsia/eclampsia: influence of HELLP syndrome on the primigravida].

Perinatal losses have been studied in 37 pre-eclamptic/eclamptic patients: 13 of them exhibited a HELLP syndrome. Only 23 newborns survived (62.5%); after deduction of 4 pregnancies of less than 28 weeks, the perinatal mortality was 27%. Comparison with the local perinatal mortality showed a 6-fold higher risk for the fetus when pregnancy was complicated by pre-eclampsia/eclampsia, regardless of a superimposed HELLP syndrome. For reasons beyond our understanding the fetal losses with HELLP syndrome are much higher in primigravidae.

The sonographically thick placenta: a predictor of increased perinatal morbidity and mortality.

In order to determine the prevalence and significance of sonographically thick placentas, we reviewed the computerized records of 18 827 viable, singleton pregnancies. Of these, 116 (0.6%) had thick placentas diagnosed by ultrasound examination. Perinatal mortality was markedly increased among pregnancies with thick placentas (odds ratio = 13.1, 95% confidence limits (CL) = 8.3-20.8), accounting for 6.2%; of the total. The rates of abruptio placentae (odds ratio = 2.9, CL = 1.1-8.1), neonatal intensive care unit admissions (odds ratio = 4.6, CL = 3.1-6.9) and anomalies (odds ratio = 8.4, CL =

Risk determinants of perinatal mortality in a Swedish county, 1980-1984.

A retrospective case-referent study was performed to analyse risk factors of perinatal mortality. Overweight defined by body mass index, smoking greater than 10 cigs/day, infertility, earlier birth weight, earlier perinatal death were significant risk determinants of the obstetric history. Overweight and smoking, measured by the etiologic fractions, had the greatest impact on perinatal mortality. Significant risk factors of the present pregnancy were low weight gain, pre-eclampsia, cervical incompetence, and late hemorrhage. Intra-uterine growth retardation had the highest etiologic fraction.

Influence of antenatal screening on perinatal mortality caused by syphilis in Swaziland.

In a survey of 283 deliveries in Swaziland, active syphilis (positive results in the Treponema pallidum haemagglutination assay (TPHA) and the rapid plasma reagin (RPR) test) was found in 37 (13.1%) and possibly active infection (positive TPHA but negative RPR test results) in a further 87 (30.7%). The perinatal mortality of untreated mothers with active disease was 21.9% (7/32). The RPR test carried out antenatally by nurses had a sensitivity of 36% (13/36) and predictive accuracy of 48% (13/27). Awareness of this incidence of syphilis led to improved antenatal clinic measures and the prophyl

Perinatal mortality in twin pregnancy. A review of 799 twin pregnancies.

There were 799 deliveries of twins at Harare Hospital during 1983. Of the 1598 infants 155 died, giving a perinatal mortality rate for twin pregnancy of 97/1,000, 3 times higher than the rate for singleton pregnancies. The most common cause of death was immaturity. The perinatal mortality rate was higher for second twins than for first twins. Factors important in decreasing perinatal mortality in twin pregnancy are discussed with reference to the literature.
