Late Choroidal Effusion after Phacoemulsification in Eyes with Previous Trabeculectomy.

AIM: To present 5 cases of late choroidal detachment occurring spontaneously in pseudophakic glaucomatous eyes with previous trabeculectomy before cataract surgery. We discuss the causes, risk factors, frequency, diagnostic methods, differential diagnosis, and treatment of this disease. MATERIALS AND METHODS: In the presented 5 cases the choroidal effusion is proved by ophthalmoscopy and echography. Late choroidal detachment occurs after phacoemulsification 6 months at the earliest and 9 years at the latest (mean period 2.5 years) in glaucomatous eyes with previous trabeculectomy. To prove ch…

Macular cytomegalovirus retinitis following dexamethasone intravitreal implant combined with phacoemulsification.

A 60-year-old diabetic patient, who had undergone a renal transplant 2 years earlier, presented with sudden decrease in vision in his left eye (LE). He had undergone phacoemulsification combined with intravitreal dexamethasone implant injection in his LE 2 months earlier, for coexistent cataract and diabetic macular edema. Examination revealed necrotizing retinitis with hemorrhages in the macula. A diagnosis of cytomegalovirus retinitis was made, which was confirmed on vitreous polymerase chain reaction. Intravitreal and systemic ganciclovir led to the resolution of retinitis and improvement o…

Annular Salzmann degeneration: Avoiding perturbations and pitfalls in phacoemulsification surgery.

UNLABELLED: We highlight potential problems and pitfalls in cases of Salzmann nodular degeneration by reporting the case of an 84-year-old man with reduced visual acuity due to particularly extensive bilateral Salzmann nodules and moderate cataracts. Large annular Salzmann lesions were removed by superficial keratectomy in each eye. The reproducibility and accuracy of keratometry and biometry improved significantly, and uneventful phacoemulsification cataract surgery was performed 4 weeks after the keratectomy. The phacoemulsification procedures were routine other than the development of prono…

Corneal endothelial cell loss after phacoemulsification using nuclear cracking procedures.

This study assessed corneal endothelial damage in endocapsular phacoemulsification surgery using a nuclear cracking procedure in which the nucleus was divided into four quadrants and emulsified. Forty-five eyes had phacoemulsification using this technique, and 33 eyes had phacoemulsification using undivided sculpting techniques (without cracking). We examined the extent of endothelial cell loss at one and three months after surgery using specular microscopy. We also measured ultrasound time, time spent for phacoemulsification, and infusion volume during surgery. Endothelial cell loss was signi…
